Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - Saint Francis Hospital Vinita, Inc.. Saint Francis Hospital Vinita conducted its Fiscal Year 2025 Community Health Needs Assessment (CHNA) in collaboration with the Oklahoma State Department of Health District 4 and the University of Oklahoma Hudson College of Public Health (OU COPH), which provided public health expertise, data analysis, and evidence-based methodology consistent with IRS standards. The assessment included participation from individuals with special knowledge and expertise in public health, including public health officials from the Oklahoma State Department of Health and academic public health professionals from OU COPH. These partners assisted in data interpretation, prioritization criteria development, and validation of findings. This CHNA covered the three-year period through FY25 with an assessment period that occurred between April 2024 and January 2025. The hospital considered input from people representing the interests of the community through community engagement meetings, advisory group discussions, and a community-wide survey distributed throughout Craig County. The survey was developed using a Social Determinants of Health framework and made available in English, Spanish, and Burmese to promote accessibility. A total of 181 surveys were completed online and mailed responses. Survey topics included access to care, housing stability, transportation, mental and behavioral health, nutrition, employment, and other factors affecting community health. Community engagement meetings included representatives from public health agencies, community-based organizations, faith-based organizations, academic institutions, and healthcare providers. Hospital representatives participated in these meetings and reviewed assessment findings. Advisory groups met throughout the process to evaluate data, identify emerging issues, and assist in prioritizing significant health needs. Special consideration was given to medically underserved, low-income, minority, and other vulnerable populations in Craig County, including uninsured individuals, rural residents, seniors, and people with disabilities. Demographic data from County Health Rankings & Roadmaps and U.S. Census sources were analyzed to identify disparities. Community input and data findings were incorporated into a defined prioritization process using criteria such as size and severity of the need, impact on vulnerable populations, feasibility, and sustainability. Based on this community-informed process, Saint Francis Hospital Vinita identified Access to Healthcare, Food Insecurity, and Employment Opportunities as the significant health needs to be addressed in its Implementation Strategy.
Schedule H, Part V, Section B, Line 6a Facility , 1Facility , 1 - Saint Francis Hospital Vinita, Inc.. The CHNA Joint Assessment was conducted by Saint Francis Health System leadership on behalf of Saint Francis Hospital Vinita.
Schedule H, Part V, Section B, Line 6b Facility , 1Facility , 1 - Saint Francis Hospital Vinita, Inc.. The CHNA Joint Assessment was conducted in partnership with the Oklahoma State Department of Health District 4 and The Oklahoma University Hudson College of Public Health (OU COPH).
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - Saint Francis Hospital Vinita, Inc.. Based on insights from community engagement meetings, surveys, and resident feedback, Craig County's most significant health-related needs were identified. These include: * Housing Stability- Homelessness, housing instability, and lack of affordable options, strongly tied to mortality, mental health, and safety. * Mental and Behavioral Health - High suicide rates, firearm fatalities, poor mental health days, and barriers to care access. * Access to Healthcare - Challenges with insurance coverage, medical bills, chronic disease management, and emergency preparedness, with disparities worsened during COVID-19. * Transportation - Limited options restricting access to food, healthcare, jobs, and social connections. * Employment Opportunities - While unemployment is relatively low, underemployment and economic instability create barriers to meeting basic needs. * Nutrition and Food Security - Barriers to affordable, nutritious food, particularly for low-income households, often linked to transportation and poverty. Following the identification of significant community health needs, Saint Francis Health System (SFHS) undertook a multi-step prioritization process involving review of survey and data findings, input from approximately 20 leaders, and discussions with Craig County leadership. While SFHS acknowledges the importance of all identified needs, three were selected as priorities for the formal CHNA implementation strategy: (1) Access to Healthcare, due to cost, insurance, and appointment barriers identified as the top concern; (2) Food Security, based on both internal screening data and survey findings highlighting affordability and preparation challenges; (3) Employment, given challenges with limited job opportunities, work experience, education and necessary skills. These prioritized needs reflect areas where SFHS is well positioned to lead or collaborate. Other significant needs identified in the CHNA, including housing stability, transportation, and mental and behavioral health, were not selected as primary priorities for the implementation strategy during this assessment cycle. While these needs remain important to community health, SFHS on behalf of Saint Francis Vinita determined that other community organizations and public agencies are better positioned to lead initiatives addressing these issues. SFHS may continue to collaborate with community partners where appropriate but will focus its direct implementation resources on the prioritized needs described above.

Additionally, a self-pay discount of 60 percent is provided on all charges for uninsured patients.
Schedule H, Part I, Line 7g Subsidized Health Services100% of Subsidized health services from worksheet 6 are allocatable to physician clinic costs.

Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountSAINT FRANCIS HOSPITAL VINITA, INC. HAS AN ESTABLISHED PROCESS TO DETERMINE THE ADEQUACY OF THE ALLOWANCE FOR UNCOLLECTABLE RECEIVABLES THAT RELIES ON A NUMBER OF ANALYTICAL TOOLS AND BENCHMARKS TO ARRIVE AT A REASONABLE ALLOWANCE. NO SINGLE STATISTIC OR MEASUREMENT DETERMINES THE ADEQUACY OF THE ALLOWANCE FOR UNCOLLECTABLE RECEIVABLES. SOME OF THEANALYTICAL TOOLS THAT SAINT FRANCIS HOSPITAL VINITA, INC. Schedule H, Part III, Line 3 Bad Debt Expense MethodologyTHE BAD DEBT EXPENSE ATTRIBUTABLE TO PATIENTS ELIGIBLE UNDER THE ORGANIZATION'S FINANCIAL ASSISTANCE POLICY IS CALCULATED FROM A SAMPLE REVIEW OF ALL BAD DEBT ACCOUNTS AND SUBSEQUENT INFORMATION. Many patients would qualify for financial assistance under the Saint Francis Financial Assistance Policy if the application process was completed, but for a number of reasons, patients are sometimes unable to complete the process. As result, their accounts end up in bad debt. For this reason, the cost associated with these accounts should be considered community benefit.Saint Francis exhausts all efforts to either collect from the patientor help them to completethe financial assistance process. After all efforts have been exhausted, Saint Francis Hospital uses data sources and quantitative analysis to identify the population of patients that, more likely than not, would have qualified for charity care had theybeen able to completethe financial assistance application. A cost to charge ratio is applied to the gross charges of the identified patients.

Schedule H, Part VI, Line 2 Needs assessmentUsing a mixed-methods approach grounded in a Social Determinants of Health framework, the CHNA incorporated a community survey, direction from a community advisory board, stakeholder engagement meetings with local leaders and organizations, and secondary data analysis using County Health Rankings and Roadmaps (CHRR). A structured governance model was established to ensure broad input and accountability. The core group oversaw survey development, data analysis, and prioritization of health needs. Advisory groups, including the local county health departments, provided additional guidance and alignment. Engagement groups, consisting of community subject matter experts and organizational stakeholders, were convened at key stages to provide context, interpret findings, and validate emerging themes. This structure ensured that community voices were central to the process and that the final CHNA reflected both quantitative data and the lived experiences of the communities served. The organization's CHNA process is comprehensive; therefore, no additional assessments are undertaken.
