Schedule H, Part V, Section B, Line 3E
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - SAINT FRANCIS HOSPITAL MUSKOGEE, INC.. Saint Francis Muskogee conducted its Fiscal Year 2025 Community Health Needs Assessment (CHNA) in collaboration with the Oklahoma State Department of Health District 7 and the University of Oklahoma Hudson College of Public Health (OU COPH). These partners provided public health expertise, secondary data analysis, community survey development, prioritization methodology, and validation of findings using evidence-based standards consistent with IRS requirements. The assessment period occurred between April 2024 and January 2025 and covered the three-year period through FY25. The CHNA was adopted by the Saint Francis Health System Board of Directors in April 2025. In conducting the CHNA, Saint Francis Muskogee solicited and considered input from persons representing the broad interests of the community served, including representatives of medically underserved, low-income, minority, rural, and uninsured populations. Input was obtained through community engagement meetings, advisory group discussions, collaboration with the Muskogee County Rural Health Network, and a county-wide community survey distributed throughout Muskogee County. Specific organizations consulted during the CHNA process included the Oklahoma State Department of Health District 7; the University of Oklahoma Hudson College of Public Health; the Muskogee County Rural Health Network; local healthcare providers; community-based social service organizations; public school representatives; and nonprofit agencies serving low-income and minority populations. Representatives from organizations serving uninsured individuals, individuals experiencing food insecurity, residents facing transportation barriers, and households experiencing housing instability participated in engagement meetings and advisory discussions. Where possible, members of medically underserved and vulnerable populations were directly engaged through survey participation and community forums. The community survey was developed using a Social Determinants of Health framework and was made available in English, Spanish, and Burmese to promote accessibility. A total of 354 surveys were completed through online distribution and mailed responses. Survey topics included access to healthcare, nutrition and food security, transportation, housing stability, mental and behavioral health, preventive health services, employment, and other social and economic factors affecting health outcomes in Muskogee County. Community engagement meetings included approximately 50 participants representing public health agencies, community-based organizations, healthcare providers, and other local stakeholders. Advisory and engagement groups met throughout the assessment process to review quantitative and qualitative data, identify emerging community concerns, and apply a structured prioritization methodology. Significant health needs were prioritized using criteria including size and severity of the need, impact on vulnerable populations, feasibility of intervention, sustainability, and availability of evidence-based strategies. Special consideration was given to medically underserved, low-income, minority, rural, and other vulnerable populations in Muskogee County, including uninsured adults (18% of adults under age 65), households experiencing food insecurity, residents facing transportation barriers (with 83% of workers driving alone), households experiencing severe housing problems (15%), and individuals reporting poor mental health days above state and national averages (6.1 days). Demographic and health indicator data from County Health Rankings & Roadmaps, U.S. Census Bureau sources, Oklahoma State Department of Health data, and community survey results were analyzed to identify health disparities and inequities.
Schedule H, Part V, Section B, Line 6a Facility , 1Facility , 1 - SAINT FRANCIS HOSPITAL MUSKOGEE, INC.. The hospital facility included in this CHNA was Saint Francis Hospital Muskogee, Inc.
Schedule H, Part V, Section B, Line 6b Facility , 1Facility , 1 - SAINT FRANCIS HOSPITAL MUSKOGEE, INC.. THE CHNA WAS CONDUCTED IN CONJUNCTION WITH THE FOLLOWING NON-HOSPITAL FACILITIES: OKLAHOMA UNIVERSITY HUDSON COLLEGE OF PUBLIC HEALTH AND OKLAHOMA STATE DEPARTMENT OF HEALTH DISTRICT 7 TEAM.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - SAINT FRANCIS HOSPITAL MUSKOGEE, INC.. Based on insights from community engagement meetings, surveys, and resident feedback, Muskogee County's most significant health-related needs were identified. These include: * Housing - Homelessness, housing instability, and lack of affordable options, strongly tied to mortality, mental health, and safety. * Mental and Behavioral Health - High suicide rates, firearm fatalities, poor mental health days, and barriers to care access. * Access to Healthcare - Challenges with insurance coverage, medical bills, chronic disease management, and emergency preparedness, with disparities worsened during COVID-19. * Transportation - Limited options restricting access to food, healthcare, jobs, and social connections. * Food Security - Barriers to affordable, nutritious food, particularly for low-income households, often linked to transportation and poverty. * Preventive Health Services - Low rates of preventive care contribute to avoidable diseases. Interventions could include community-based health programs, on-site vaccination clinics, and outreach for free or low-cost preventive services. Following the identification of significant community health needs, Leadership of Saint Francis Health System (SFHS) undertook a multi-step prioritization process involving review of survey and data findings, input from approximately 30 leaders, and discussions with Muskogee County leadership. While the Leadership of SFHS acknowledges the importance of all identified needs, four were selected as priorities for the formal CHNA implementation strategy: (1) Access to Healthcare, due to cost, insurance, and appointment barriers identified as the top concern; (2) Food Security, based on both internal screening data and survey findings highlighting affordability and preparation challenges; (3) Transportation, given high cost, unreliability and safety issues. These prioritized needs reflect areas where SFHS is well positioned to lead or collaborate. The remaining needs did not reach the same level of priority as the four needs highlighted, and SFHS is not best positioned to address these needs, as other community stakeholders are already working on solutions for them. The Leadership of Saint Francis Health System on behalf of Saint Francis Muskogee will address the prioritized community health needs through several implementation strategies. To improve access to healthcare services, the hospital will strengthen community partnerships, expand telemedicine outreach, and support workforce development initiatives to increase provider availability and improve access to care. To address food insecurity, the hospital will collaborate with community organizations to improve access to nutritious foods and provide nutrition education and support programs aimed at vulnerable populations. To improve transportation access, the hospital will work with community partners to expand transportation options and coordinate resources that help patients access healthcare services and other essential community supports. These efforts will be implemented through partnerships with public health agencies, community organizations, and local service providers to improve health outcomes for residents of Muskogee County.

Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceCOSTING METHODOLOGY: A RATIO OF PATIENT CARE COST TO CHARGES, AS DETERMINED IN WORKSHEET 2, WAS USED TO REPORT THE AMOUNTS IN PART I, LINES 7A - 7C. FOR AMOUNTS REPORTED ON LINES 7E - 7I, ACTUAL EXPENSES FOR EACH COMMUNITY BENEFIT ACTIVITY ARE TRACKED AND REPORTED USING THE ORGANIZATION'S ACCOUNTING GENERAL LEDGER AND ARE NOT BASED ON A COST TO CHARGE RATIO. THE SUPPLEMENTAL HOSPITAL OFFSET PAYMENT PROGRAM (SHOPP) WAS CREATED AND IMPLEMENTED IN CALENDAR YEAR 2011 FOR THE PURPOSE OF ASSURING ACCESS TO QUALITY CARE FOR OKLAHOMA MEDICAID MEMBERS. THE PROGRAM IS DESIGNED TO ASSESS OKLAHOMA HOSPITALS, UNLESS EXEMPT, A SUPPLEMENTAL HOSPITAL OFFSET PAYMENT PROGRAM FEE. THE OKLAHOMA HEALTH CARE AUTHORITY (OHCA) DOES NOT GUARANTEE THAT ALLOCATION WILL EQUAL OR EXCEED THE AMOUNT OF THE SUPPLEMENTAL HOSPITAL OFFSET PAYMENT PROGRAM FEES THAT WERE PAID BY SAINT FRANCIS HOSPITAL MUSKOGEE, INC.
Schedule H, Part II Community Building ActivitiesCommunity-building activities are undertaken to improve community health and safety by addressing the root causes of health problems. These activities enhance the community's capacity to promote the health and well-being of residents through the expertise and resources of the healthcare organization. Related costs include cash contributions and expenses associated with the development of community-building programs and partnerships. Additional information regarding the community-building activities of Saint Francis Health System, Inc. and Saint Francis Hospital Muskogee, Inc. is reported this Schedule H Part V, Section B, Line 5.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountSAINT FRANCIS HOSPITAL MUSKOGEE, INC. HAS AN ESTABLISHED PROCESS TO DETERMINE THE ADEQUACY OF THE ALLOWANCE FOR UNCOLLECTABLE RECEIVABLES THAT RELIES ON A NUMBER OF ANALYTICAL TOOLS AND BENCHMARKS TO ARRIVE AT A REASONABLE ALLOWANCE. NO SINGLE STATISTIC OR MEASUREMENT DETERMINES THE ADEQUACY OF THE ALLOWANCE FOR UNCOLLECTABLE RECEIVABLES. SOME OF THE ANALYTICAL TOOLS THAT SAINT FRANCIS HOSPITAL MUSKOGEE, INC. UTILIZES INCLUDE, BUT ARE NOT LIMITED TO, HISTORICAL CASH COLLECTION EXPERIENCE, REVENUE TRENDS BY PAYER CLASSIFICATION, AND REVENUE DAYS IN ACCOUNTS RECEIVABLE. ACCOUNTS RECEIVABLES ARE WRITTEN OFF AFTER COLLECTION EFFORTS HAVE BEEN FOLLOWED IN ACCORDANCE WITH SAINT FRANCIS HOSPITAL MUSKOGEE, INC.'S POLICIES.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyThe bad debt expense attributable to patients eligible under the organization's financial assistance policy is calculated from a sample review of all bad debt accounts and subsequent information. Many patients would qualify for financial assistance under the Saint Francis Financial Assistance Policy if the application process was completed, but for a number of reasons, patients are sometimes unable to complete the process. As result, their accounts end up in bad debt. For this reason, the cost associated with these accounts should be considered community benefit. Saint Francis exhausts all efforts to either collect from the patient or help them to complete the financial assistance process. After all efforts have been exhausted, Saint Francis Hospital uses data sources and quantitative analysis to identify the population of patients that, more likely than not, would have qualified for charity care had they been able to complete the financial assistance application. A cost to charge ratio is applied to the gross charges of the identified patients.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteSaint Francis Health System, Inc. 73-1501972, the parent organization of Saint Francis Hospital Muskogee, Inc., produces consolidated financials which include the financial position and results of financial operations of Saint Francis Hospital Muskogee, Inc. Saint Francis Health System, Inc's audited consolidated financial statements provide a separate footnote addressing the organization's net patient accounts receivables on page 9. Saint Francis Health System, Inc. reports bad debt in accordance with Generally Accepted Accounting Principles (GAAP).
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsCOSTING METHODOLOGY: MEDICARE ALLOWABLE COSTS WERE CALCULATED USING A COST-TO-CHARGE RATIO AND THE MEDICARE FILED COST REPORT.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceSaint Francis Hospital Muskogee, Inc.'s debt collection policy is to pursue collections of patient balances from patients who can pay for the services. Saint Francis Hospital Muskogee, Inc. applies its collections efforts consistently and fairly to all patients regardless of insurance. Saint Francis Hospital Muskogee, Inc. works with those individuals who do not have the financial resources to pay outstanding balances to qualify for Saint Francis Hospital Muskogee, Inc.'s financial assistance policy. Schedule H, Part VI, Line 2 Needs assessmentUsing a mixed-methods approach grounded in a Social Determinants of Health framework, the CHNA incorporated a community survey, direction from a community advisory board, stakeholder engagement meetings with local leaders and organizations, and secondary data analysis using County Health Rankings and Roadmaps (CHRR). A structured governance model was established to ensure broad input and accountability. The core group oversaw survey development, data analysis, and prioritization of health needs. Advisory groups, including the local county health departments, provided additional guidance and alignment. Engagement groups, consisting of community subject matter experts and organizational stakeholders, were convened at key stages to provide context, interpret findings, and validate emerging themes. This structure ensured that community voices were central to the process and that the final CHNA reflected both quantitative data and the lived experiences of the communities served. The organization's CHNA process is comprehensive; therefore, no additional assessments are undertaken.
