Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - CLARITY CHILD GUIDANCE CENTER. CLARITY CHILD GUIDANCE CENTER (CCGC) CONDUCTED MULTIPLE MEETINGS AND INTERVIEWS WITH KEY STAKEHOLDERS IN THE COMMUNITY. THE PROCESS INVOLVED COLLECTION AND ANALYSIS OF DATA SPECIFICALLY RELATED TO CHILDREN'S MENTAL HEALTH, AS CCGC IS A UNIQUE, SPECIALTY CARE HOSPITAL SERVING THE NEEDS OF CHILDREN WITH SEVERE MENTAL ILLNESSES, REPRESENTING AGES 3 TO 17. ADDITIONALLY THE PROCESS ALSO INVOLVED REVIEW OF REPORTS INCLUDING EXTENSIVE INTERVIEWS WITH STAKEHOLDERS THAT HAVE SPECIFIC KNOWLEDGE AND/OR RESPONSIBILITY TO THE WELL-BEING OF OUR COMMUNITY IN RELATION TO MENTAL HEALTH CARE. COMMUNITY INPUT WAS PROVIDED THROUGH THE REVIEW OF RECENT STAKEHOLDERS AND COMMUNITY STUDIES SHOWING THE NEEDS, Community input was provided through the review of recent stakeholders and community studies showing the needs, challenges and opportunities in our region. It includes reports from the City of San Antonio, Bexar County, Texas Lyceum, Annie E. Casey Foundation, Child Stats, Kids County Data Center, American Association of Child and Adolescent Psychiatry, Texas Democratic Center and American Psychological Association, among others. Community Input was also taken from an April 2024 Survey by the City of San Antonio that was made up of 22 high school students appointed by the mayor and city council members. Additionally, from a poll of parent of public-school student K-12 by the National PTA Association and the CDC Foundation.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - CLARITY CHILD GUIDANCE CENTER. Clarity Child Guidance Center's leadership evaluated the opportunities revealed in the Community Health Needs Assessment and with the guidance of the Board of Directors, developed a strategic plan to address gaps in the community. Clarity took action by launching the following initiatives since our last published CHNA in 2021: 1. HEROES: The Campaign for Clarity - a $16 million, multi-phase capital campaign - launched in 2022 that will allow Clarity to serve an additional 1,000 kids a year. The goals of the campaign are: -Renovate an existing building into an 8-bed unit dedicated to children and teens with very complex issues. The new unit will provide dedicated space for programming and a private, secure courtyard. It is estimated the eight additional beds will allow Clarity to serve an additional 700 children per year. - Expand the kitchen and renovate the dining room to serve our growing number of patients. - Build out the existing shell building on the third floor of the Outpatient Building to allow for 4,500 more outpatient appointments annually. - Expand the Crisis Services Department to provide critical assessments to an additional 60 kids per month. 2. In the past three years, Clarity created two programs to address the current national predicament: limited access to a mental health professional. First Step is a one-time consultation for families who are not sure what their child needs or where to get it. The child receives a thorough assessment, a psychologist conducts an interview and the results are then reviewed with the family in order to identify areas of concern. A care coordinator works with the family to connect patients to an appropriate level of care. The Next Step center connects families to services in order to provide immediate, transitional outpatient therapy, including brief therapy, bridge appointments, school referrals and care coordination services. 3. In 2024, Clarity and the Southside Independent School District Board of Trustees approved a first-of-its-kind partnership to have an on-site mental health center housed on their high school campus. Southside ISD is primarily rural; therefore, transportation and access to mental health is challenging as there are no psychiatrists and only two known therapists in the South San Antonio area zip codes. Clarity will provide onsite services regardless of families' ability to pay to Southside ISD students including partial hospitalization, crisis assessments, and individual and family therapy sessions. 4. Trauma-informed Care - Clarity estimates 90% of patients have experienced trauma-whether it's "Big T Trauma", like physical and/or sexual abuse, or "Little t trauma", such as witnessing domestic violence, lack of housing or food, neglect or verbal abuse, emotional detachment, and/or loss or separation from relative. Research has shown that child trauma survivors may experience higher incidences of learning problems, lower grades, more suspensions and expulsions; increased use of health and mental health services; increased involvement with the child welfare and juvenile justice systems; and are more likely to experience long-term health problems (e.g., diabetes and heart disease). Clarity began the transition to trauma-informed care treatment over 13 years ago. Over the last year, it has taken it a step further, and require all members of the Clarity staff to participate in training. Clarity has also applied to become a Trauma Informed Care campus. This extensive certification process, conducted by the Ecumenical Center, will "build resilience and empower individuals by providing physical, psychological, and emotional support through the principles of safety, trustworthiness and empowerment." 5. In response to the shooting tragedy at Robb Elementary School in Uvalde, Texas in 2022, Clarity professionals traveled to Uvalde to offer free counseling. It also coordinated and still provides a three-prong program-underwritten by a grant from H-E-B-offering: - Three telehealth assessment stations at Uvalde community partner locations: Children's Bereavement Center in Uvalde. This will allow Clarity staff to complete a level of care assessment and determine if hospitalization is required, avoiding unnecessary trips to San Antonio and back to Uvalde. - Trauma-informed transportation using Alternative Transportation Solutions to the hospital or treatment center of the parents' choice. ATS utilizes behavioral health-trained drivers and aides that transport children in unmarked vans designed specifically for behavioral health transports. - Offer gas cards to Uvalde families so that trips to San Antonio for in-person sessions and transportation back home after discharge are not a financial burden. 6. Clarity is an original partner of the San Antonio Mobile Mental Wellness Collaborative with Jewish Family Services of San Antonio, Family Service Association, Rise Recovery and the Children's Bereavement Center of South Texas. The first of its kind in the nation, this collaborative serves as an upstream collaboration of nonprofits focusing on behavioral health prevention so that our area public schools do not have to focus fully on intervention. Through each agency's expertise, professionals are able to work with all student age groups and meet their behavioral health needs in their own learning environment, incorporating parents and guardians, teachers, and administrative staff into the mental wellness program. Services are offered on school district property or virtually to help eliminate the transportation barrier that often limits access to mental health services. Partners now include Clarity, Rise Recovery, Family Service Association, Joven and Endeavors. 7. Clarity continues to host Claritycon, an annual children's mental health conference that provides professional development (including CEUs and CNEs) by 40+ renowned experts to 500+ educators, therapists, physicians, nurses, social workers, mental health professionals and community members from the region. Claritycon 2024 was the 11th anniversary of the conference, reaching a record number of attendees. 8. In May 2024, Texas Public Radio and Clarity Child Guidance Center collaborated to host a free film screening of Screenagers-an award-winning documentary about raising teens in the scary world of social media - to the community. A panel discussion was held to address attendees questions related to mental health and social media. The film's creator Delaney Ruston, MD, provided the keynote presentation for Claritycon 2024. 10 community health needs assessment December 2024 9. In 2023 and 2024, Clarity participated in the Texas Cavaliers River Parade, making Clarity visible to the 250,000+ people who attend the parade and the additional 300,000 people watching the parade on TV. Clarity was invited to participate again in 2025. 10. Clarity prioritizes volunteer and donation initiatives. Food for the Soul invites volunteers to serve a meal to the kids. Bridge the Gap and an onsite food pantry provide food and clothing for patients. Our Amazon wish list includes clothing ideas and educational supplies that individuals can purchase throughout the year. Giving Tree is our holiday campaign where churches, businesses or organizations host a Giving Tree during November and December to gather presents for the children receiving inpatient treatment at Clarity's hospital over the holidays. 11. Clarity executed a customized ad campaign in 2024 to reach target markets and community-at-large in San Antonio and central/south Texas to educate the public about Clarity's services, new programs, Claritycon and other events. Publications included main-stream media as well as communication vehicles for nonprofit agencies. Clarity's services, new programs, Claritycon and other events.

Schedule H, Part I, Line 3cFACTORS OTHER THAN FPG DETERMINING FREE OR DISCOUNTED CARE: THE HOSPITAL USES THE FOLLOWING CRITERIA TO DETERMINE ELIGIBILITY FOR FREE OR DISCOUNTED CARE -MEDICAL INDIGENCY -INSURANCE STATUS -UNDERINSURANCE STATUS
Schedule H, Part V, Section B, Line 8The hospital facility is reporting a non-willful failure to meet the timing requirement under section 501(r)(3) and Treasury Regulation ยง1.501(r)-3 relating to formal adoption of its implementation strategy following completion of its community health needs assessment (CHNA). The CHNA for the hospital facility was completed in June 2025 for the taxable year ended June 30, 2025. Under the applicable rule, the implementation strategy was required to be adopted by the governing body on or before November 15, 2025. Although the implementation strategy was fully developed and in use by June 2025, the governing body did not formally adopt the implementation strategy until May 2026. This failure occurred at a single hospital facility and involved a single occurrence relating to the implementation strategy associated with the CHNA for the taxable year ended June 30, 2025. The cause of the failure was an inadvertent administrative oversight in routing and tracking the item for timely governing body approval. The hospital facility otherwise maintained section 501(r) compliance policies and procedures, and the failure did not involve any deficiency in the substantive content of the implementation strategy or any delay in making the strategy publicly available. The hospital reported that the CHNA was conducted and described the implementation strategy on Form 990, Schedule H for the taxable year ended June 30, 2025. No patients, community members, or other individuals were financially affected by this failure. The failure had no financial assistance policy, limitation-on-charges, or billing and collection implications. The failure was corrected in May 2026 when the governing body formally adopted the implementation strategy and publicly posted it on the website. In connection with that correction, the hospital confirmed that the implementation strategy used since June 2025 was consistent with the version formally adopted by the governing body. To prevent recurrence, the hospital revised its section 501(r) compliance procedures to require a documented compliance calendar and approval checklist for CHNAs and related implementation strategies, including specific routing, deadline tracking, and confirmation of governing body action. Oversight of these procedures has been assigned to the compliance officer, who is responsible for monitoring completion of required approvals within the applicable deadlines.
