Our organization has been striving to serve the needs of our region since we were established. We here at Carroll County Memorial Hospital are loyal to our community and will continue to strive for excellence in healthcare. Being part of the community has been an important goal throughout the years, and with your help and loyalty we can continue to provide health services that you can depend on.The Hospital provides financial assistance to patients with limited to no ability to pay for the healthcare services they receive. Patients who qualify for patient financial assistance are given a discount from established charges through the completion of the application for financial assistance based on the most recent published poverty guidelines. The Hospital determines the costs associated with providing patient financial assistance by aggregating the direct and indirect costs, including salaries, benefits, supplies and other operating expenses based on the overall cost-to-charge ratio. The costs of caring for these patients for the year ended Jun 30, 2025 was approximately $356,000.Desc

Carroll County Memorial HospitalPart V, Section B, Line 5: The community health needs assessment was conducted through a partnership with the Carroll County Health Department. The CHNA community survey consisted of 36 questions offered electronically and on paper. The CHNA survey opened on February 9, 2024, and was closed on March 10, 2024. Overall, 335 surveys were completed. The aggregated and analyzed results were provided as a primary data source during the community forum and were an important part of the prioritization process when identifying new health needs priorities. The community forum consisted of community members that represented the medically underserved, low-income, and minority populations in the community served by the hospital. The organizations included: MVCAA, HELP Services, HCC Network (CHW), Burrell Behavioral Health, Sheriff/Probation & Parole, Ministerial alliance/community faith organizations, public schools, pharmacy partners, and others regularly serving low income, uninsured/underinsured, seniors, and youth at risk.
Carroll County Memorial HospitalPart V, Section B, Line 6b: The CHNA was conducted in collaboration with the Carroll County Health Department and supported by Quality Works (Missouri Hospital Association subsidiary) for technical assistance.
Carroll County Memorial HospitalPart V, Section B, Line 11: The community health needs assessment (CHNA) was completed in fiscal year 2025. The following needs were identified.1) Social Determinants/Drivers of Health Transportation, Housing, Internet Access, Poverty2) Mental Health & Substance Use3) Chronic Diseases & Obesity (heart disease, diabetes, cancer)The Implementation Strategy notes that while not every issue identified in the CHNA is included in the plan, CCMH and CCHD will continue to support broader efforts. The three selected priorities were chosen using magnitude, severity, disparities, community assets, and ability to leverage collaborations. Certain needs were not prioritized due to limited resources, existing community capacity to lead specific efforts, or stronger upstream impact of selected priorities (e.g., SDOH focus to influence multiple unmet needs).CCMH initiated and/or advanced multiple implementation activities across all three CHNA priorities - SDOH, Mental Health & Substance Use, and Chronic Disease & Obesity with concrete outputs (meetings held, materials distributed, screenings delivered, youth reached, employer engagement, extended clinic hours) and defined measures to monitor impact over the 2025-2027 cycle.Social Determinants of Health (Transportation, Housing, Internet Access, Poverty)- Formed an SDOH Focus Group as an extension of existing community task forces - Updated and improved the Community Resource Guide, creating QR code flyers and business cards and distributing at least 100 copies, with additional emphasis on digital sharing.- Maintained and distributed transportation resources through CCMH's Population Health team; provided transportation assistance through the CCMH Foundation's Hope Fund.- Continued implementation of the Mobile Integrated Health community paramedicine program, including early stage care coordination work with the Ambulance District and local pharmacies; collaborated with community partners addressing housing, utility assistance, and family support.- Supported community internet access through the Carrollton Public Library, which offered free internet and hot spot lending.- Expanded support for uninsured and underinsured residents through quarterly Community Health Worker assistance at the Health Department and through material support such as school shoe donations.Mental Health & Substance Use- Provided mental health and substance use prevention activities at First Friday events, including interactive booths and onsite health screenings in May 2025.- Delivered workforce ethics training to 70 local youth in collaboration with employers and the Chamber of Commerce in June 2025.- Initiated advisor outreach to school based leadership groups (FBLA, FCCLA, Star Leaders) to establish peer to peer mental health programming for upcoming school years.- Expanded behavioral health access pathways through partnerships with Burrell Behavioral Health, supported by new regional crisis center capacity.Chronic Disease & Obesity (Heart Disease, Diabetes, Cancer)- Delivered the Strength in Numbers community education series from February to May 2025, with 27 participants completing all four sessions and 40 completing at least one session.- Conducted 344 community blood pressure screenings at senior housing sites and the local grocery store as of March 2025.- Promoted healthy living environments through participation in community garden plots maintained by CCMH staff and through walking trail improvements supported by completed fundraising.- Provided employer sponsored wellness programming to 265 participants (employees and spouses) with a 98% engagement rate.- Expanded Medication Therapy Management efforts through pharmacy partnerships and supported additional preventive services such as monthly blood draws and chronic illness education.- Advanced Chronic Care Management enrollment and collaborated with the Ambulance District and behavioral health partners to reduce avoidable emergency room readmissions.
Carroll County Memorial HospitalPart V, Section B, Line 13h: Catastrophic medical cases where a patient/guarantor who has suffered a catastrophic medical incident resulting in medical bills that exceed one's annual income, or if in any given three (3) consecutive months, whose medical bills exceed 20% of their gross income plus assets for the year; thereby concluding that one's unpaid hospital charges exceed their ability to pay. The balance owed of these patients will be subject to a sliding scale and partial assistance may be applied. In the event there is no evidence to support a patient's eligibility for financial assistance, Carroll County Memorial Hospital could use outside agencies in determining estimate income amounts for the basis of determining financial assistance eligibility and potential discount amounts. Once determined, due to the inherent nature of the presumptive circumstances, the only discount that can be granted is a 100% write-off of the account balance. Presumptive eligibility may be determined on the basis of individual life circumstances which may include: State-funded prescription programs; Homeless or received care from a homeless clinic; Participation in Women, Infants and Children programs (WIC); Food stamp eligibility; Subsidized school lunch program eligibility; Eligibility for other state or local assistance programs that are unfunded (e.g., Medicaid spend-down); Low income/subsidized housing is provided as a valid address; Patient is deceased with no known estate; and Patient has declared bankruptcyActive Out of State Medicaid

Part I, Line 3c:Catastrophic medical cases where a patient/guarantor who has suffered a catastrophic medical incident resulting in medical bills that exceed one's annual income, or if in any given three (3) consecutive months, whose medical bills exceed 20% of their gross income plus assets for the year; thereby concluding that one's unpaid hospital charges exceed their ability to pay. Part I, Line 7:Charity care expense was converted to cost on line 7a based on an overall cost-to-charge ratio addressing all patient segments. Unreimbursed Medicaid on line 7b was calculated using the costing methods to prepare the cost reports. The cost for subsidized health services on line 7g was determined using the Medicare cost report.
Part III, Line 2:The amount on line 2 represents implicit price concessions. The Organization determines its estimate of implicit price concession based on its historical collection experience with this class of patients.
Part III, Line 3:The estimated amount of the Organization's implicit price concessions attributable to patients eligible under the Organization's charity care policy is calculated based on an approximate 16.4% of individuals living below the federal poverty level in the service area.
Part III, Line 4:The footnote to the organization's financial statements that describes implicit price concessions is located in Footnote 1 on pages 12 and 13 of the attached financial statements.
Part III, Line 8:Services are provided to patients under the Medicare program even though the hospital knows they will not recover all of the costs from providing the services. This program is considered subsidized by the hospital and other payors. Patients rely on these services for their health and well-being and are essential for their access to healthcare. Total revenue received from Medicare is the gross reimbursement plus settlement. Both total revenue received from Medicare and the Medicare allowable costs are reported from the Medicare Cost Report. The Medicare Cost Report is completed based on the rules and regulations set forth by Centers for Medicare and Medicaid Services.Hospital services remibursed on a fee schedule are not included in the Medicare calculations per the 990 instructions. Had this been reported the Hospital would have had a greater loss from Medicare services of $47,806.Medicare fee schedule revenue $37,639Medicare estimated costs of care relating to payment ($85,445) Net surplus (shortage) ($47,806)
Part III, Line 9b:The Hospital's collection policy includes specific protection for patients eligible for financial assistance. At least 30 days before any referral to an external collection agency, the Hospital provides a written notice describing available payment options, explaining the availability of financial assistance and how to apply, and stating that the account may be referred to collections if unresolved. No extraordinary collection actions are initiated before 120 days from the first post discharge billing statement. The Hospital accepts and processes financial assistance applications for 240 days after the first post discharge billing statement, and all collection activity is suspended during the review of any application. Complete applications receive a determination within 30 days; incomplete applications receive written notice identifying missing information. The Hospital also evaluates patients for presumptive eligibility based on participation in certain assistance programs, homelessness, bankruptcy, residence in a Health Professional Shortage Area (HPSA), CHNA identified underserved status, or similar circumstances. Presumptively eligible patients may receive a 100% write off of account balances. In addition, an account may be designated as bad debt only after all collection procedures have been followed and all FAP protections have been applied. Before an account is classified as bad debt: (1) all third party payers must have adjudicated the claim; (2) applicable self pay discounts must be applied; (3) the patient must have been offered and subsequently defaulted on a reasonable payment plan; and (4) the patient must have been offered the opportunity to apply for financial assistance prior to any extraordinary collection actions.
Part VI, Line 2:In addition to the triennial Community Health Needs Assessment (CHNA), the Hospital continuously assesses needs using the same framework as the CHNA. Throughout the year, CCMH reviews internal utilization and quality data (e.g., ED use, readmissions, access to primary care, behavioral health demand), Financial Assistance and Patient Advocate data on barriers to care, and social determinant indicators. CCMH gathers qualitative input via ongoing meetings with local public health, school districts, social service agencies, and community coalitions, as well as patient and caregiver feedback. The Hospital compares local indicators to state and national benchmarks used in the CHNA and applies the same prioritization criteria to identify emergent needs. Findings are reported to leadership and used to update implementation activities between CHNA cycles, ensuring services remain responsive to the community.
Part VI, Line 3:The Hospital informs and educates patients about their eligibility for financial assistance and government programs through multiple methods. The Hospital posts conspicuous notices and summaries of its Financial Assistance Policy (FAP) in admissions areas, emergency department waiting areas, registration desks, and other locations where patients are likely to be present. Copies of the full policy, a plain language summary, and financial assistance applications are available at all registration points, at the Patient Advocate offices, by mail upon request, and on the Hospital's website.During the intake and registration process, patients receive information about the availability of financial assistance and contact information for obtaining help with the application process. The Hospital also includes a plain language summary of the FAP, along with financial assistance contact information, in patient billing statements and with certain discharge materials.Patient Advocates discuss with patients the availability of federal, state, and local government benefits, such as Medicaid, and assist them in applying when appropriate. All communication is provided in clear, easy to understand language, and staff proactively review whether patients may qualify for public assistance programs or the Hospital's FAP.
Part VI, Line 4:Carroll County Memorial Hospital serves the residents of Carrollton, Missouri, the surrounding communities of Carroll County, and patients seen at its satellite clinics in Brunswick and Hale. The county is predominantly rural, with a 2024 population estimate of 8,414 residents, of which approximately 23% are age 65 and older, reflecting an aging population. The median age in Carroll County is 43.6-43.7 years, which is higher than state and national averages. The county's population is primarily White (approximately 93-95%).Economically, Carroll County has a median household income of approximately $61,712 and a per capita income of approximately $33,666-$35,161. Poverty affects about 12.9-13.7% of residents. These economic factors contribute to significant need for accessible healthcare and financial assistance services. The county contains roughly 4,380-4,416 housing units and 3,443-3,522 households, with most households consisting of approximately 2.4 persons. The median value of owner occupied housing is approximately $110,500. Educational attainment in Carroll County includes 89.7% of adults holding a high school diploma and 20.2% holding a bachelor's degree or higher. As a rural county with an aging population, modest household incomes, and above average poverty levels, Carroll County relies heavily on accessible medical care and financial assistance options. Carroll County Memorial Hospital provides essential services to this geographically dispersed and socioeconomically diverse population, helping ensure access to care regardless of ability to pay.
Part VI, Line 5:Carroll County Memorial Hospital (CCMH) furthers its exempt purpose by promoting the health and well being of the communities it serves through governance structure, community responsive clinical services, public health initiatives, community partnerships, and reinvestment of surplus funds into healthcare access and quality.Community Based GovernanceA majority of the Hospital's governing body consists of individuals who reside within the Hospital's primary service area and who are not employees, contractors, or family members of such individuals. This community based board structure ensures that Hospital decisions reflect the needs, priorities, and concerns of local residents and supports the Hospital's charitable mission.Open Medical StaffCCMH extends medical staff privileges to all qualified physicians in its community who meet the Hospital's credentialing standards. This open staff approach expands patient access to care, increases clinical capacity, and enhances the availability of services for all individuals in the area.Reinvestment of Surplus FundsAny surplus revenue is reinvested directly into improving patient care, clinical services, facility upgrades, medical equipment, community health programming, and staff recruitment and retention. Surplus funds also support expanded access to behavioral health, preventive care initiatives, and technology improvements such as virtual visits and telehealth.Community Health Needs Assessment and ImplementationCCMH conducts a Community Health Needs Assessment (CHNA) every three years to evaluate community health priorities and develop targeted strategies to address identified needs. Current priorities include improving lifestyle behaviors, increasing access to care, and strengthening mental health resources for both youth and adults. Action plans are developed in partnership with community stakeholders and implemented throughout the service area.Mental Health and Substance Use InitiativesThrough participation in the Carroll County Coalition on Mental Health and Substance Use Disorder, CCMH advances several community wide strategies, including:- Increasing awareness of resources through community resource guides- Collaborating with local schools to identify at risk youth and provide support- Offering education and assistance for substance use disorder prevention and treatment- Providing psychiatric services and outpatient therapyImproving Access to Preventive and Primary CareTo expand healthcare access, especially for uninsured and under insured residents, CCMH:- Provides community education on preventive care and available services- Operates satellite clinics in surrounding communities- Supports the Nurse Navigator program to guide patients through the care continuum- Offers expanded telehealth and virtual visit options to overcome geographic and mobility barriersWellness, Rehabilitation, and Preventive ServicesCCMH operates the CCMH Wellness Centeran integrated rehabilitation and fitness facility that includes an indoor pool, sauna, hot tub, walking track, and exercise equipment. Certified therapists and trainers provide evidence based physical therapy, occupational therapy, cardiac rehabilitation to support overall community well-being.Emergency and Essential ServicesThe Hospital operates an emergency department open to all individuals regardless of insurance status or ability to pay, ensuring critical access to lifesaving care in a rural community with limited alternative options.Community Benefit and Public Program ParticipationCCMH participates in Medicare, Medicaid, and other government sponsored programs, ensuring access to healthcare for low income and medically vulnerable individuals. The Hospital also provides financial assistance and charity care for eligible patients under its Financial Assistance Policy.Education, Workforce Development, and Community EngagementThe Hospital supports education and training of healthcare professionals through student clinical rotations, partnerships with regional schools and colleges, and continuing education opportunities for staff. CCMH also encourages community involvement through volunteer opportunities, public health events, and collaboration with advisory groups and local organizations.Commitment to Rural HealthcareAs one of the primary healthcare providers in a predominantly rural county, CCMH plays a critical role in maintaining local access to comprehensive medical care. The Hospital continually evaluates emerging needs and adapts services to ensure that all residentsregardless of age, income, or health statushave access to reliable, high quality healthcare close to home.
