DescriptionAt St. Peter's, surgeons operate in the area's newest and best surgical suites, featuring the latest diagnostic and surgical technology. It's why more than 15,000 surgeries are performed at St. Peter's each year.Supported by a nationally-recognized nursing staff and an expert support staff, surgeons at St. Peter's have the tools and the advanced technology to transform and save lives.Minimally invasive procedures, including robotic, computer-assisted and image-guided surgeries, are performed in the areas of cardiac, vascular, gynecologic, neurological, orthopedic, otolaryngologic, thoracic, urologic and general surgery. Types of procedures being performed at St. Peter's Hospital:- Bariatric Surgery - Breast Surgery - Cardiac Surgery - Colorectal Surgery - Dental/Maxillofacial Surgery - General Surgery - Gynecologic Oncology - Gynecological Surgery - Hepatobiliary (Liver, Biliary, Pancreas) - Minimally-invasive Surgery - Neurosurgery - Orthopaedic Surgery - Otolaryngology Surgery - Pediatric Surgery - Plastic and Reconstructive Surgery - Surgical Oncology/Cancer - Thoracic Surgery - Urological Surgery - Vascular Surgery Robotic Surgery: The da Vinci Surgical SystemThe surgeon's surgical capabilities are enhanced by the da Vinci surgical system, a totally "intuitive" laparoscopic surgical robot. The system is so precise, the instruments recheck their position by computer 1,500 times per second. St. Peter's gynecologic oncologists were the first in New York to use the state-of-the-art da Vinci surgical system. This new system integrates robotics and extremely sophisticated computer technology with the skills of the surgeon. It allows the surgeon to perform minimally invasive procedures in a manner never experienced before. Robotic-assisted laparoscopy combines dexterity and precision with advanced 3-D imaging to overcome the surgical limitations of a standard laparoscopy. The new technology gives surgeons precise control and visualization, bringing a whole new dimension and flexibility to surgery. The system's camera gives the surgeon a highly detailed, three-dimensional view of the patient's body. Advanced imaging allows much greater precision, thereby allowing for small incisions, more exact placement of sutures and more exact use of the operating instruments. For most patients, robotic surgery offers several benefits, such as:- Shorter hospital stay- Less pain- Less risk of infection- Less blood loss and transfusions- Less scarring- Faster recovery- Quicker return to normal activitiesIn addition, it is possible to return to regular activities in one to two weeks, instead of the traditional six weeks that may be required for a complete recovery.Surgical Services: In 2010, St. Peter's Hospital's operating room serviced 6,942 inpatient surgical cases (Orthopedic - 2,232, General Surgery - 848, Neurological - 1,043, Thoracic - 598, Gyn & Gyn Oncology - 394, Gen Colorectal - 380 and various others). The operating room also serviced 8,129 outpatient cases (General Surgery - 2,556, Ear Nose and Throat - 1,149, Orthopedic - 566, Urology Closed - 689, Gyn & Gyn Oncology - 1,396, Neurological - 520 and other cases).Description

DescriptionA nationally recognized, Top 100 Cardiovascular Hospital, St. Peter's Cardiac & Vascular Center is led by a premier team of cardiologists, cardiac surgeons, and vascular specialists. This world-class facility is a leader in the Capital Region and one of the most active centers in the state of New York for the diagnosis, treatment and rehabilitation of persons with cardiac and vascular disease. The Cardiac and Vascular Center's new cardiovascular surgery facilities are among the nation's most advanced surgical environments for surgery and post-surgical care of patients with complex heart and vascular disease. Among the innovative technology in the facility is a unique "true Hybrid Operating Room" - the ArtisZeego multi-access system for surgical angiography. Upon its completion, this new operating room system was reported by its manufacturer to be one of only 29 such installations worldwide.St. Peter's nationally recognized surgical team and state-of-the-art operating rooms are the perfect combination, and our integrated team approach helps patients achieve the best possible outcomes. We are proud to offer northeastern New York and west central New England the latest innovations in cardiac and vascular care, including our Heart Rhythm Center. At St. Peter's, we pride ourselves in being the only regional cardiac center to offer a full spectrum of individualized services including individualized rehabilitation programs, home care and long-term care.Recognition of ExcellenceSince opening in 1985, St. Peter's Cardiac & Vascular Center has been recognized for excellence by health and heart care organizations across the country. Named a Top 100 Cardiovascular Hospital for 10 of the last 12 years, St. Peter's is the only ten-time recipient of this award in the northeastern U.S. Such accomplishments underscore our commitment to creating a culture of excellence at St. Peter's. Some statistics: The cardiac Cath lab performed over 2501 cases in 2010. We performed 1,753 diagnostic procedures and approximately 748 PCI's. We also expanded our Transradial Cardiac Catheterization. Cardiac Catheterization is a procedure which enables physicians to assess the coronary arteries (the vessels supplying blood to the heart muscle). Coronary angioplasty is a procedure in which a clogged artery is opened. Both procedures, which have traditionally been performed via catheters placed through the femoral artery in the groin can now be done through the radial artery in the wrist. We have performed 748 Transradial Cardiac Catheterizations in 2010. We were designated a regional training hospital for this procedure and have trained physicians and staff from the east coast. Both our catheterization and EP program serve the Capital District and a large geographical area in upstate New York, Vermont and Massachusetts. We continue to be a mentor hospital to the surrounding counties. Our EP program is the largest program in upstate New York. We continue to see double-digit volume increase per year. Nationally, Electrophysiology studies are expected to grow between 15-18% per year. Over 1182 invasive and non invasive electrophysiology procedures were done in 2010. We continue to be the largest cardio vascular program in Northeastern New York. We performed over 638 cardiac surgeries in 2010. We are the only healthcare facility that performs minimally invasive heart surgery. We are a training site for minimally invasive surgery hosting such facilities as Brigham and Woman's Hospital, Boston. We are in a position to be on the forefront of minimally invasive valve replacement in the country. Our continuum of programs includes a cardiac rehabilitation and wellness program. This program provides physical rehabilitation, functional assessment, education and complementary healing for patients at risk or recovering from cardiac events. Over 12,922 sessions are done annually with patients. In addition, nearly 50 community programs are conducted which include education, screenings, health fairs and wellness events.Description

ExplanationExplanationIdentifierIdentifierReturn ReferenceReturnReference
Part I, Line 6a: An annual community benefit report is required by New York State. There are 3 links on our website that the Public may refer to for the Community Benefit Report. A Comprehensive Community Service Plan (CSP) is due and submitted every three years. A CSP Update is due and submitted annually. St. Peter's is also required annually to provide a Community Benefit Report to Catholic Health East.
Part I, Line 7: Charity care is free or discounted health services provided to persons who cannot afford to pay and who meet the organizations financial assistance policy criteria. Charity care is reported in terms of costs, not charges. Community benefits are programs or activities that provide treatment or promote health and healing as a response to identified community needs and meet at least one of these objectives:-Improve access to health care service-Enhance the health of the community-Advance medical or health care knowledge-Relieve or reduce the burden of government or other community efforts. The amounts reported in the Line 7 table were calculated using a cost-to-charge ratio determine in accordance with Worksheet 2.
Part I, Line 7g: Subsidized health services are clinical programs that are provided despite a financial loss so significant that negative margins remain after removing the effects of charity care, bad debt and Medicaid shortfalls. Programs included in subsidized health services ($1,239,494) are part of the St. Peter's Ambulatory Care Network:-St. Peter's Rensselaer Health Center Medical Services-St. Peter's Rensselaer Health Center Dental Services-St. Peter's Family Health Center OB/GYN Services-St. Peter's Family Health Center Pediatric Services-St. Peter's Family Health Center Medical Services-St. Peter's Dental ServicesThe roots of the Ambulatory Care Network are in the St. Peter's Family Health Center. The South End community was unable to access primary/preventive health services and requested St. Peter's to invest in the people who were poor, uninsured and could not find a medical home in the private practice of physicians. There are no health centers in the South End of the City of Albany or in the City of Rensselaer that cares for poor, uninsured populations. The population of the City of Rensselaer, also poor and underserved, has frequented the Rensselaer Health Center since its founding in 1993. In 2007, a new clinic was built on Columbia Turnpike to serve this population with primary/preventive medical care and dental services that are not available in the city from other providers. The Ambulatory Care Network has extensive relationships with community agencies and relationships with referring organizations. These relationships are based in mutual education about health issues and community resources. Outreach to these agencies and organizations helps to enhance the capacity of the community to both refer and access services. St. Peter's Dental Health Services has provided high-quality, cost-effective, comprehensive dental health care to the Capital Region for more than 20 years. St. Peter's Hospital operates two dental clinics, a Ronald McDonald Care Mobile serving inner-city children, and a fully accredited General Practice Dental Residency Program. The programs serve a diverse client population, welcoming all regardless of needs or ability to pay. Our clinics experience a combined volume of more than 20,000 visits annually. Our clients include children, the elderly, and people with physical or mental special needs. Through extended office hours, a mobile dental van, and cooperative relationships with our Emergency Department, our two nursing homes, and the Center for Disability Services, we provide access to our community's most vulnerable residents. While our region continues to be tragically underserved in terms of dental services for patients enrolled in public health insurance programs, the St. Peter's Hospital Dental Program has made great strides in increasing access to care for these individuals.
Part I, Line 7, Column (f): The Bad Debt expense included on Form 990, Part IX, Line 25, Column (A), but subtracted for purposes of calculating the percentage in this column is $ 24077550.
Part II: Programs listed under Community Building Activities:Leadership Development / Training for Community MembersLeadership Development Specialist participates in the American Society of Training and Development annual conference on Workplace Learning and Performance - local chapter committee preparation. The training promoted the health of the communities we serve by providing skill building training on topics that help people be effective with patients, physicians, and co-workers. These effective relationships then enhance the experience and healing of the patients in our health care settings. The topics included communication, team work, leadership skills, dealing with different generations, and conflict resolution.Asthma CoalitionSt. Peter's Staff participates on a steering committee on a monthly basis to coordinate community activities related to Asthma for the Underinsured and Uninsured children of the capital region.HCDI - Healthy Capital District Initiative Board The Healthy Capital District Initiative (HCDI) is a not-for-profit that has helped over 35,000 needy children and adults in the capital region get health services that they might not have accessed otherwise. Healthy Capital District Initiative (HCDI) represents an ongoing partnership effort to develop a strategic action work plan designed to improve the health of the residents of the Albany, Schenectady and Rensselaer Counties. HCDI was undertaken in the spirit of Communities Working Together For A Healthier New York, an initiative to use community forums to identify high-priority health concerns.Hope House BoardThe mission of Hope House, Inc. is to respond to needs in the community to provide education, intervention and treatment to persons whose lives have been affected by any form of chemical dependency or family dysfunction. The Board of Directors and staff are dedicated to providing an array of high-quality residential, outpatient and community-based services that are developmentally focused to both adult and youth populations. These services are provided irrespective of color, creed, age, national origin, marital status, disability, arrest/conviction record or ability to pay. All services strive to empower each individual and family to achieve their highest level of functioning. In addition to enhancing the quality of life of the persons it serves, the agency is committed to providing a supportive and professional work environment for its staff.
Part III, Line 4: We calculated the cost of bad debt using the ratio cost to charge (RCC) methodology. We divided the hospital's total expenses by the hospital's gross charges. We applied this ratio to the hospital's bad debt expense (which is comprised of gross charges) to calculate the cost of bad debts. We are taking our bad debt at cost and offsetting the portion of the indigent care pool receipts that relates to bad debts. The organization does not consider discounts properly accorded to patient accounts in its determination of bad debt expense. The portion of bad debts (at cost) attributable to patients who would be eligible for our charity care policy if sufficient information were available to make a determination is estimated. The test of our financial statement footnote describing bad debt expense is:The Hospital provides an allowance of doubtful accounts for estimated losses resulting from the unwillingness of patients to make payment on services. The allowance is determined by analyzing historical data and trends. Accounts receivable are charged off against the allowance fordoubtful accounts when management determines that recovery is unlikelyand the Hospital ceases collection efforts.
Part III, Line 8: The costs are from the 2010 Medicare Cost report using the step down methodology and Medicare cost Accounting regulations.
Part III, Line 9b: In order to preserve the dignity of patients and to facilitate the process of securing necessary information, SPH strongly prefers to perform financial screening upon scheduling, admission or registration (discharge processing in the ED) as part of the overall financial counseling process as described in the CHE Key Performance Indicator System policies 1. SPH refers patients who represent increased financial risk as a result of the amount they are expected to owe "out-of-pocket" to a Financial Counselor for assistance in applying for alternative payment programs, determining Charity Care eligibility, establishing payment plans or other payment arrangements. 2. SPH makes financial counseling services available to all patients.
Part VI, Line 2: St. Peter's participates in collaborative planning through the Healthy Capital District Initiative (HCDI) to assess community health needs. As part of the Healthy Capital District Initiative, St. Peter's collaborated with local public health agencies, community agencies, the business community and other providers to create the 2009 Community Health Profile. HCDI member organizations include Albany County Department of Health, Albany Medical Center, Catholic Charities of the Catholic Diocese of Albany, Capital District Physicians' Health Plan, Ellis Hospital, Fidelis Care, Northeast Health/Samaritan Hospital/Albany Memorial Hospital, Rensselaer County Department of Health, Schenectady County Public Health Services, Senior Whole Health, Seton Health/St. Mary's Hospital, St. Peter's Health Care Services and Whitney M. Young, Jr. Health Services. St Peter's administered a paper and pencil community needs assessment that targeted the five zip code areas that represented communities with the highest Community Need Index scores (12202, 12204, 12206, 12210, 12144). The five zip codes are located in the primary service area of St. Peter's Hospital and are within a five mile radius of the hospital. St. Peter's participates in the Albany County Strategic Alliance for Health initiative intended to combat the concurrent epidemics of cardiovascular disease, diabetes, and obesity. In collaboration with the Healthy Capital District Initiative, St. Peter's participated in the production of a televised Community Health Forum to provide citizen input to the state-mandated Community Health Assessment. The one-hour program featured a studio audience of about 80 public health experts and stakeholders as well as a broad range of consumers from the three counties. Also call-in questions and comments from viewers were broadcast live.
Part VI, Line 3: As members of the Catholic healthcare ministry who are deeply committed to caring for those who are poor, St. Peter's Hospital has established respectful and effective procedures for addressing the needs of those persons who are unable to pay for all or most of their care. Patients who represent increased financial risk as a result of the amount they are expected to owe "out-of-pocket" are referred to a Financial Counselor for assistance in determining Charity Care eligibility. As a not-for-profit Catholic Healthcare facility St. Peter's has remained consistent in the administration of our charity care program. St. Peter's prides itself on providing free and reduced health care services to those who are in need that live in our community. Annually, St. Peter's budgets an amount for Community Benefit, which includes our financial aid program. The improvements implemented to the Hospital's existing Charity Care policy are simply advising the patients and their families of the availability of our programs. This information is posted throughout the facility, on our user-friendly web site, on all patient-billing statements, in the Ambulatory Care Network clinics and by word-of-mouth.St. Peter's has programs in place to provide comprehensive financial counseling to our patients. Uninsured patients are screened by a financial counselor at the point of registration to identify which type of program they may be eligible. In 2009, St. Peter's invested in software to enable us to improve the up-front financial counseling process to validate patient information upon registration which enables us to assist patients who need it most in real-time without any delay. We have formed a Charity Care workgroup which meets bi-weekly to review Charity Care cases that assist us to continually improve our systems and processes to best serve the needs of the most vulnerable. The greatest challenges regarding St. Peter's Financial Aid Program has always been to encourage the patients to complete the Charity Care application along with providing any documentation that demonstrates the patients' income levels. Ongoing patient education is required in order for us to make a determination on providing free or reduced cost care. More information about our Charity Care program is available at www.sphcs.org/CharityCare.Public notice of the availability of financial assistance is made through each of the following means:- Notices are posted in a visible manner in locations where there is a high volume of inpatient or outpatient admitting/registration, such as emergency departments, billing offices, admitting offices, and hospital outpatient service settings.-Information regarding financial aid (Charity Care) is included on all patient liability statements (bills) and statements sent to patients indicating: a) that if the patient meets certain income requirements that the patient may be eligible for a government sponsored program or for financial assistance from the hospital, and b) a hospital contact name and phone number that patients may call in order to gain information on the hospital's Charity Care, reduced payment, and other financial assistance policies.-Availability of assistance, contact names and phone numbers posted on St. Peter's Hospital web site.-Uninsured patients are provided with a copy of the Charity Care Application and Brochure upon registration.-St. Peter's annually makes available to the public on a reasonable basis the costs of charity care provided and the unreimbursed costs of care provided to beneficiaries of government programs that serve the poor.-Posted notices are in the primary language(s) of the service area and in a manner consistent with all applicable federal and state laws and regulations.
Part VI, Line 4: St. Peter's identifies Albany and Rensselaer Counties as its primary services areas and portions of Schenectady, Saratoga and Columbia Greene Counties for its secondary service areas in planning for the Community Service Plan. The process included review of a comprehensive community needs assessment, current market and demographic data and numerous focus groups of key stakeholders; the sponsors, Board members, physicians and management staff. In addressing the Prevention Agenda, St. Peter's will address the priorities of Access to Quality Health Care and Chronic Disease with a focus on health coverage, dental care, breast cancer, lung cancer, colorectal cancer, diabetes, coronary heart disease and congestive heart failure through our goal to reach out to those most in need in order to demonstrate an improvement in the health of the vulnerable populations in the communities we serve. In collaboration with our community partners, we are committed to: - Increase the percentage of adult New Yorkers with health coverage. - Develop baseline data on the number of applicants referred to private and public sponsored health insurers.- Increase the percentage of adults who see a dentist.- Increase percentage of breast cancer cases diagnosed at an early stage- Reduce the incidence of lung cancer- Reduce the mortality rate for colorectal cancer- Reduce the prevalence of adult diabetes and hospital complications of diabetes - Complete an assessment of current diabetes program at St. Peter's - Reduce coronary heart disease hospitalizations - Expand Choices program scope to demonstrate reduced hospital readmissions for Congestive Heart Failure (CHF)St. Peter's provided Open Medical Staff Grand Rounds in 2010 on the following topics:- Update on Suicide Risk Assessment- Update on Inflammatory Bowel Disease (IBD)- The Use of Stereotactic Radiosurgery (SRS) and Stereotactic Body Radiation Therapy (SBRT) for Trigeminal Neuralgia and Stage I Non-Small Cell Lung Cancer- New Therapies for Non-Small Cell Lung Carcinoma (NSCLC) - Tobacco Cessation for the Overworked Clinician: It's Easier for You Than You Think- Interventional Approaches to Acute Stroke- Treating the Addicted Patient- Medication Safety: 2010 Update- The Spectrum of HPB Surgery- Achieving Glycemic Control in the Hospital Setting- Family Practice Case Discussion - Ethical Question, Does the Patient have Capacity?- Men's Health Literacy: Implications for low male engagement in health care, and poor health outcomes- Non-Cardiac Causes of Elevated Troponin- Urologic Oncology Update- Update in Female Pelvic Surgery- Combating the Diversion of Prescription Controlled Substances- The Final Five: The Ten Most Common Eye Disorders in Primary Care- The Benefits of Holistic Medicine for Your Patient and for You- Antibiotic Stewardship Program: Findings and Guidance- Degos Disease: Solving a Deadly Mystery- Noninvasive Assessment of Coronary Artery Disease - Appropriate Utilization of Testing in The Outpatient Setting: A Guidelines Based Approach- New York State Mandated Infection Control Training Program- Metastatic Colon Cancer- Update in Liver Transplant- Hematuria: Work-up and Causes- Adrenal Incidentalomas- Influenza: Looking Back - The 2009 H1N1 Pandemic and Looking Forward - The 2010/2011 Flu Season- M.O.L.S.T. - Translating Patient Preferences into Medical Orders- The Science of the Mind: Neuroplasticity, Allostasis and Health Outcomes- Family Medicine Ethics Case Discussion- Is There a Link Between Testosterone and Cardiovascular Status?
Part VI, Line 7: 2. As a member of Catholic Health East, our Vision, Mission and Core Values compel us to identify unmet community needs to demonstrably improve the health of vulnerable populations. In 2010, Catholic Health East provided over $230,700 million in community benefit programs and services. But faces speak louder than numbers. Individuals and families whose lives have been touched and changed through interaction with our ministry's community benefit programs and services - are the guiding force to continue to serve our communities. Remaining faithful to extending the mission of person-centered care, especially for those who poor in a changing economic and political environment, CHE strives to expand its participation in the community benefit process with a partnership in assessing the need, planning, implementing and evaluation of the community benefit provided at each Regional Health Corporation (i.e. St. Peter's) Community benefit is grounded in the belief that those living in poverty have a moral priority for services. Programs designed to increase access to health care and to improve the health and lives of low-income and homeless persons should have top priority. Not-for-profit health care also has a responsibility to work to improve health in communities by focusing on programs addressing underlying causes of health problems and collaborating with others to make the community a healthier place to live and work. Those living in poverty have a moral priority for services.
