Part VI - Additional InformationPart VI - Needs Assessment (continued)The indicators are related to income (percentage of households over age 65 below poverty line; percentage of families with children under 18 below poverty line; percentage of single female families with children under 18 below poverty line), education (percentage of population over 25 without a high school diploma), culture (percentage of population that is minority including Hispanic/Latino ethnicity; percentage of population over age 5 that speaks English poorly or not at all), insurance ( percentage of population in the labor force, aged 16 or more, without employment; percentage of population without health insurance), and housing (percentage of households renting their home).During FY10, in addition to the continued use of the "Ulster County Community Health Assessment" report, we also use a range of other available needs assessments to identify and respond to local needs. In fiscal year 2009, we undertook a physician Community Needs Assessment Study, another key component at examining the community needs and access to quality healthcare providers. This study reviewed by specialty in Health Alliance of the Hudson Valley's ("HAHV") designated geographic areas comprising or existing within the confines of HAHV's Kingston primary service area. This community need was determined based upon demonstrable and documented objective evidence. The board of Trustees, via the Audit and Compliance committee of the Board, reviewed and approved the community need study/report.In order to define community need, we followed the 2007 Federal Register guidelines establishing the necessary link of community need in the Primary Service Area ("PSA") and physician recruitment activities:"The geographic area served by the hospital, is the area composed of the lowest number of contiguous zip codes from which the hospital draws at least 75% of its inpatients."HAHV demonstrated and documented Community Need whereby the physician to population ratio in our PSA for the respective physician's medical specialty is below the recommended ratio resulting in an FTE deficit on the "community need model."By fulfilling these two requirements, HAHV established the necessary link of community need in the PSA and physician recruitment activities. The analysis demonstrated a documented community need for additional certain medical specialties such as pulmonary and cardiology, as well as a need for most surgical specialties. HAHV is working with existing physician practices to recruit new physicians to these specialties to insure the community need and access are improved. In 2010, TKH worked collaboratively with private and public stake holders, the Ulster County Commissioner of Health and the County Executive to identify underinsured healthcare. This study was published in 2010 entitled "Healthy Ulster - A Public Health Approach". A recent report ranked Ulster County 33rd in health outcomes and 30th in health factors compared to other counties (62) in New York State. The leading causes of death in Ulster County are heart disease, cancer and stroke. Based on the above needs assessments, the major community needs identified for fiscal year 2010 were:*Identify community and government partners who could work in a collaborative way to improve access and delivery of Behavioral Health services.*The need to increase access to quality health care, especially for children, pregnant women, uninsured adults and seniors.*The need to obtain medical care for the underserved by enrolling eligible residents to Medicaid, MHIP and other insurance programs and by building a better system of care for the uninsured.*The need to eliminate racial and ethnic health disparities by providing culturally and linguistically competent care and target diseases that are more prevalent in minority populations.*To provide health education, disease prevention and chronic disease management (including obesity) programs to improve the health status of the community.Continued on Schedule O.
Part VI - States Where Community Benefit Report FiledNY
Part VI - Affilated Health Care System Roles and PromotionThe Kingston Hospital is a member organization of HealthAlliance. Fellow member organizations include Benedictine Hospital, Margaretville Memorial Hospital and Woodland Pond at New Paltz. As a not-for-profit health system, HealthAlliance reinvests its profits back into the community through programs to serve the poor and uninsured, manage chronic conditions like diabetes, health education and promotion initiatives, and outreach for the elderly. In Fiscal Year 2010, for TKH, this included nearly $2.5 million in such community benefits. Therefore, HealthAlliance takes a systems approach in its community benefit planning and implementation, and is consequently able to ensure that its member hospitals and other entities/affiliates are helping promote and address the health needs of their respective communities.Ongoing Program Initiatives:The Kingston Hospital continues to grow and develop specific programs identified as community needs. These programs continue to be planned and implemented with the support and participation of the Board, medical staff and administration. These include the following:Outpatient Addiction Therapy: The hospital's out-patient clinic, BridgeBack offers eight different programs, at various levels of care, including day rehab, family, gender adolescent programs and a "special needs" group and intensive outpatient. The "Special Needs" group is fast becoming very popular in the treatment community, especially with the local courts and social services departments. The program has been successful at reducing recidivism, as its primary target population is the client who traditionally has been difficult to engage and keep in treatment. BridgeBack continues to offer an adolescent program and works closely with local schools like Kingston High School who will send a student for an intake assessment they suspect needs a substance abuse program. TKH's Methadone Treatment Program ("MTP") is located on the opposite side of the same building, which makes it very convenient for those clients who participate in both programs. Both BridgeBack and Methadone are licensed by the NYS Office of Alcoholism and Substance Abuse Services (OASAS). In order to meet its clients' special needs, the treatment team has been paying close attention to providing more individualized treatment. This has been effective in the adolescent program where the retention rate is varied. All of the substance abuse programs accept most insurance carriers. Charity applications and a sliding fee scale are provided to all clients who do not have an ability to pay for substance abuse services. The Kingston Hospital continues to sponsor an annual "Recovery Conference" for all professionals in the community and engages well-known presenters in the field of substance abuse.Stroke Designation Program: The Kingston Hospital administrative and clinical staff continues to place priority on keeping the hospital current with the best practices in disease management of the stroke patient. Our stroke patient management program offers a continuum of care from onset of stroke symptoms to long-term recovery. Four beds have been designated as a Stroke Step Down Unit. Staffs are telemetry and ACLS trained. All requirements for Stroke Designation have been fulfilled. Our Stroke Center is accredited by the New York State Department of Health. The Stroke Center is accredited by the New York State Department of Health. The Stroke Center participated in the American Stroke Association "Get with the Guidelines" Stroke Program and regularly inputs data resulting from efforts to improve acute stroke treatment and prevent secondary events. TKH received the 2009 American Heart Association/American Stroke Association's Get With The Guidelines Stroke Gold Plus Performance Achievement Award. The award recognizes TKH's commitment and success in implementing excellent care for stroke patients, according to evidence-based guidelines. Continued on Schedule O.
Part VI - Explanation Of How Organization Furthers Its Exempt PurposeThe Kingston Hospital has a 10 member community board comprised of a majority of community members. Only one of the 10 Board members is employed by The Kingston Hospital or HealthAlliance, the Hospital's parent corporation (one HealthAlliance Executive). The medical staff of The Kingston Hospital is organized in the public interest. Medical staff privileges in the Hospital are open and available to all qualified physicians and we have a large, diverse medical and dental staff (376 members) in which most share joint privileges with Benedictine Hospital.Effective May 2010, The Kingston Hospital operates the sole Emergency Room in the city of Kingston. It is accessible to anyone needing care regardless of ability to pay. The consolidated Emergency Room contains state of the art technology and highly qualified practitioners to ensure delivery of timely and quality emergent care.No part of the income of The Kingston Hospital insures to the benefit of any private individual nor is any private interest being served. All surplus funds are reinvested into the facility, equipment or programs of the hospital to improve the quality of patient care, expand our facilities, and advance our medical training, education and research programs. The Kingston Hospital actively advocates on behalf of poor and vulnerable individuals, especially for healthcare coverage for all persons and improved community health. During 2010, we encouraged our employees, as healthcare workers, to write to their local senators and congressional representatives about the need for healthcare reform. The Kingston Hospital has a vibrant volunteer program, offering varied opportunities to members of the community to volunteer. Currently, Kingston Hospital has 127 on their active service list with 109 volunteers contributing their time in 2010.
Part VI - Community InformationThe Kingston Hospital, one of three hospitals within a two-market area, primarily serves the residents of Ulster County, an area with a total population of approximately 180,000. Our Primary service area consists of 22 zip codes in Ulster and Dutchess County that account for the top 75% of discharges. Our secondary service area is comprised of zip codes in Ulster, Eastern Delaware and Northern Dutchess Counties that make up the next 15% of discharges.Approximately 18% of the 149,000 residents in our primary service area are minorities, compared to 39.5% for the State. Demographic analysis reveals that almost 22% earn less than $25,000 and 47% less than $50,000. The area also has a significant number of women of childbearing age (20%) and Adults 55+ (28%). Within our service area, zip codes to the South (New Paltz and Ulster Park), East (Highland), and North (Saugerties) of Kingston Hospital are the fastest growing zip codes, however Kingston still accounts for largest population density.Based on the NYS Interim Expanded Behavioral Risk Factor Surveillance System report for the period July 2008 - June 2009, The Ulster County Community Health Assessment reports that Ulster County's rate (13.1%) of uninsured adults is slightly below the estimated NYS average rate (13.4%).
Part VI - Patient Education of Eligibility for AssistanceThe Kingston Hospital is committed to:*Providing access to quality healthcare services with compassion, dignity and respect for those we serve, particularly the poor and the underserved in our communities.*Caring for all persons, regardless of their ability to pay for services.*Assisting patients who cannot pay for part or all of the care they receive.*Balancing needed financial assistance for some patients with broader fiscal responsibilities in order to sustain viability and provide the quality and quantity of services for all who may need care in a community. In accordance with AHA recommendations, TKH has adopted the following guiding principles when handling the billing, collection and financial support functions for our patients:*Provide effective communications with patients regarding hospital bills*Make affirmative efforts to help patients apply for public and private financial support programs*Offer financial support to patients with limited means*Implement policies for assisting low-income patients in a consistent manner*Implement fair and consistent billing and collection practices for all patients with patient payment obligationsTKH effectively communicates with patients regarding patient payment obligations. Financial counseling is provided to patients about their payment obligation and hospital bills. Information on hospital-based financial support policies and external programs that provide coverage for services are made available to patients during the pre-registration and registration processes and in response to patients seeking financial assistance. Information regarding the financial assistance is also provided by the onsite financial counseling staff in both inpatient and outpatient areas. Patient accounting also supports the financial counseling program by providing patients with information and applications while handling customer service calls. Our Medicaid vendor and county Medicaid workers also provide guidance regarding the financial assistance program when necessary. Information regarding the financial assistance program is provided by the collection agency working with our patient accounting department as well.Financial counselors, both internal staff and a paid external vendor, make affirmative efforts to help patients apply for public and private programs for which they may qualify and that may help them obtain and pay for healthcare services. The hospital has onsite Medicaid eligibility representatives through ACH and also has representatives from Fidelis Insurance Company on site to help patients who are in need of insurance coverage navigate through the application process. The patient does not need to have an account with TKH; they just have to be in a situation where they are in need of insurance coverage. Fidelis screens the patient to see which plan will fit them and they are certified to enroll the patients in many different insurance policies based on their insurance needs. Every effort is made to determine a patient's eligibility prior to or at the time of admission or service. However, determination for financial support can be made during any stage of the patient's stay after stabilization or collection cycle.TKH offers financial support to patients with limited means. This support is available to uninsured and underinsured patients who do not qualify for public programs or other assistance. Notification about financial assistance, including contact information, is available through the hospital website, in hospital welcome packages located in patient rooms, hospital posters and flyers, financials assistance applications and hospital statements. TKH has established a written policy for the billing, collection and support for patients with payment obligations. TKH makes every effort to adhere to the policy and is committed to implementing and applying the policy for assisting patients with limited means in a professional, consistent manner. Continued on Schedule O.
Part VI - Needs AssessmentPursuant to New York State Public Health Law Section 2803-L, The Kingston Hospital("TKH") is required to file a comprehensive community service plan ("CSP") with The New York State Department of Health ("DOH") every 3 years. The text of New York State Public Health Law Section 2803-L is as follows:Community service plans. (1) The governing body of a voluntary non-profit general hospital must issue an organizational mission statement identifying at a minimum the populations and communities served by the hospital and the hospital's commitment to meeting the health care needs of the community. (2) The governing body must at least every three years: (i) review and amend as necessary the hospital mission statement; (ii) solicit the views of the communities served by the hospital on such issues as the hospital's performance and service priorities; (iii) demonstrate the hospital's operational and financial commitment to meeting community health care needs, to provide charity care services and to improve access to health care services by the underserved; and (iv) prepare and make available to the public a statement showing on a combined basis a summary of the financial resources of the hospital and related corporations and the allocation of available resources to hospital purposes including the provision of free or reduced charge services. (3) The governing body must at least annually prepare and make available to the public an implementation report regarding the hospital's performance in meeting the health care needs of the community, providing charity care services, and improving access to health care services by the underserved. (4) The governing body shall file with the commissioner its mission statement, its annual implementation report, and at least every three years a report detailing amendments to the statement and reflecting changes in the hospital's operational and financial commitment to meeting the health care needs of the community, providing charity care services, and improving access to health care services by the underserved. The above-referenced CSP requirement was enhanced by the DOH's Prevention Agenda initiative. This initiative is a process that asks hospitals such as TKH to work with local health departments and community partners to assess community health needs, jointly develop plans to address two or three of the identified needs and include this collaborative work in the TKH's CSP update submitted to DOH. Information on the New York State DOH Prevention Agenda can be found at www.health.state.ny.us/prevention/prevention_agenda.TKH identifies unmet community health needs by participating in community coalitions, partnerships, boards, committees, commissions, advisory groups, and panels. On an annual basis, the hospital analyzes internal patient surveys and publicly available data on the market including demographics and health services utilization. Local needs assessments and reports, such as the latest Ulster County Community Health Assessment 2010-2013 and the community needs index developed by healthcare business of Thomson Reuters, are used as they became available to determine the types and locations of community benefit programs implemented.Using the community needs index, The Kingston Hospital gathers vital socio-economic and demographic factors to support internal decision making for resource allocation and to determine the geographic location of new programs to meet emerging needs. For each zip code, the community needs index methodology aggregates five socioeconomic indicators/barriers to healthcare access that are known to contribute to health disparity. Continued at "Additional Information" section.
Part III, Line 9b - Provisions On Collection Practices For Qualified PatientsThe Credit and Collection Policy for Self Pay Balances provides that, in some cases, non insured patients that do not qualify for Medicaid qualify for charity care. In those instances the Credit and Collection Department will follow the guidelines as set forth in the Charity Care policy. The Charity Care Policy provides that the Financial Counselor evaluates the information provided in patients Charity Care application in conjunction with Federal Income Poverty Guidelines to determine the write-off percentage. If the patient/guarantor qualifies for Charity Care based on the sliding scale, the patient will be required to pay the outstanding amount. A payment plan is established not to exceed 10% of the guarantors gross monthly income.
Part III, Line 8 - Explanation Of Shortfall As Community BenefitThe amount reported on Line 6 was derived by using the Medicare allowable costs as reported on the Hospital's New York State Institutional Cost Report and the applicable adjustments from Worksheet B.
Part III, Line 4 - Bad Debt ExpenseA/F/S Footnote - Receivables for Patient Care and Allowances for Doubtful Accounts:Patient accounts receivable result from the health services provided by the Hospital. Additions to the allowance for doubtful accounts are recorded to the provision for bad debts. Accounts written-off as uncollectible are deducted from the allowance for doubtful accounts. The amount of the allowance for doubtful accounts is based upon managements assessment of historical and expected net collections, business and economic conditions, trends in Medicare and Medicaid health care coverage and other collection indicators.A/F/S Footnote - Charity Care:Federal and state law requires that hospitals provide emergency services regardless of a patients ability to pay. Uninsured patients seen in the emergency department, including patients subsequently admitted for inpatient services, often do not provide information necessary to allow the Hospital to qualify such patients for charity care. Uncollectible amounts due from such uninsured patients represent a portion of the provision for bad debts reflected in the accompanying consolidated statements of operations. Uncompensated care reported as provision for bad debts for the years ended December 31, 2010 and 2009 approximated $6.4 million and $5.8 million, respectively.Bad debt expense at cost is $1,829,877. Bad debt expense per the audited financial statement is $6,362,752.The cost-to-charge ratio derived from The New York State Institutional Cost Report was applied to the bad debt expense per the audited financial statements to determine the amount reported in Part III, line 2.
Part I, Line 7 - Explanation of Costing MethodologyThe Kingston Hospital's costing methodology was based upon the 2010 New York State Institutional Cost Report and the 2010 Medicare (Form 2552) Cost Report. These cost reports are filed with the New York State Department of Health and the applicable CMS intermediary, respectively. The cost-to-charge ratio derived from The New York State Institutional Cost Report was used for the various sub-line items of line #7.

To keep in line with the consolidation of services between The Kingston and Benedictine Hospitals, the maternity units of both facilities were relocated on The Kingston Hospital campus mid-2009. Entitled "The Family Birth Place", it is our belief that every woman is entitled to a joyful and healthy birth experience. We believe in providing a range of choices for the expectant women of our community who have different needs and know what they want. We are committed to providing a secure, yet family-friendly environment where the well-being of our mothers and babies is our highest priority. As the experienced obstetric nurses and technicians from BOTH Benedictine and The Kingston Hospital came together to create this new state-of-the-art birthing unit, our patients will get the best of both worlds as we offer our community more than ever before.
Schedule H, Part VI, Line 7 continuedThe hospital continues to follow best practice guidelines outlined by Greater NY Hospital Association and CDC as well as new measures and interventions implemented by the Center for Medicare and Medicaid Services over the past three years. Kingston Hospital maintains a current infection rate for Central Lines in the ICU and outside of the ICU at O% for 2010. Kingston Hospital is also proud to report that we have had one only Class I and zero Class II Surgical Site Infections for 2010, a current rate of 0.08%, which is down 0.34% from 2009 and well below the benchmark of NNIS of 3.3%.Occupational Health & Wellness: The Occupational Health & Wellness Program (Kingston WoRx) continues to work with employers throughout a three-county area to provide prompt and comprehensive occupational health services to meet their business and employees health care needs. These services include health screening, pre-placement physical exams, injury management for work related injuries and drug screenings for federally mandated required employers. In 2010, the Program served 54 volunteer fire departments and over 1,500 firefighters. The Occupational Health & Wellness Program (Kingston WoRx) continues to focus on delivering educational programs to the business community on key occupational issues that impact the health, safety and well-being of their employees. These forums have included seminars on ergonomic, workers compensation injury management, preventing violence in the workplace, developing behavioral safety programs, stress management and training of supervisors to identify employees with substance abuse problems.Through our collaborative work with the Ulster County Office of the Aging, we continue to assist in delivering health-screenings services to the seniors in Ulster County through the Care-A-Van program. The Care-A-Van provides educational and screening services three days a week. The Kingston Hospital provides the medical directorship and nursing service component to assist in delivering these screenings. The Kingston Hospital continues to work cooperatively with the American Red Cross to assure at least three blood drives annually.Physician Resident Training Programs: The Kingston Hospital remains the primary hospital teaching affiliate of the Institute for Family Health's Residency Training Programs. This program provides for the training of self-reliant, caring, competent family physicians via hands on, clinically oriented learning with strong emphasis on longitudinal pre-natal and continuity care and serving at risk under-served populations. The Residency Training Program's success is evident with over 100 graduates, many of who practice locally and contribute to undergraduate and graduate medical education teaching. The Kingston Hospital continues to be proud of being the initial sponsor of the program in the late 1970s.The Kingston Hospital continues its affiliation with Benedictine Hospital's Podiatric Residency Training Program. The residents in this training program in addition to assisting surgeons in the operating room train with physicians in the Department of Internal medicine and many subspecialties. They also receive training and instruction on performing history and physical examinations. The residents also do rotations through the outpatient Wound Healing Center.
Schedule H, Part VI, Line 7 continuedThe Greenspan Center also offers Nuchal Translucency ultrasound that coupled with a simple, finger stick blood test, can diagnose Downs Syndrome in the early stages of pregnancy. Additionally, the Newborn Hearing Screening Program continues to enable us to do the most up to date screening available to the State. This beautiful new unit is designed to make every delivery a safe, natural and comfortable experience. Patients are provided with spacious new rooms, private bathrooms, personalized care with attention to detail, many proven methods of low intervention labor management, a new specially equipped operating room for Cesarean Sections and a family dining room allowing for comfortable meals together. The unit also offers a variety of support groups including Bereavement, Child Preparation Classes, infant CPR, etc. These programs are free to the public.Final Consolidated Services:In May 2010, the final phase of the clinical consolidation occurred. The consolidated Emergency Department (ED), Intensive Care Unit (ICU) and Medical/Surgical (M/S) projects were finalized. The renovated ED will treat 50,000 patients annually in a state of the art facility. In addition, a contract was executed to bring the physician component under the direction of EMA, Emergency Medical Associates. The 20-bed ICU supports the volume of patients that will be seen, treated and transferred to the new unit. While most of the M/S patients will be treated at The Kingston Hospital, Benedictine Hospital will retain a nominal number of such beds to treat the elective surgical procedures.Surgical Services: The Kingston Hospital Surgical Services Department continues to work closely with many educational facilities (Kingston High School, Ulster County BOCES and Ulster County Community College) to provide educational opportunities and career advancement for high school and adult learners in our community. Resources were allocated to implement a Surgical Information System that allows a more efficient method of scheduling and data collection. This system provides many features that enhance patient safety and help manage the patient's medical record. Reports can be generated to track utilization and supply usage. Expansion of the functionality of this system is underway to include the Surgical Services on the Benedictine Campus. The perioperative documentation module was also purchased which enhances patient care, patient safety and patient outcomes. The Kingston Hospital has a committed focus on improving the surgical services provided to the community. A clinical ladder program has been developed for the surgical technicians, as well as a program to assist the staff in achieving national certification. The Kingston and Benedictine Surgical Services Departments are working in collaboration to improve efficiencies, reduce inventory and develop uniform policies. Staffs are involved in cross training on both campuses and in developing competencies to assure quality. Financial resources were allocated to update specialty instrumentation, equipment and technology. The hospitals are continuing to pursue the recruitment of additional general surgeons and other specialty physicians, particularly orthopedics, which is needed in the community. The Surgical Services team was also actively involved with the design, construction and implementation for the new Foxhall Ambulatory Surgery Center ("FASC"). Working with architects, administration, physicians and staff, a unique two operating room facility was opened in March 2009 to further enhance the services provided in our community. The facility continues to provide proscribed services to the residents of Ulster County.
Schedule H, Part VI, Line 7 continuedHospitalist Program: The Kingston Hospital initiated its Hospitalist Program in the first quarter of 2006. The program has grown in size over the past several years and continues to be recognized as a new concept in medical care, providing in-house care for patients that do not have a primary care physician or at the request of the patient's primary care physician to manage the patient's entire hospital care from admission to discharge. The program, as expected, has shortened the average patient's hospital stay and increased the patient's comfort and knowledge since the Hospitalists are in-house and available to meet with the patient and family to answer questions and respond quickly to changes in patient status. The presence of a hospitalist physician on site has improved communication between the hospital and physician staff. Results have improved correlation between patient treatment requirements and level of care. The Physician Assistants previously located on the Benedictine Campus have joined the Hospitalist program and formed a group named Mid-Hudson Physicians, PC. In addition to the PA's and hospitalists, the group also employs intensivists and coverage is provided at both the Kingston and Benedictine campuses. As reflected in the Mission and Vision statements, The Kingston Hospital is committed to playing an even stronger leadership role in our community, including:*Enhanced efforts to communicate with, and advocate for, all sectors of the community, including a commitment by hospital administration to interact directly with community members on an on-going basis.*Dedication of additional resources for expanded provision of needed community education programs.*Fostering collaboration among health care providers to effectively address service gaps.
Schedule H, Part VI, Line 3 continuedTKH educated staff members who work closely with patients (including those working in Patient Registration and Admitting, Financial Assistance, Customer Service, Billing and Collections) about these policies with an emphasis on treating all patients with dignity and respect regardless of their insurance status or their ability to pay for services. All patient registration staff receives in-service training regarding the Financial Assistance Program. Patient Accounting also receives information about the program and how to handle patients seeking financial assistance. Financial counselors receive in depth training to handle financial assistance requests, process applications, and manage outcomes.
Schedule H, Part VI, Line 2 continued*In summary, our activities focus primarily on positively impacting the health of our community with programs that are culturally and linguistically tailored to meet the unmet needs of women, infants, seniors, and racial and ethnic minorities.*To work with physician practices to recruit and retain obstetricians which would allow the hospital to continue to provide quality OB/GYN services. The Kingston Hospital's interdepartmental leadership, executive management, and its Board of Trustees plan, monitor and evaluate the Hospital's community benefit efforts. Initiatives are thoughtfully planned to ensure that links exist between the Hospital's clinical expertise and unmet community needs. To determine the needs of the community, The Kingston Hospital has a Chief Executive Officer review committee on community benefit (an internal, interdepartmental committee) that utilizes available data (E.G., needs assessments, hospital patient data, community needs index) and community input to develop the hospital's community benefit work plans.HAHV and Ulster County have formed leadership steering committees to focus on future planning and solutions to two key community health concerns: ED usage by the uninsured or under insured and innovative employee health initiatives. These steering committees meet bi-monthly.
