Form 990, Part III, line 4a:Joslin Clinic: Meeting the Challenge of the Diabetes Epidemic Diabetes affects more than 6.2% of the population in Massachusetts, putting each person at high risk for diabetes complications which are a devastating burden to patients, their families and the healthcare system. Three out of five people with diabetes have at least one complication, such as cardiac disease, kidney disease, circulatory problems, and eye disease. Joslin each year provides up to 25,000 patients (including 51-76% of kids with diabetes in Massachusetts, as well as 23-34% of kids with diabetes in New England) with a comprehensive spectrum of multi-disciplinary care for diabetes and endocrine disorders in both the inpatient and outpatient setting. Over time, high blood glucose from poorly controlled diabetes can have harmful effects on the body, leading to complications such as neuropathy (nerve damage), nephropathy (kidney disease), and retinopathy (eye disease). Despite the advances in treatments for patients with diabetes, the disease is still the leading cause of amputations and preventable vision loss. The impact Joslin has in preventing diabetes complications is second to none. Joslin was founded in 1898 by the diabetes pioneer Dr. Elliott P. Joslin, as the first clinic in the world devoted exclusively to the treatment of people with diabetes. The clinic is today affiliated with the Harvard Medical School and continues to be the world's most respected and most innovative diabetes care facility. Dr. Joslin's belief that the key to managing diabetes lay inpatient involvement, education and empowerment is embodied in today's JoslinCare(TM), a multi-disciplinary program that enables lifelong health in people with diabetes through state-of-the-art medical care, sequenced diabetes education, personalized nutrition and exercise prescriptions, and the aggressive prevention and management of complications. Joslin Clinic has more board-certified physicians who are expert in treating all forms of diabetes and its complications than anywhere in the world. We also have the world's largest staff of Certified Diabetes Educators - nurses, dietitians, exercise physiologists, nurse practitioners and mental health professionals. JoslinCare is provided by this highly specialized and multi-skilled team, expert in all facets of diabetes, including adult and pediatric diabetes, kidney disease, eye care, mental health and pregnancy. The outcomes from the JoslinCare model are proof that with the recommended standard of care and management, complications of diabetes can be prevented or reversed. Joslin Clinic consistently ranks among the best in the care of patients with diabetes as measured by national quality indicator data collected by American Diabetes Association, National Committee on Quality Assurance and numerous private and for-profit health data consortiums, including Bridges to Excellence and U.S. News and World Report's list of the nation's top endocrinology providers. Beetham Eye Institute: Integral to JoslinCare is the delivery of the world's finest diabetes eye care through Joslin's Beetham Eye Institute. Techniques pioneered at the Beetham have preserved the vision of more than nine million people around the globe. Patient Education: Joslin offers an extensive list of group programs that range from diabetes management to pump therapy to weight control to mental health and support groups. We address every aspect of diabetes care. Pediatric Health Services In addition to adult diabetes care, Joslin Clinic has a reputation for pioneering care and support of pediatric patients and their families. Our staff includes: - Board-certified pediatric endocrinologists - Pediatric nurse practitioners - Pediatric diabetes nurse educators - Pediatric nutritionists - Child psychologists and social workers - Child life specialists - Exercise physiologists - Care ambassadors Joslin pediatrics provides the most diabetes patient and family support programs anywhere, including groups for families with very young and school-age children, and for parents of teens with diabetes; online discussion boards for adolescents and their parents; and multiple clinical research studies evaluating the newest approaches to family-based care. In addition, Joslin has one of the largest and most experienced pediatric insulin pump programs in the world. Joslin has a unique protocol for introducing insulin pump therapy for kids of all ages. JumpStart Program When a child or teen is diagnosed with diabetes, the whole family has a great deal to learn in a short time. JumpStart is a Joslin program designed to make the process of fitting diabetes into family life more manageable. The goal is to make sure every child with type 1 or type 2 diabetes gets off on the right track. From research we know that young people who learn good management skills early on are more likely to enjoy a lifetime of better health. We assign children a team of professionals to help them learn how to manage their diabetes. This includes a physician, nurse, dietitian and a care ambassador, who is in regular contact. In addition to family support, JumpStart provides financial assistance to patients who do not have insurance, or who may experienced a lapse in coverage. A child or teen (age 18 or under) who has had diabetes for less than two years is eligible for this program. Also eligible are children 18 or under who have had diabetes longer than two years, but had not previously received care at Joslin or who have financial need. This program is only available at Joslin in Boston. The JumpStart program includes: - Care by our pediatric endocrine specialists - Education by certified diabetes educators - Services of a care ambassador to help the entire family with any diabetes-related issues - Routine laboratory work - Round-the-clock access to the healthcare team - Regular communication with the child's primary care provider - Access to our health services, including eye, kidney and mental health specialists - Special programs, including the Young Children and the School Age Children programs A outcomes study by researchers at Joslin showed that adding a case manager - or a "care ambassador" - to keep in touch with the child and the family between medical visits, to help coordinate the child's diabetes care and to provide additional educational services, can make a significant difference in helping the child and the family control blood glucose levels. Those studied also came for more clinic appointments, had fewer episodes of severe hypoglycemia (low blood glucose) and fewer emergency room visits or hospitalizations. The care ambassador coordinates efforts with the Joslin healthcare team - including the doctor, nurse, dietitian and mental health professional - to make sure the child and the family are getting the help and support they need. Since its origin, approximately 1,838 patients have come through the program. More than 250 needy families have benefited from JumpStart's financial assistance arm, which ensures their children receive life-saving diabetes care at little or no cost. Joslin Diabetes Education Program for School Nurses In conjunction with the American Diabetes Association, Joslin Diabetes Center has designed a one-day program to update school nurses. School nurses who participate receive continuing education credit. The program includes information on the following: - Etiology and pathophysiology of type 1 diabetes and type 2 diabetes. - Current diabetes treatment plans, including a discussion about insulin pump therapy and the latest technologies in blood glucose monitoring and insulin delivery. - School planning: diabetes health care plans, 504 forms and IEPs. - Student meal plans, along with a discussion about obesity, physical activity and nutrition in schools. - "It takes a village" concept: The role of school nurses in helping families live with diabetes. Multi-Cultural Health Services As part of our clinical services, we reach out to communities who are at risk for developing diabetes. African Americans, Latinos, Asian Americans and Native Americans develop diabetes at much higher rates than the general population. Learning to manage a disease like diabetes is hard enough. But if your primary language is not English or you come from a different culture, you face even greater hurdles to getting good care.Community Benefit Statement
Through two initiatives - Joslin's Latino Diabetes Initiative and Joslin's Asian American Diabetes Initiative - we offer culturally-specific diabetes care staffed by bilingual healthcare providers. By providing services and resources in a patient's native language and cultural context, we aim to build trust, confidence and self-care skills. We also offer services to the African American community. Latino Diabetes Initiative Latinos are the fastest growing and largest minority group in the United States. Their risk of developing diabetes is one and a half times greater than non-Latino white Americans. They also have a greater chance of developing complications. Joslin's Latino Diabetes Initiative is designed to improve health outcomes in adult Latinos with diabetes or at risk for the disease through culturally and linguistically appropriate clinical and educational services. The Initiative consists of four main components: 1) patient care and education; 2) community outreach; 3) provider education; and 4) clinical research. Through Joslin's Latino Clinic, a multi-disciplinary clinical and administrative team offers services in Spanish to Latino patients. We provide one-on-one appointments, group education classes, support groups and a diabetes and pregnancy clinic. Activities include adaptation, translation and development of patient education curricula and materials, the implementation of group classes in Spanish, as well as a weekly support group and a Latino Diabetes Web site. The Initiative developed a unique patient tool specifically for low health literacy patients. Rosa's Story is an audio-CD with an accompanying patient handbook that presents the story of Rosa, a 53-year-old Latina with diabetes. In addition to teaching the basic aspects of diabetes care and prevention, Rosa's Story also aims to address common myths and perceptions about diabetes within the Latino population that may affect diabetes care. Asian American Diabetes Initiative Diabetes disproportionately affects Asian Americans who are two times more likely to develop the disease than Caucasians. The Asian American Diabetes Initiative was established in 2000 by Joslin Chief Scientific Officer George L. King, M.D., and friends of Joslin Diabetes Center in recognition of the growing challenge of diabetes in Asian Americans. The Initiative's goals are: - To study diabetes in the Asian American population and disseminate Joslin's research findings to healthcare providers and Asian American communities. - To increase diabetes awareness through innovative and culturally appropriate educational materials. - To design and implement clinical treatment programs for Asian Americans. Joslin's Asian Clinic integrates Joslin Clinic's exceptional JoslinCare patient care and education approach with a focus on nutrition therapy. Joslin's Asian Clinic provides one-on-one appointments with physicians and a dietitian/diabetes educator who are culturally sensitive to the Asian American population. We offer diabetes care through a team of endocrinologists, an ophthalmologist, an optometrist, a registered dietitian, a care ambassador and a medical assistant. The care ambassador serves as a coordinator who makes patient appointments, handles prescriptions and serves as a liaison for patients with Joslin Clinic. Our care providers and coordinator speak Chinese (Cantonese and Mandarin), Vietnamese and Hindi. Charity Care Joslin Clinic is not an inpatient institution and does not participate in nor does it receive reimbursement from the Commonwealth's free care pool. Despite that, Joslin Clinic offers free care to any patient in need who meets financial qualifications. Many patients without financial means come to Joslin Clinic, a number that continues to increase, particularly in these difficult economic times. The chart below illustrates the number of visits by persons who either received Medicaid or free care in the past three years. # of Visits 2008 2009 2010 Internal Medicine 1,138 1,462 1,365 Pediatrics 618 795 631 Ophthalmology 1,133 1,342 1,334 Mental Health 126 254 348 Nephrology 120 161 172 Education Services 478 909 750 Donors make Joslin's quality of work possible. Both lines of work - research and patient care - create financial deficits for Joslin. Charitable contributions support operations not covered by grants and fees. Community Outreach Program Joslin Clinic has a robust Community Outreach Program that provides culturally appropriate and bilingual diabetes education and screening to Boston's diverse ethnic communities through our Latino Diabetes Initiative and the Asian American Diabetes Initiative. Staff from the Clinic are also working with the Mayor's Office to develop programs to reduce and prevent obesity in the city's schools.
