Southern New Hampshire Medical CenterPart V, Section B, Line 5: Southern New Hampshire Medical Center worked with officials from the City of Nashua Division of Public Health and other members of the community, including local police and fire departments, school officials, ambulance service leaders, directors of community agencies, hospital leaders, physicians and area residents through focus groups to gather input for the community health needs assessment (CHNA).A Community Health Assessment is a vital tool for understanding and improving the health outcomes of our community. By leveraging data from local organizations, healthcare systems, and state agencies, we gain valuable insights into where to focus our efforts. However, our commitment to community health extends beyond data collection. We believe in the power of listening. That's why we hosted listening sessions and distributed surveys to hear directly from community members about their health concerns, experiences during the COVID-19 pandemic, and what they love about their community. The 2023 CHA employed numerous research methods to gather input from the community, including online surveys, in-person interviews, and data analysis. The CHA utilized these methods separately and decisively for each of its identified community health needs. The City of Nashua Division of Public Health and Community Services (DPHCS) utilized listening sessions, community surveys, and online polling to invite Greater Nashua residents to help determine what the biggest health priorities were in their communities, environmental health hazards, and socail and economic factors that effect the populations's health. The DPHCS also regularly published health-related information and shared sources for where additional information regarding health matters such as COVID-19 could be obtained.Through these efforts, the CHA found mental health, homelessness, substance use, access to care, air and water quality, inequities in health, access to food, public safety, education, and child abuse and neglect to be the top-ten biggest health issues of the community.
Southern New Hampshire Medical CenterPart V, Section B, Line 6a: Southern New Hampshire Medical Center took part in the 2023 Greater Nashua Community Health Assessment for its most recent Community Health Needs Assessment ("CHNA"). This Assessment was conducted by the Nashua Department Division of Public Health and their Public Health Advisory Council partners. It is a product of the dedication and collaboration of many organizations, programs, and City departments serving the GNPHR. A full list of the hospital facilities that participated in this Assessment can be found through the City of Nashua Community Health Assessment homepage at:https://nashuanh.gov/560/Community-Health-Assessment
Southern New Hampshire Medical CenterPart V, Section B, Line 6b: Southern New Hampshire Medical Center took part in the 2023 Greater Nashua Community Health Assessment for its most recent Community Health Needs Assessment ("CHNA"). This Assessment was conducted by the Nashua Department Division of Public Health and their Public Health Advisory Council partners. It is a product of the dedication and collaboration of many organizations, programs, and City departments serving the GNPHR. Southern New Hampshire Medical CenterPart V, Section B, Line 11: Southern New Hampshire Medical Center coordinates with the City of Nashua Division of Public Health and Community Services (DPHCS), as well as other community agencies, partners, and members to create a collaborative and comprehensive Community Health Improvement Plan (CHIP). The CHIP is published in coordination and in response to the Greater Nashua Department of Health CHA. Southern New Hampshire Medical Center also independently prepares and publishes its own Community Health Needs Assessment and Implementation Strategy, unique to Southern New Hampshire Health and SolutionHealth. The Greater Nashua PHAC Executive Committee and CHIP Planning Team chose the following health priority areas for the CHIP 2022 (the most recently published CHIP) based on the 2020 Greater Nashua CHA: 1. Behavioral Health (Suicide, Mental Health, and Substance Use) 2. Chronic Disease (Heart Disease/ Stroke, Diabetes, and Asthma) 3. Communicable diseases 4. Maternal and Child Health 5. Public Health Emergency PreparednessBEHAVIORAL HEALTH:Behavioral health is the scientific study of the emotions, behaviors, and biology relating to a person's mental well-being, their ability to function in everyday life, and their concept of self. Behavioral health encompasses mental health, substance misuse, and suicide prevention and is a top priority in the Greater Nashua Public Health Region (GNPHR) for a variety of reasons. These reasons include our region's high prevalence of substance misuse, high rate of opioid overdose fatalities, high rate of suicide attempts and deaths by suicide, lack of access to timely treatment for those seeking behavioral health services, and limited mental health promotion and awareness. Approximately one in eight visits to emergency departments in the United States (U.S.) involves mental and substance use disorders.Collectively, the main objectives under Behavioral Health include: 1) Reduce substance use among all age groups (illicit Substances, alcohol, tobacco, marijuana 2) Increase awareness of behavioral health resources to improve or maintain mental health in all age groups 3) Improve access to substance use and mental health services through workforce developmentCHRONIC DISEASESApproximately six in ten Americans live with at least one chronic disease, such as heart disease and stroke, cancer, or diabetes. Many chronic diseases are caused by a variety of risk factors that can be reduced by making healthy lifestyle choices. Not only the leading cause of death and disability, chronic diseases are also the leading driver of health care costs in the United States (US). It is a public health priority in the Greater Nashua Public Health Region (GNPHR) to provide health promotion and education to help our community prevent chronic diseases and manage symptoms when prevention is not possible to reduce the burden of chronic disease on our society.Collectively, the main priorities under chronic disease include: 1) Increasing prevention efforts for Type II diabetes 2) Increasing control of hypertension 3) Increase identification and treatment of depression for those with chronic diseaseCOMMUNICABLE DISEASESCommunicable, or infectious diseases, are caused by microorganisms such as bacteria, viruses, parasites and fungi that can be spread, directly or indirectly, from one person to another. Transmission can occur in a variety of ways, including direct physical contact with an infectious person, consuming contaminated foods or beverages, contact with contaminated body fluids, contact with contaminated inanimate objects, or being bitten by an infected insect. In some cases, pathogens that result in infectious diseases can become airborne and spread via the air we breathe (i.e., through inhalation). Some disease agents can be transmitted from animals to humans, and some of these agents can be transmitted in more than one way.Collectively, the main priorities under communicable disease include: 1) Increase vaccination uptake for vaccine preventable diseases 2) Increase community preparedness for communicable disease outbreaks 3) Increase effectiveness of public health system in preventing and addressing communicable disease.MATERNAL AND CHILD HEALTHMaternal health is the health of a woman during her pregnancy, birth, and postpartum period, making it an important predictor of newborn health. Children's health is the physical, mental, emotional, and social well-being of children from infancy all the way through adolescence. Many factors affect maternal and child health, including race and ethnicity, age, income level, educational attainment, medical insurance coverage, access to medical care, pre-pregnancy health, and general health status. The well-being of infants, children, and mothers determines the health of our next generation and can help predict future public health challenges for families, communities, and the healthcare system.Health behaviors and health status before a woman is pregnant can be influenced by a variety of environmental and social factors, such as access to medical care and chronic stress. Some of these factors can affect and compound others, creating a rippling effect. For instance, factors ranging from age to medical insurance coverage affect a woman's general health status; a woman's health status, in turn, directly influences her risk of pregnancy complications and her child's cognitive and physical development.Main takeaways from the Maternal and Child Health CHA: - In 2018, 93.7% of NH moms reported ever breastfeeding, and 69% reported breastfeeding for more than 8 weeks. According to the CDC, only one in four infants in the U.S. are exclusively breastfed for six months. - Children in the GNPHR are at increased risk of high blood lead levels, as 17% of homes were built before 1950, and 10.8% of six year olds are living below the federal poverty level. - In 2018, 7.5% of pregnant women in NH smoked cigarettes in their last three months of pregnancy, and 5.9% used marijuana or hash during their pregnancy. - In 2018, 14.9% of women in NH received follow-up care for depression after giving birth, and 13% women were diagnosed with depression after giving birth. - Approximately 4.6% of Nashua youth (aged 6-18) are uninsured.Collectively, the main priorities under Maternal and Child Health include: 1) Improve access and availability of universal screenings 2) Support access to affordable healthcare and referrals for families 3) Support children's social & emotional development through easily accessible services & supports 4) Improve access and supports for healthy nutrition 5) Increase the capacity of all families to advocate for themselves and their childrenPUBLIC HEALTH EMERGENCY PREPAREDNESSFrom natural disasters to terrorism, all crises have the potential to negatively impact public health. The goal of Public Health Emergency Preparedness (PHEP) is to increase a community's ability to respond in an efficient manner to incidents in order to preserve health and wellbeing of the population. There are 15 functional areas that encompass PHEP as outlined in the CDC's Public Health Emergency Preparedness and Response Capabilities.Main takeaways from the PHEP CHA 2020: - Fewer than 12% of households in Nashua have a written disaster plan. - 97.8% of Nashua households have a smoke detector. - 19% of Nashua households feel well prepared to handle an emergency. - 40% of U.S. adults don't consider themselves prepared but plan to prepare for emergencies and 68% of U.S. adults have set aside some money for an emergency.Collectively, the main priorities under PHEP include: 1) Improving the regional communications structure 2) Increasing public health planning, exercising and training among various sectors 3) Strengthening the working relationship between the region (GNPHR) and the State of New HampshireThe full 2022 CHIP can be found through the following website, in either an interactive web-based format or as a readable PDF:https://nashuanh.gov/564/Community-Health-Improvement-Plan-CHIPThe Southern New Hampshire Health and SolutionHealth Implementation Strategy echos many of the ideas and conclusions of the Greater Nashua Health Department CHIP, focusing on these core areas: 1) Health Behaviors - Tobacco Use, Diet & Exercise, Alcohol & Drug Use, Sexual Activity. 2) Clinical Care - Access to Care, Quality of Care. 3) Social & Economic Factors - Education, Employment, Income, Family & Social Support, Community Safety. Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I, Line 3c:Southern New Hampshire Medical Center offers a discount equal to the average of their three largest payors plus Medicare to all self-pay patients. Discounted care is provided to all uninsured and underinsured patients.
Part I, Line 7:The costing methodology used was the ratio of cost to charge as outlined in Worksheet 2 of the instructions. The only exception to this was for subsidized service area where the cost structure was specific to that service area. For those areas, our Cost Accounting system was used.
Part II, Community Building Activities:Southern New Hampshire Medical Center is committed to improving the health and well-being of the Greater Nashua area and our community building activities help us achieve this goal. The board of directors is made up of community members with varied backgrounds and industries. The Hospital utilizes any surplus funds to further its mission by providing free and low-cost healthcare services, educations offerings, free screenings, support groups, and works collaboratively with other healthcare agencies to meet the identified healthcare needs of our community. Furthermore, executives of Southern New Hampshire Medical Center have been recognized for visionary leadership and their commitment to public health and healthcare excellence. The Medical Center also regularly publishes practical guidance and healthy living instructions online, as well as offering in person classes and seminars. For example, the Medical Center has begun offering instructions on the connection between mental health, depression, and weight gain; and the Medical Center routinely publishes materials on healthy eating and exercise. Additionally, Southern New Hampshire Medical Center deploys a pool of volunteers as well as its own staffing resources to further community engagement and improvement initiatives, through causes such as Holiday food drives, sexual health awareness campaigns, grief and mental health support services, and community events to bring citizens together and to fight the loneliness epidemic. Southern New Hampshire Medical Center makes an intentional effort to ensure its community and public programs reach a wide array of individuals in its service area, providing instructions, resources, and services to persons of all ages, race, or gender.
Part III, Line 2:Southern New Hampshire Medical Center's Bad Debt Expense includes accounts that have been referred to a collection agency after making reasonable attempts to determine eligibility under the financial assistance policy, along with estimates of current accounts receivable based in historical experience.
Part III, Line 3:A significant portion of Southern New Hampshire Medical Center's uninsured patients will be unable or unwilling to pay for the services provided. Thus, Southern New Hampshire Medical Center records a significant provision for bad debts related to uninsured patients in the period the services are provided. The FASB issued an accounting standards update that states that a health care entity that recognizes significant amounts of patient service revenue at the time services are rendered, even though it does not assess the patient's ability to pay, must present bad debts as a reduction of net patient revenue and not as a separate item in the operating expense. The ratio of cost to charge methodology was used to determine costs of these services as outlined in worksheet 2 of the instructions. Consistent with the accounting treatment mentioned above, since the system does not assess the patient's ability to pay, the cost of bad debt for those patients without any form of insurance should be classified as community benefit, consistent with charity care.
Part III, Line 4:See Footnote 2 of the audited financial statements for disclosures regarding the Southern New Hampshire Health System's significant accounting policies.
Part III, Line 8:The ratio of cost to charge methodology was used to determine cost as outlined in Worksheet 2 of the instructions. There are service areas where government programs such as Medicare and Medicaid do not reimburse the organization adequate amounts to cover the costs to provide those services. Southern New Hampshire Medical Center must provide those services regardless of the financial shortfall.
Part III, Line 9b:Southern New Hampshire Medical Center recognizes that not all patients who qualify for financial assistance will follow through with an application for financial assistance. Prior to placing any account with a collection agency, Southern New Hampshire Medical Center uses a financial assistance qualification service to categorize patients based primarily on income and assets estimated from census tract data. Those patients whose estimated income/assets fall within Southern New Hampshire Medical Center's financial assistance guidelines are awarded charity or free care assistance for their encounter.
Part VI, Line 2:Southern New Hampshire Medical Center worked with the City of Nashua Division of Public Health officials and other members of the community, including local police and fire departments, school officials, ambulance service leaders, directors of community agencies, hospital leaders, physicians and area residents through focus groups to gather input for the Community Health Needs Assessment (CHNA).A Community Health Assessment (CHA) is a collaborative process that identifies key health needs and issues through systematic, comprehensive data collection and analysis. CHAs provide information on a variety of health topics and help identify resources which assist with policy formulation, program implementation, and evaluation. CHAs are also a crucial component of accreditation as they help measure how well a public health system is fulfilling the assurance function, one of the three fundamental purposes of public health. The Greater Nashua CHA is part of an ongoing comprehensive community health improvement process. The purpose of the CHA is to identify vulnerable populations using comparable data within the Greater Nashua Public Health Region (GNPHR) and to subsequently identify trends in health issues, environmental health hazards, and social and economic factors that affect the populations' health.Collected and analyzed data will be used to identify priority issues within the GNPHR and develop strategies for further action which are outlined in the form of a community health improvement plan (CHIP). The CHIP creates a foundation for a work plan to improve the health of the community over the course of three years' time. One of the main objectives of the most recent Community Needs Assessment's research was to determine the biggest health concerns in the community in order to determine community health priorities. The research for this report indicated that most people in the Greater Nashua Region are concerned about mental health, homelessness, substance use, access to care, air and water quality, inequities in health, access to food, public safety, education, and child abuse and neglect to be the top-ten biggest health issues of the community.
Part VI, Line 3:Southern New Hampshire Medical Center complies with the Southern New Hampshire Health financial assistance policy. Southern NH Health is committed to providing charitable care to persons who have health care needs and are uninsured, underinsured, and ineligible for governmental programs and are otherwise unable to pay for medically necessary care based on their individual financial situations. Southern NH Health strives to ensure that the financial capacity of people who need health care services does not prevent them from seeking or receiving care. Charitable care is not considered to be a substitute for personal responsibility and patients are expected to cooperate with Southern NH Health's policies and procedures for obtaining charitable care and to contribute to the cost of their care based on their individual ability to pay. Patients with the financial capacity to purchase health insurance shall be encouraged to do so, as a means of access to health care services, for their overall personal health and protection of their individual assets. Since July 2010, the State of New Hampshire has mandated that any uninsured patient who receives nonelective, medically necessary hospital services are billed an amount no greater than the amount generally received from patients covered by insurance. Additionally, the 2015 Federal 501(r) regulations further require that when calculating this discount, Medicare reimbursement must be included. Therefore, Southern NH Medical Center reduces the amounts owed by uninsured patients by the uninsured discount prior to the patients receiving their first statements of balances due. These discounted balances, and not gross charges, are the balances upon which financial assistance is based, granted and classified for reporting purposes. The Patient Protection and Affordable Care Act of 2010 (PPACA) mandates insurance coverage for individuals starting on January 1, 2014. Additionally, it expands Medicaid coverage to all individuals at or below 100% of the Federal Poverty Level (FPL). The New Hampshire Legislature approved Medicaid expansion effective August 15, 2014. The Granite Advantage Health Care Program (GAHCP) extends NH Medicaid eligibility to all individuals at or below 138% of the Federal Poverty Level. The granting of charitable care shall be based on an individualized determination of financial need, and shall not take into account age, gender, race, social or immigrant status, sexual orientation or religious affiliation. At each registration point, Southern New Hampshire Medical Center has signage and brochures explaining our various forms of financial assistance, including charity care. Additionally, patient registrars offer the charity brochures to self pay patients. Patients who call regarding billing concerns are offered charity assistance. Additionally, verbiage regarding charity care is printed on each billing statement the patient receives. Also, charity care information is available on our hospital website. There is staff available on site at the hospital to assist with financial assistance, Medicaid, and Expanded Medicaid applications. We actively participate in Medicaid Exchange programs and a Prescription Assistance Program to help those with financial needs.Additional information regarding Southern New Hampshire Medical Center's financial assistance policies can be found on the Southern New Hampshire Health website, as provided on this Schedule H.
Part VI, Line 4:Southern New Hampshire Medical Center serves people of all ages, race, gender, religion, ethnicity regardless of their ability to pay. Southern New Hampshire Health's service area consists of nineteen New Hampshire and four Massachusetts towns. The service area is divided into a Primary Service Area (PSA) of twelve New Hampshire towns (Amherst, Brookline, Hollis, Hudson, Litchfield, Lyndeborough, Merrimack, Milford, Mont Vernon, Nashua, Pelham, and Wilton), a Secondary Service Area (SSA) of seven New Hampshire towns (Derry, Greenville, Londonderry, New Boston, New Ipswich, Salem, and Windham) and a Massachusetts Service Area (MSA) of four neighboring Massachusetts towns (Dunstable, Pepperell, Townsend, and Tyngsboro). For its 2024 tax year (fiscal year ending June 2025), Southern New Hampshire Medical Center continued to refer and rely on the collaborative Greater Nashua 2023 Community Health Assessment that was prepared and informed in association with the City of Nashua. The four geographies mentioned most often throughout this report are the City of Nashua, the Greater Nashua Public Health Region ("GNPHR"), Hillsborough County, and the State of New Hampshire (NH). Part VI, Line 5:Southern New Hampshire Medical Center provides a variety of services that promote the health of the greater Nashua community. Throughout the year we hold Community Medical Education seminars that offer area residents an opportunity to look behind the scenes of medicine and learn more about the latest medical technology, research and advances. We also perform free public health screenings, such as impact testing for sports teams at local schools, and balance and hearing screenings for the elderly. Our commitment to community health is also evident in the time our leadership spends on the boards of several local health organizations to help improve and promote health services available to area residents. While the COVID-19 Pandemic naturally came to the forefront of Southern New Hampshire Medical Center's community health and care programs over some of its most recent years, Southern New Hampshire Medical Center continued to engage its community and to promote the wellbeing of its fellow citizens throughout the year on a variety of levels. For example, Southern New Hampshire Medical Center continued to focus its attention on educating, maintaining, and supporting a highly educated and competent workforce. Additionally, Southern New Hampshire Medical Center continued to advocate for community safety, nutrition, and personal wellness.Additionally, Southern New Hampshire Medical Center continues to focus its community health promotion efforts on the core health concern causes and risk factors identified in its community health assessments and implementation strategies. This includes efforts and services to address: mental and behavioral health; access to care; health education; and social, economic, and physical environmental factors. For example, in furtherance of these causes, Southern New Hampshire Medical Center routinely publishes resources that advise on matters of weight, dietary health, exercise, sexual health, aging in excellence, the value of social connectedness, cancer risks, and childhood health initiatives. The Medical Center provides these resources both online and in-person and ensures such community health initiatives are accessible to the diverse range of populations within the Medical Center's community.
Part VI, Line 6:Southern New Hampshire Health System, Inc. is a not-for-profit entity organized to support Southern NH Medical Center and Foundation Medical Partners, Inc.
