Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - CHATHAM HOSPITAL, INC.. For this assessment, the Chatham County Community Survey (CCCS) was implemented to take into account input from a broad representation. The CCCS was conducted using the Community Assessment (CA) cohort, which was initiated in 2018 through a structured protocol approved by Chatham County Public Health Department (CCPHD) and the Scientific Advisory Committee (SAC). In conducting the 2024 CCCS, there was an opportunity to revisit the original sampling scheme and modify the sampling scheme to reflect recent changes in the Chatham County population using the most current data available from the U.S. Census Bureau. Additionally, CCPHD wanted to expand the cohort to replace those lost to follow-up and to recruit more participants for better representation from Black/African American and Hispanic/Latinx community members and residents in rural areas of the county. The sampling methodology was modified first to account for changes in the median household income across census block groups in Chatham County using the 2019 American Community Survey 5-year estimates. Using this data, the census blocks were grouped into three categories, low, medium and high incomes, representing the percentiles of the median household income distribution across census blocks. To ensure an even number of census blocks in each stratum and to expand the CA cohort, 15 additional census blocks were selected for a total of 76 census blocks included in the CA cohort sampling. The additional blocks were selected with probability proportionate to the Black and Hispanic or Latino population, based on 2020 Decennial Census population estimates adapted to existing census block boundaries. For each newly sampled census block, seven households randomly sampled from residential address data, and for the originally sampled blocks with existing cohort members, replacement households were randomly sampled and added as necessary within each block to obtain a total of seven households in each block. In March 2024, households were contacted with two mailings - a postcard followed by a mailed survey packet containing information about the CCCS, a paper copy of the survey, and a pre-addressed stamped envelope to mail the completed survey back to CCPHD. All communications and the CCCS survey were available in both English and Spanish. Starting in mid-April 2024, teams of CCPHD staff and volunteers went door-to-door to collect surveys from selected households. This continued through May 2024. In total, 411 eligible responses were received through drof-off, mail, online, phone, and in-person data collection. The majority (253) were collected through door-to-door surveying. In addition to the cohort survey, a short supplemental survey was offered to all members of the Chatham County community as an opportunity for those not selected as part of the sample to participate and share thoughts about their experiences with health and well-being in Chatham County.
Schedule H, Part V, Section B, Line 6b Facility , 1Facility , 1 - CHATHAM HOSPITAL, INC.. THE ORGANIZATION'S COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA) WAS CONDUCTED WITH THE FOLLOWING ORGANIZATIONS OTHER THAN HOSPITAL FACILITIES: CHATHAM COUNTY PUBLIC HEALTH DEPARTMENT AND CHATHAM HEALTH ALLIANCE.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - Chatham Hospital, Inc.. The Hospital has identified the following three health priorities: Healthcare Access Healthcare access, including physical health services, mental health services, and other support services and resources, are hugely impactful on the health of an individual and the community. Lack of access due to cost, insurance status, language, or other barriers ranging from complex referral systems to lack of available services altogether can prevent many from attaining their best health; this access is also unequal among all members of the community, which can create or worsen existing health disparities. Nearly 4 in 10 Chatham County households (37.3%) faced some barrier to healthcare in 2024, especially primary care and dental care. A recurring theme across multiple focus groups conducted as a part of the Community Assessment was the need for expanded healthcare in the county. Activities for Youth and Young Adults Chatham County has an estimated population of 81,624 people as of 2023. Of them, 19% of those are under 18 years old and 14.6% are between 6 years old and 18. Community members advocated for more recreational and entertainment activities for youth, especially teenagers. Chatham County has transportation issues that also compound the dearth of activities for youth and young adults. Only 1.3% of respondents said they use public transportation to get around, meaning youth who do not have driver's licenses or cars may have difficulty getting to activities or places within the county, even of such activities or places were more available. There is also an intersection between activities for youth and other domains like mental health. Activities can help youth and students build their social emotional skills, as youth feel more involved and build social connections which can help their emotional well-being. Substance Use: Treatment and Support From the year 2000-2022, more than 36,000 North Carolinians died due to a drug overdose. The devastating effects of the opioid epidemic can be felt in every state, and North Carolina is not an exception. The overdose death rate per 100,000 residents in Chatham County was 24.5 in 2023, representing 20 people who died of an overdose that year. This was a decrease from 2022, when the overdose death rate in Chatham peaked at 37.1. It is also below the state rate of 41.0. Of those 20 deaths, 70% of them were illicit opioid overdoses, representing 14 people. In 2024 Chatham County had 11 fentanyl-positive deaths, of which all were under age 55. Males are over-represented based on population numbers compared to females in deaths. The need for more treatment options and support for people who use substances arose as theme across several focus groups conducted for the Community Assessment. UNC Health Chatham's strategic plan to address the identified health priorities: 1. UNC Health Chatham leadership will continue as a member of the Chatham Health Alliance Steering Committee. 2. A UNC Health Chatham leader will continue as a member of the Chatham Health Alliance. 3. UNC Health Chatham will have representation on Healthcare Access committee and ensure collaboration with the other two committees, particularly Substance Use Treatment and Support. 4. UNC Health Chatham will continue to collaborate with Chatham County Health Department on community health needs. 5. UNC Health Chatham will continue to collaborate with UNC Physicians Network on primary and specialty care services for Chatham County. 6. UNC Health Chatham will collaborate with the Chatham County Health Alliance to ensure clear metrics of success are created and monitored for each priority health need. This three-year Community Health Needs Assessment (CHNA) Strategic Implementation Plan (SIP) was presented and adopted by the Chatham Hospital Board of Directors on June 17, 2025 with the vision that UNC Health Chatham will use the plan as a road map to collaborate with the Chatham County Health Alliance and Chatham County Public Health Department to track and assess progress towards meeting the community health goals. Chatham CHNA task force will make adjustments along the way enabling collaboration with community partners while responding to local conditions and changes in community priorities and leadership. UNC Health Chatham will evaluate progress at the end of each calendar year and modify the plan accordingly. Priorities identified in the previous Community Assessment remain concerns, including Mental Health and Substance Use, Transportation and Employment along with Childcare, Built Environment, Healthy Aging, and Healthy Food Systems. While Chatham Hospital recognizes these health needs, their resources are better used in addressing the top three health priorities listed above.
Schedule H, Part V, Section B, Line 13 Facility , 1Facility , 1 - CHATHAM HOSPITAL, INC.. The Financial Assistance Policy implemented non-income based financial assistance criteria as of 1/1/2025. The non-income eligibility criteria include homelessness, mental incapacitation with no one to act on the patient's behalf, patient or child in patient's household enrolled in any Medicaid or Managed Medicaid insurance, and enrollment in another means-tested public assistance program including, but not limited to: Women, Infants and Children Nutrition Program (WIC), Supplemental Nutrition Assistance Program (SNAP) or Temporary Assistance for Needy Families (TANF).
