Committed to an outstanding patient experience for allSouth Jersey is mix of vibrant, diverse communities, each with unique needs, yet one common concern when it comes to their health: access to excellent care. Virtua -West Jersey Health System's broad network of facilities is filled with people who share a passion for ensuring that every resident has access to exceptional care, regardless of ability to pay. The employees of Virtua hospitals, wellness centers, nursing homes, rehabilitation centers, and more, reach out in countless ways to educate, nurture and care for the communities they serve. In every neighborhood, we have witnessed the lingering uncertainty of families missing an income or lacking insurance. We have been touched by their stories and inspired to go beyond the support expected of a trusted healthcare partner. In addition to free healthcare for the uninsured, our comprehensive community benefit program provides extensive health education, health screenings, wellness events, community outreach and other financial support. To have the greatest possible impact, we plan our support based on the needs identified by the health departments of the counties we serve. These needs are well-aligned with our expertise, and we've been able to significantly improve patient wellness in areas such as obstetrics and gynecology, pediatrics, geriatrics, cardiovascular health, oncology and diabetes. We keep pace with the increasing demands for services in South Jersey, by opening our Washington Township Health and Wellness Center. Throughout 2010, we employed thousands in the construction of the Voorhees Replacement Hospital, and completed infrastructure improvements to our hospitals in Mount Holly and Berlin. We also began planning for additional expansion and improvements in Mount Holly and a wellness center in Moorestown. In 2010, Virtua - West Jersey Health Systems, Inc. subsidized almost $50 million in care for the uninsured. We delivered or supported health improvement programs with an additional $26.1 million throughout our region. And through our relationships with regional educational institutions and other business partnerships, we fostered job growth and development. Our 2010 Community Benefit Report documents the wide variety of programs and services provided to the community free of charge or subsidized by Virtua - West Jersey Health Systems, Inc.Hospital Charity Care: $22,230,047Charity care is made available to Virtua's patients who may need financial assistance. Some uninsured or underinsured patients will qualify for assistance under the State of New Jersey charity care program. Determination for eligibility considers both income and assets, with the income test being based on the federal poverty guidelines (up to 300%). For patients that qualify, care is provided without charge or at amounts less than Virtua's charges. Some patients in need of assistance will not qualify for the State's program due to not meeting the specific criteria. Virtua believes that charity care should be available to a larger segment of our community. Accordingly, the Virtua Charity Assistance Program ("CAP") was implemented. Under the CAP, qualification is based solely on income, not assets. Patients that document their total household income to be below 500% of federal poverty guidelines are provided a substantial reduction to charges. In addition, patients who qualified for a partial (less than 100%) charity care adjustment under the State's program will receive an incremental reduction under the CAP to bring their total reduction to the maximum of the two programs. After all efforts to identify insurance coverage, qualification for coverage under an assistance program (Medicaid, for example), or qualification for charity care relief under the State or CAP criteria, Virtua attempts to obtain an estimate of annual income relative to the responsible party. If the estimated income meets the CAP qualification criteria, the charity care adjustment will be applied. Charity care assistance is also available under CAP relative to individuals whose income is too high to qualify for relief under Chapter 160 or regular CAP criteria, but they are experiencing medically catastrophic circumstances.Medicaid shortfall: $14,803,856Virtua cares for many individuals who have qualified for coverage under the Medicaid program. Some are enrolled in the program as administered directly by the State of New Jersey, while others are enrolled in Medicaid plans that are managed by health maintenance organizations. Aggregate payments from all Medicaid plans are not adequate to pay for the total cost incurred providing care to these individuals. Accordingly, there is an unreimbursed component of the cost of caring for those having coverage under the Medicaid program, which is accounted for under this category.Preventing disease begins with awareness. That's why Virtua devotes expansive resources to health and wellness education, providing prevention strategies for people of all ages. Part II: Virtua is directly involved in and supports numerous community building activities which promote the health and betterment of the communities served. The annual goals and objectives established for all managers must include individual community involvement goals. Overall, Virtua's initiatives fall under many of the specific community building activity categories noted. Costs related to any of these activities are not included elsewhere on Form 990, Schedule H. The following are examples of specific community building activities. Virtua is involved in various initiatives intended to facilitate economic development in our communities, whose combined unemployment rate has exceeded that of the state. Virtua supports and participates on the boards of the local county Chambers of Commerce, whose goals include promoting business growth. Virtua has also created a career transition partnership to offer support and counseling to those seeking employment. Virtua managers provide counseling on subjects such as networking, resume development, and interviewing.Virtua participates in the Camden Higher Education and Healthcare Task Force, and the Greater Camden Partnership. Goals include drawing businesses to the city, keeping the city streets clean and safe, and assisting with planting trees and cleaning up neighborhoods. All Virtua hospitals participated in a state-wide pandemic flu emergency exercise. This is an exercise that simulates a large increase in the number of patients seeking medical attention for flu-like illnesses. To ensure that Virtua is doing all it can as a responsible community member to protect our environment, the Virtua Green Initiative was established with the mission of promoting environmental stewardship through the effective use of resources throughout the Virtua community. The Initiative is carried out under the guidance of a Green Council and location specific teams that drive efforts to reduce, reuse and recycle through education, communication and standardization of green initiatives. In partnership with Camden Area Health Education Center, Virtua sponsors a seasonal farmers market on our Camden site as a way for Camden residents to access fresh produce and to obtain information on healthy eating. Blood pressure and glucose screenings are also provided. Virtua has also partnered with Big Brothers Big Sisters to provide a Day of Play, an effort to expose staff to a day of mentoring Camden city youth and expand the base of volunteer mentors in Camden County and the city. Virtua is a member of the Camden Coalition of Healthcare Providers, a citywide organization that serves health needs with the mission to improve the health status of Camden residents and to make the health system more accessible for the underserved. Virtua is also a member of the Camden Healthy Futures Committee, whose mission is to develop and maintain a sustainable system of communication and collaboration in healthcare planning, program/service development and implementation, to address the top five health priorities in Camden city. Virtua has partnered with a local program, "The Medical Arts Academy", that is offered by a county technical school. The program accepts bright, underprivileged high school students who, through a partnership with a local college, can earn college credits while attending high school. The program is designed for those planning to pursue a college degree in the medical field. Students in the program spend part of their day onsite in medical facilities working side-by-side with physicians, nurses, technicians and hospital administrators. After being exposed to various health-related career fields over their four years of high school, graduates will be able to make a more informed decision when choosing a career path, while having saved time and money as they earned transferable college level credits. Virtua provided many lectures on H1N1 prevention, general infection prevention, and the importance of hand washing to outside organizations including state societies and businesses, as well as to the general public at libraries and community dinners.Virtua has analyzed physician service needs for zip codes within its service area, and has worked with a physician recruiter to attempt to bring physicians into underserved areas.
Part I, line 7A cost accounting system was utilized to determine the costs of various services. The cost accounting system determines cost at the charge description master (CDM) item level. Department specific costs are allocated to the individual CDM item based on applying relative value unit (RVU) measures or the relationship of CDM item charges. This approach results in a truer allocation of cost in comparison to the application of an overall cost to charge ratio. The methodology addresses the service segments of inpatient, same day surgical, emergency department, clinic, and other outpatient care as well as being delineated at the payer level (Medicare, Medicaid, health maintenance organizations, self pay, etc.).Part III, line 4Bad debt expense (at cost) was calculated by applying a cost to charge ratio to the total provision for bad debt for the year. Discounts provided for services would not be included in bad debt expense, but would rather be reported as allowances that offset revenues. Payments received on patient accounts would directly reduce the provision for bad debts. No amount is reported relative to patients who likely would qualify for financial assistance under the hospital's charity care policy, if sufficient information had been available to make a determination of their eligibility. The reason for this is that prior to any patient account being written off as bad debt, Virtua-West Jersey Health System (Virtua) attempts to obtain an estimate of annual income relative to the responsible party from a credit management company. If the estimated income meets established criteria, a reduction to charges is applied. Qualification for the reduction is presumed in cases when a match can not be made for an individual, as well as for deceased individuals. Allowance for doubtful accounts is described as follows in the footnotes to the financial statements: Virtua provides an allowance for doubtful accounts for estimated losses resulting from the unwillingness or inability of patients to make payments for services. The allowance is determined by analyzing historical data and trends. Accounts receivable are charged off against the allowance for doubtful accounts when management determines that recovery is unlikely and Virtua ceases collection efforts. Losses have been consistent with management's expectations.Part III, line 8The costs associated with Medicare patients are based in total on the submitted cost report, which utilizes a cost to charge methodology for determining cost. Necessary adjustments were made to this total to exclude the costs already recognized in other sections of Schedule H, such as costs related to subsidized health services and graduate medical education. Virtua is of the opinion that the full Medicare shortfall total should be deemed/considered as community benefit. As reimbursement does not cover the costs incurred in the provision of care for Medicare patients, there is benefit to the government who is not paying the full cost of care. Also, many Medicare patients have low or fixed income levels. The Internal Revenue Service does regard a hospital's participation in Medicare or Medicaid as a key factor that assists in establishing that a hospital satisfies the Community Benefit standard. Also, many Medicare patients have low and fixed income levels. The Medicare Payment Advisory Commission reported in their June 2011 "A Data Book: Healthcare Spending and the Medicare Program", the following percentage breakdown of income status relative to federal poverty guidelines based on characteristics of the Medicare population as of 2007:Below poverty: 14%100-125% of poverty: 9%125-200% of poverty: 19%200-400% of poverty: 33%Over 400% of poverty: 25%Under Virtua's Charity Assistance Program, families having gross income of less than 500 percent of the federal poverty level are eligible.Part III, line 9bIf a patient is known or expected to qualify for assistance, their account is placed on hold while the formal determination is made. If subsequently approved for 100% qualification, collection activity ceases. If approved for a percentage reduction less than 100% of the amount due, the account is adjusted accordingly and an attempt is made to establish a payment plan relative to the remaining balance. If the account does not qualify for assistance the patient is notified accordingly and an attempt will be made to collection the balance.
Part VI, Line 2: Virtua has been an active participant in initiatives undertaken by the three counties that comprise its primary service area in southern New Jersey; Burlington County, Camden County, and Gloucester County. Individually these counties have assessed the healthcare needs of their residents, which have resulted in the creation of three distinct reports; The Burlington County Community Health Improvement Plan. The Camden County Mobilizing for Action Through Planning and Partnership (MAPP) Coalition Community Health Improvement Plan, and Achieving a Healthier Gloucester County. Burlington County has identified emergency preparedness, nutrition and physical activity, preventative healthcare, environmental health, peace and well being including alcohol and drug abuse reduction, and parenting as priorities. Camden County priorities include obesity and nutrition, cardiovascular health, cancer, environmental health, and mental health. In the Gloucester County assessment, priority areas include increasing awareness of existing services (health education), encouraging regular screenings and check-ups (especially for heart, cancer, diabetes, sexually transmitted diseases), and promoting healthy behaviors (especially diet and exercise).Virtua also is involved with the Camden City Healthy Futures Committee, which has conducted a health needs assessment of the city and has put action plans in place for each of the top health priorities identified. Priorities have been set relative to obesity (heart disease, diabetes, stroke) and access to care, high cancer mortality rate, mental health, violence and safety, environmental health (lead poisoning and asthma), and family health (specifically teen pregnancy and access to dental care). The Mental Health/Co-occurring Disorders Workgroup focuses on addressing the need for services and advocacy for co-occurring disorders such as drug and alcohol abuse and mental health in adolescents and young adults. The workgroup created a provider survey to gather data regarding co-occurring services and gaps in Camden County.Virtua also works with the Camden County Cancer Coalition, a group that has completed a cancer needs assessment for the county. Camden County's major strategy for eliminating disparities in cancer care is the Cancer Education and Early Detection (CEED) programs. There are CEED programs based at Virtua and they are the force behind the plans to increase minority screening rates, increase prostate cancer screening rates, and reduce smoking.Virtua participates on disaster and flu planning groups in both Burlington County and Camden County, as well as their public health planning committees. Part of the mission is to identify and determine how to address community public health needs. Virtua has implemented a risk assessment and screening process for admitted hospitalized in-patients with a multi-drug resistant organism, based on prevalence studies within the hospitals and community evaluation. These efforts have resulted in patient screening and isolation protocols. Through participation in various community meetings and forums, Virtua receives input from the service area relative to community health needs. Through its Program of Excellence structure, Virtua monitors community needs specific to its service lines and identifies available resources it can call upon to address them.
Part VI, Line 3: The availability of the financial assistance policy is communicated in both English and Spanish in a variety of ways. Financial assistance information is provided by registration staff and is covered in financial counseling appointments. Brochures are distributed and also made available in the patient/family waiting areas. Bilingual signage is posted throughout the hospital, including in the emergency departments and outpatient registration areas. The admission booklet and handouts provided at registration/admission contain information about financial counseling and guidance should the patient have difficulty in paying their hospital bill. Availability of charity care assistance is also indicated on all statements and letters sent to patients. Virtua's website contains information on charity care assistance along with the application. On a one to one basis, financial counseling services are provided to patients that are uninsured or underinsured. Support is provided to help patients complete relevant applications for assistance under the State of New Jersey charity care program guidelines, the State of New Jersey Medicaid program, Virtua's own charity care program, and any other assistance for which they may be eligible. Completion of applications is conducted through bedside interviews with admitted patients, and via letters, phone calls, and field service visits to patient homes, when appropriate. Bilingual staff are available on-site and interpretation services are available over the phone.
Part VI, Line 4: Virtua Health, Inc. has four acute care hospitals; There are Virtua-West Jersey Health Systems, Inc. (Berlin, Marlton and Voorhees) and Virtua-Memorial Hospital Burlington County, Inc. Virtua Health, Inc. is the largest health system in Southern New Jersey, serving the 1.25 million individuals residing in Burlington, Camden, and Gloucester counties. In addition to Virtua's four acute care hospitals, there are five other health organizations operating a combined total of eight hospitals (including one heart and lung specialty hospital) within these three counties. This area represents a variety of geographies and socioeconomic statuses, from the affluent suburbs of Philadelphia, to the rural, agricultural communities in East Burlington and South Gloucester counties, to the city of Camden where over 50 percent of individuals reside below the federal poverty level. The U.S. Department of Health and Human Services' Health Resources and Services Administration has designated Pemperton/Browns Mills (Burlington County), the city of Camden (Camden County), and Gloucester City as medically underserved areas or populations. Residents 65 and over represent a large percentage of the service area, and this population is projected to grow by 15 percent in the next five years. Though the Virtua service area is mostly Caucasian, minority populations are projected to experience the highest population growth over the next five years, particularly Hispanic and Asian communities. As such, Virtua is committed to providing care for the diverse and growing community.Virtua also cares for a community with a higher rate of chronic diseases than the rest of New Jersey. There are higher incidences of diabetes, cancer, heart disease, and obesity in the Tri County area than in the rest of New Jersey, and as the market leader of this community, Virtua provides care for these populations.
Part VI, Line 6: The Board of Trustees is community based, consisting of individuals with various professional backgrounds, including some physicians. Almost all of the board members reside in the hospital's primary service area. The organization has an open medical staff that will provide privileges to qualified physicians from within the community. Virtua has and will continue to utilize its financial resources to invest in projects, technologies, and programs that will contribute to the health status of its community members. A new healthcare campus that will include a state-of-the-art digital hospital and an outpatient center that provides easy access to a wide variety of comprehensive services is currently under construction.
Part VI, Line 7: Virtua- West Jersey Health System is a controlled entity of a community-owned health system, Virtua Health, Inc. In its entirety, Virtua Health, Inc. consists of four acute care hospitals, numerous outpatient services and clinics, two nursing homes, two fitness centers, a home health agency, a physician organization, a foundation, and an insurance captive. Community benefit programs are coordinated across the Virtua organization. The individual hospitals will develop, implement, and fund programs specific to the needs of its local community. In addition, under the parent company's centralized Program of Excellence structure, initiatives are undertaken that have impact across all Virtua entities and communities.
