Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - THE CHRIST HOSPITAL. The Christ Hospital Health Network (TCHHN) conducted a Community Health Needs Assessment (CHNA) in Fiscal Year 2022 and again in Fiscal Year 2025 (July 1, 2024 - June 30, 2025). Input from a variety of community partners provided valuable insight into the overall health and well-being of the community we serve. One of our primary partners was The Health Collaborative, a nonprofit organization that works with its member hospitals on healthcare improvement projects and shares best practices. For the fourth time, The Health Collaborative convened local hospitals and led efforts to better understand community health needs. Their regional report shares data for Southwest Ohio, including the Greater Cincinnati area, Northern Kentucky, and Southeastern Indiana. Because hospital service areas vary, this approach provides the most thorough picture of local and regional health needs. The Fiscal year 2025 CHNA explored the social determinants of health (SDOH) contributing to health behaviors and outcomes, along with the systemic barriers that drive disparities and health inequities within the community. Recognizing that physical, environmental, and behavioral factors play a critical role in health, the CHNA placed a strong emphasis on understanding the SDOH and the structural issues that shape them - factors that must be addressed to improve health outcomes for all community members. The priorities were determined by the number of votes in community meetings, the number of mentions in surveys, and data trends that were worse than state or national averages and impacted at least 16 counties (secondary data). The identified priorities ranked in the top eight across all primary data sources. Prevalent health concerns identified in Hamilton County include: * Heart Disease and Stroke Prevention * Mental Health * Homelessness Through a collaborative and meticulous process, TCHHN and its community partners have reviewed these concerns and established a course of action. This plan focuses on programs, research, and education geared to the community's top identified health concerns. Our goal is to make a measurable impact on community health. We recognize that one entity's expertise cannot be broad enough to address all needs; therefore, collaborations and partnerships are key to meeting them. The Implementation Plan was approved by TCHHN Board of Directors on 10/22/2025. Heart Disease and Stroke Prevention Chronic high blood pressure, or hypertension, can lead to heart disease and stroke - serious conditions that rank among the leading causes of death and the most frequent emergency department diagnoses in our region. These conditions are linked to inadequate housing and mental health challenges. We developed the following goals so that we may continue to make an impact on the community: 1. Increase access to PREVENTATIVE screening services. To achieve this goal, we will deploy a mobile heart unit to provide vascular screenings directly within the community, removing barriers to care for underserved populations, partnering with community organizations to engage at-risk individuals for testing. 2. Increase mental health support in the heart disease population. To achieve this goal, we will expand behavioral health consultant support within TCHHN's cardiology service line to integrate mental health care into cardiovascular disease management. We will coordinate with community-based mental health services and payor resources to ensure comprehensive, integrated care. Mental Health Mental health treatment and crisis prevention are crucial for individuals and communities. Mental health is closely linked to physical health and influenced by relationships, employment, and living environments. Lack of timely, affordable mental health services can contribute to poor outcomes, while supportive communities with access to quality housing, education, and jobs promote positive mental health. We aim to continue making an impact through the following goals: 1. Expand access to behavioral health consultants in ambulatory specialty services. To achieve this goal, we will recruit additional behavioral health consultants within ambulatory specialty services, focusing on high-risk populations to ensure timely and equitable access. We will strengthen our work with community-based mental health services and payor partners to align resources and extend program impact. 2. Reduce readmissions related to mental health diagnoses. To achieve this goal, we will ensure patients are seen by a provider within seven days of discharge to reduce relapse or readmission and improve continuity of care. We will partner with community organizations and payors to ensure seamless support during transition from hospital to community care. Homelessness Safe, stable housing is essential for health and well-being. Affordable rent, adequate space, and housing stability prevent stressors that can lead to chronic health conditions. High housing costs may limit resources for other necessities such as childcare, nutritious food, and healthcare. Poor quality housing can increase chronic stress, blood pressure, and mental health risks. We will continue to address these needs through the following goals: 1. Expand access to resources for patients facing housing crises. To achieve this goal, we will increase screening for SDOH to better identify needs and connect patients to appropriate resources. We will leverage community and payor partnerships to fill gaps in support as needed. 2. Provide quality, holistic medical care to people experiencing homelessness. To achieve this goal, we will offer financial support to the Center for Respite Care to allow patients without stable housing to receive needed care in a safe environment, while breaking the cycle of homelessness. The ongoing collaboration with the Center for Respite Care will ensure coordinated, comprehensive, and PATIENT-CENTERED services.

Schedule H, Part V, Section B, Line 22 PART V, SECTION B, LINE 22DINDIVIDUALS ELIGIBLE FOR FINANCIAL ASSISTANCE - HOSPITAL DETERMINATION OF THE MAXIMUM AMOUNTS THAT CAN BE CHARGED TO FAP-ELIGIBLE INDIVIDUALS FOR EMERGENCY OR MEDICALLY NECESSARY CARE. TCH MAINTAINS A FINANCIAL ASSISTANCE PROGRAM POLICY THAT OUTLINES THE MAXIMUM AMOUNT THAT CAN BE CHARGED TO FINANCIAL ASSISTANCE PROGRAM ELIGIBLE INDIVIDUALS FOR EMERGENCY OR OTHER MEDICALLY NECESSARY CARE. THE AMOUNT CHARGED TO FINANCIAL ASSISTANCE PROGRAM ELIGIBLE INDIVIDUALS FOR EMERGENCY OR MEDICALLY NECESSARY CARE IS REPRESENTATIVE OF AN ANNUAL COMPUTATION OF AMOUNTS GENERALLY BILLED (AGB) TO INDIVIDUALS WHO HAVE INSURANCE COVERING SUCH CARE. TCH PROVIDES FURTHER ADJUSTMENTS TO AMOUNTS CHARGED BASED ON A VARIETY OF FACTORS, INCLUDING PATIENT INCOME INFORMATION AND OTHER SOURCES WHEN INADEQUATE INFORMATION IS PROVIDED BY THE PATIENT TO QUALIFY UNDER TCH'S CHARITY PROGRAMS. TCH WORKS WITH ALL PATIENTS BASED ON THEIR INDIVIDUAL SITUATION TO ENSURE OUR MISSION IS BEING CARRIED OUT WITHIN THE COMMUNITY IN WHICH WE SERVE.
Schedule H, Part I, Line 7 COLUMN FBAD DEBTS ARE NETTED AGAINST REVENUES IN PART VIII, AND AS SUCH ARE NOT INCLUDED IN THE TOTAL EXPENSES IN ORDER TO CALCULATE THE PERCENTAGE REPORTED IN COLUMN F ON SCHEDULE H, PART I AND PART II.
Schedule H, Part III SECTION BIN ADDITION TO HOSPITAL-BASED REVENUE AND ALLOWABLE COST GENERATED FROM MEDICARE (INCLUDING DSH AND IME) ELIGIBLE PARTICIPANTS REPORTED IN PART III SECTION B, THE CHRIST HOSPITAL PROVIDES HOSPITAL-BASED SERVICES TO OTHER MEDICARE ELIGIBLE PARTICIPANTS, THROUGH MEDICARE PLANS SPONSORED BY MANAGED CARE PROVIDERS (MEDICARE MANAGED CARE PLANS). MEDICARE MANAGED CARE PLANS ARE HEALTH MAINTENANCE ORGANIZATIONS (HMOS) OR PREFERRED PROVIDER ORGANIZATIONS (PPOS) THAT PROVIDE BASIC MEDICARE COVERAGE PLUS OTHER COVERAGES TO FILL GAPS IN MEDICARE COVERAGE. THESE PLANS HAVE ARRANGEMENTS WITH CERTAIN PHYSICIANS, HOSPITALS AND HEALTH CARE PROVIDERS TO SERVE PATIENTS WHO ARE PLAN MEMBERS AT A CONTRACTED RATE. BELOW IS THE SHORTFALL RELATED TO HOSPITAL-BASED REVENUE AND ALLOWABLE COSTS UNDER MEDICARE MANAGED CARE PLANS: REVENUE RECEIVED FROM MEDICARE MANAGED CARE PLANS $214,663,747 ALLOWABLE COST OF CARE RELATED TO PAYMENTS FROM ABOVE $(266,579,752) SHORTFALL FROM MEDICARE MANAGED CARE PLANS $(51,916,005) ALSO, THE CHRIST HOSPITAL HAS CERTAIN MEDICARE PART B (PHYSICIAN-BASED) REVENUE AND ALLOWABLE COST RELATED TO SERVICES RENDERED ON BEHALF OF MEDICARE ELIGIBLE PARTICIPANTS BY EMPLOYED PHYSICIANS OF THE CHRIST HOSPITAL. THIS WOULD INCLUDE SERVICES PROVIDED UNDER TRADITIONAL MEDICARE, AS WELL AS MEDICARE MANAGED CARE PLANS. BELOW IS THE SHORTFALL RELATED TO THE DISREGARDED ENTITIES OF THE HOSPITAL: REVENUE RECEIVED FROM MEDICARE PART B PLANS (PHYSICIAN-BASED) $127,341,188 ALLOWABLE COST OF CARE RELATED TO PAYMENTS FROM ABOVE $(203,498,313) SHORTFALL RELATED TO THE MEDICARE PART B PLANS (PHYSICIAN-BASED) $(76,157,125)
Schedule H, Part VI, Line 6N/A
Schedule H, Part VI, Line 7N/A
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceTHE CHRIST HOSPITAL (TCH) APPLIES A COST-TO-CHARGE RATIO FROM WORKSHEET 2 OF THE FORM 990, SCHEDULE H INSTRUCTIONS TO DETERMINE FINANCIAL ASSISTANCE PROVIDED AT COST.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountTHE PROVISION FOR BAD DEBTS IS BASED UPON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS CONSIDERING HISTORICAL BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN HEALTH CARE COVERAGE, AND OTHER COLLECTION INDICATORS. PERIODICALLY THROUGHOUT THE YEAR, MANAGEMENT ASSESSES THE ADEQUACY OF THE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS BASED UPON HISTORICAL WRITE-OFF EXPERIENCE BY PAYOR CATEGORY. THE RESULTS OF THIS REVIEW ARE THEN USED TO MAKE MODIFICATIONS TO THE PROVISION FOR BAD DEBTS TO ESTABLISH AN APPROPRIATE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyTHE ESTIMATED AMOUNT OF BAD DEBT EXPENSE ATTRIBUTABLE TO PATIENTS ELIGIBLE UNDER THE CHRIST HOSPITAL'S FINANCIAL ASSISTANCE POLICY IS BASED UPON MANAGEMENT'S REVIEW OF PATIENT ACCOUNT RECORDS FOR WHICH PATIENT APPLIED FOR FINANCIAL ASSISTANCE AND WAS SUBSEQUENTLY DENIED DUE TO MISSING OR INCOMPLETE DOCUMENTATION. THE METHODOLOGY USED BY THE CHRIST HOSPITAL TO DETERMINE THIS AMOUNT REASONABLY ESTIMATES BAD DEBT ADJUSTMENTS ATTRIBUTABLE TO PATIENTS WHO COULD HAVE QUALIFY FOR FINANCIAL ASSISTANCE, BUT FOR WHOM INSUFFICIENT INFORMATION WAS OBTAINED TO DETERMINE THEIR ELIGIBILITY.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteIN MAY 2014, THE FINANCIAL ACCOUNTING STANDARDS BOARD (FASB) AND INTERNATIONAL ACCOUNTING STANDARDS BOARD ISSUED ACCOUNTING STANDARDS UPDATE (ASU) 2014-09, REVENUE FROM CONTRACTS WITH CUSTOMERS (TOPIC 606). TCH ADOPTED ASU 2014-09 ON JULY 1, 2018 USING A FULL RETROSPECTIVE BASIS. UPON ADOPTION, THE MAJORITY OF WHAT WAS PREVIOUSLY CLASSIFIED AS PROVISION FOR UNCOLLECTIBLE ACCOUNTS AND PRESENTED AS A REDUCTION TO PATIENT SERVICE REVENUE ON THE CONSOLIDATED STATEMENT OF OPERATIONS AND CHANGES IN NET ASSETS IS TREATED AS AN IMPLICIT PRICE CONCESSION THAT REDUCES THE TRANSACTION PRICE, WHICH IS REPORTED AS PATIENT SERVICE REVENUE. AS SUCH, BAD DEBT EXPENSE IS NOT REFERENCED IN TCH'S AUDITED FINANCIAL STATEMENTS. THE PROVISION FOR BAD DEBTS IS BASED UPON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS CONSIDERING HISTORICAL BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN HEALTH CARE COVERAGE, AND OTHER COLLECTION INDICATORS.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsMEDICARE SURPLUS (OR SHORTFALL) OCCURS WHEN THE FEDERAL GOVERNMENT REIMBURSES HOSPITALS IN EXCESS, OR BELOW, THEIR COSTS FOR TREATING MEDICARE PATIENTS. THE CHRIST HOSPITAL, LIKE MOST HOSPITALS AND HEALTH SYSTEMS, INCURRED A MEDICARE SHORTFALL FOR THE YEAR ENDED JUNE 30, 2025. THE SHORTFALL AMOUNT DISCLOSED IN PART III, SECTION B IS BASED ON HOSPITAL-BASED REVENUE AND ALLOWABLE COST (INCLUDING DSH AND IME) FROM TRADITIONAL MEDICARE PROGRAMS (I.E., THOSE NOT SPONSORED BY A MANAGED CARE PLAN). NON-NEGOTIABLE MEDICARE RATES FOR THE YEAR ENDED JUNE 30, 2025 WERE OUT-OF-LINE WITH THE TRUE COSTS OF CARING FOR THESE PATIENTS, DRIVING THIS SHORTFALL. BY CONTINUING TO TREAT PATIENTS ELIGIBLE FOR MEDICARE, THE CHRIST HOSPITAL IS ALLEVIATING THE FEDERAL GOVERNMENT'S BURDEN FOR DIRECTLY PROVIDING MEDICAL SERVICES. BY CONTINUING TO SERVE PATIENTS WITH GOVERNMENT HEALTH BENEFITS, INCLUDING MEDICARE, THE CHRIST HOSPITAL IS PROMOTING THE HEALTH OF THE COMMUNITY, IN LINE WITH ITS CHARITABLE MISSION.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistancePATIENTS KNOWN TO QUALIFY FOR CHARITY CARE AND FINANCIAL ASSISTANCE ARE NOT SENT TO COLLECTIONS UNLESS A DETERMINATION IN ACCORDANCE WITH THE FINANCIAL ASSISTANCE POLICY HAS BEEN MADE THAT THE PATIENT HAS THE FINANCIAL RESOURCES TO PAY SOME PORTION OF THEIR BILL BUT REFUSES TO DO SO.

Schedule H, Part VI, Line 2 Needs assessmentThe first step in conducting the 2025 Community Health Needs Assessment (CHNA) was to clearly define the community we serve. This process involved evaluating several key factors, including the geographic areas in which we operate, the populations we serve, and the Hospital's primary service lines and clinical functions. In identifying our community, we intentionally included medically underserved, low-income, and minority populations residing within our service area. We also considered all patients, regardless of their ability to pay, insurance status, or eligibility for financial assistance under our Financial Assistance Policy. This inclusive approach reflects our mission to provide equitable care and improve health outcomes across all populations. We partnered with The Health Collaborative (THC) to conduct a comprehensive and collaborative CHNA. This regional effort was facilitated by THC and included the participation of 21 hospitals and 10 health departments, spanning 18 counties across Greater Cincinnati, southeastern Indiana, and northern Kentucky. The success of this CHNA was made possible through the active engagement of local community leaders, strategic partners, and champions across the region who supported outreach, community engagement, and data collection efforts. In addition to the data collected THROUGH OUR partnership with THC, we analyzed multiple sources, including public health data, health risk factor surveys, socioeconomic needs assessments, environmental standards, and existing programs for residents in the greater Cincinnati region. We also reviewed written comments received from the previous assessment. The full Collaborative CHNA report offers a detailed account of the following: 1. The process of gathering community input, including contributions from individuals and organizations with special knowledge of, or expertise in, public health. 2. The timeline and methods used to collect community input. 3. The names and roles of participating organizations, as well as the nature and extent of their contributions. 4. Descriptions of medically underserved, low-income, and minority populations represented by participants throughout the process. Oversight of this process was provided by a diverse Advisory Committee composed of representatives from hospitals, public health departments, federally qualified health centers, community-based organizations, professional associations, funding bodies, and hospital associations. Their leadership ensured that the CHNA process remained inclusive, equitable, and responsive to the needs of the broader community. Throughout the process, we held 12 advisory committee meetings and 6 task force meetings, which included 45 total partner organizations.

In addition to providing emergency services, TCHHN uses a systematic approach to determine community health needs with input from organizations such as the Center for Closing the Health Gap, the TCHHN Family Advisory Council, and through the gathering of community health statistics by service line leaders and the marketing department. Information gathered through these sources has led to the deployment of the Palliative Care Program and the development of a Center for Health and Aging to address the special needs of geriatric patients. TCHHN also supports the community through numerous fund drives such as United Way, the American Heart Association, diabetes and cancer walks, and the Fine Arts Fund. At the beginning of each school year and the Winter Holiday Season, TCHHN team members donate school supplies and gifts to students at Taft Elementary School located in Mt. Auburn. In addition, TCHHN staff provide mentoring opportunities to undergraduate and graduate student interns/residents interested in healthcare careers, as well as to Explorer Scouts. TCHHN Team Member Resource Groups also play a large role in strengthening community engagement by supporting outreach efforts, volunteer activities, and partnerships that advance health equity and belonging across the region.
