Description	People come to the Mustard Seed from a variety of situations. They may be starting over due to the death of a family member, domestic violence situation, or fire. Some are leaving a homeless shelter. They may be unemployed, underemployed, or unable to work due to a serious illness or disability. In 2010, we served a total of 2,124 individuals, including 584 children. The Mustard Seed does this through a simple referral process that utilizes local social-service, and faith-based agencies, churches, and schools. They provide the Mustard Seed with a written referral of the client's specific needs, which we verify. Clients come to the Mustard Seed for an appointment, at which they complete a brief application and choose their furnishings. Depending on their referral, clients can choose up to four large items (sofa, table and chairs, dresser, etc.). In addition, they receive free bed and kitchen linens and kitchenware.	Description

Explanation	The Mustard Seed Furniture Bank provides household furniture and linens to families and individuals who have experienced a catastrophic situation. Material Donations: The agency accepts donations both at their facility and through picking up donations directly from our donors homes/businesses. The staff offers each material donor the opportunity to have a written receipt for their donation. Some donors do not wish to receive a receipt. Material donations are prepared for distribution back to our clients by the staff of the Mustard Seed. Clients to our agency are referred following a home visit by a social-service agency, faith-based organization, church and/or school. A written referral addressing 10 key points is provided to our staff on the referring agency's letterhead. The staff contacts the client to schedule their appointment to select their items. At that appointment, clients are asked to bring their US State Picture ID, a copy of their lease or mortgage statement, $20.00 processing fee, and to have a truck to transport their items to their home. Clients complete a brief application to provide the Mustard Seed with demographic information and also permission for possible other services by area organizations. Clients select their large furniture items and are provided with the linens, towels and basic kitchenware that are documented on their referral letter. The staff of the Mustard Seed utilizes a "pick list" so that the warehouse staff knows what the client and their family is in need of on their visit to the agency. The information is then placed into an excel document for reporting purposes. Monetary Grants: An excel spreadsheet has been prepared to log in when a grant has been applied for; then after notification information is transferred to one of two areas - grants awarded or grants denied. two areas - grants awarded or grants denied. Deposits of Financial Contributions: All financial contributions to the agency by a business or individual are documented on the deposit slip and included in a detailed description of each bank deposit.
