Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - Hillsdale Community Health Center. THE PRIMARY DATA FOR THE ASSESSMENT CONSISTED OF A HEALTH SURVEY OF THE GENERAL COUNTY POPULATION AND AN OPEN FORUM OF HUMAN SERVICES NETWORK MEMBERS. THE HEALTH SURVEY QUESTIONNAIRE WAS DESIGNED TO CREATE A PROFILE OF THE RESPONDENTS, THEIR HEALTH NEEDS, AND VIEWS ON COMMUNITY HEALTH-RELATED ISSUES. ALL RESIDENTS OF THE COUNTY WERE INVITED TO PARTICIPATE IN THE SURVEY THROUGH A WIDE-REACHING INFORMATION CAMPAIGN THAT CONSISTED OF INFORMATIONAL ADS IN THE LOCAL NEWSPAPER, THE LOCAL RADIO STATION, PAPER MAILINGS, THE HOSPITAL WEBSITE, AND PERSONAL REQUESTS BY HOSPITAL VOLUNTEERS. AS A RESULT OF THE CAMPAIGN, 1,079 INDIVIDUALS COMPLETED THE HEALTH SURVEY QUESTIONNAIRE (723 IN 2022). SECONDARY DATA WAS COLLECTED FROM A VARIETY OF COUNTY, STATE, AND FEDERAL SOURCES SUCH AS THE US CENSUS BUREAU, MICHIGAN LABOR AND EDUCATION DEPARTMENTS, MICHIGAN DEPARTMENT OF VITAL STATISTICS, MICHIGAN BEHAVIOR RISK FACTOR SURVEY, AND KIDS COUNT IN MICHIGAN.
Schedule H, Part V, Section B, Line 6b Facility , 1Facility , 1 - Hillsdale Community Health Center. HILLSDALE HOSPITAL PARTNERED WITH THE HILLSDALE COUNTY HUMAN SERVICES NETWORK TO CREATE A COMPREHENSIVE COMMUNITY BASED HEALTH NEEDS ASSESSMENT. HILLSDALE HOSPITAL HIRED AN INDEPENDENT CONTRACTOR TO HELP DESIGN THE SURVEY, ANALYZE THE DATA, FACILITATE DISCUSSIONS, AND CREATE THE FINAL REPORT.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - HILLSDALE COMMUNITY HEALTH CENTER. 1. Recommendation from Human Services Network (HSN): Decrease the number of women who receive inadequate prenatal care from 11.1 to 8.0 for the next 3-year period (2025-2028) Background: Among the 1,482 births to county residents between 2020-2022: 61.5% of mothers received adequate prenatal care compared to 68.9% in Michigan; 27.2% of mothers received intermediate care compared to 23.0% in Michigan; and 11.4% had inadequate care compared to 8.1% in Michigan. An analysis of prenatal care indicates that about 60% of Hillsdale mothers receive adequate pre-natal care, while 10% did not. Lack of adequate pre-natal care contributes to several poor outcomes for infants, including low birthweight. The percent of low and very low birthweights was higher in Hillsdale than in Michigan compared to previous years. Action Plan - 1. Strengthen Outreach & Education a. Launch a community prenatal care awareness campaign b. Provide educational materials in clinics, schools, libraries, and online c. Develop targeted messaging for young and low-education-level mothers 2. Collaborate with Great Start Collaborative a. Integrate hospital efforts with existing community programs b. Refer patients between systems for wraparound support c. Share data and best practices to identify service gaps 3. Provide Support Services for At-Risk Moms a. Offer GED program referrals and childcare resources b. Create prenatal care "starter kits" (nutrition guides, appointment calendars, transportation info) c. Assign case managers to help schedule and follow-up 4. Reduce Transportation and Scheduling Barriers a. Provide information about transportation assistance for appointments b. Offer flexible scheduling, including evenings/weekends where possible
Schedule H, Part V, Section B, Line 11 Facility , 2Facility , 2 - HILLSDALE COMMUNITY HEALTH CENTER. 2. Recommendation from HSN: Develop a Vaping Educational Campaign to provide information on the effects and dangers associated with Vaping. Background: A slightly higher percentage of Hillsdale County residents smoke compared to Michiganders. 18.2% of Branch-Hillsdale-St. Joseph County adult residents said they smoked on the Michigan Behavioral Risk Factor Survey, 2020-22 average, compared to 16.9% in Michigan. Despite anti-smoking programs, the percentage of smokers has remained relatively constant over the years. Smoking is a significant problem among pregnant women. Smoking during pregnancy is linked to a lower birth weight for babies as well as higher infant mortality. The Michigan Resident Birth Files reveal a larger percent of Hillsdale mothers used tobacco before or during pregnancy compared to Michigan mothers. Smoking prevention programs have had little impact on mothers at the county level over the previous decade. Some young adults have turned to vapor cigarettes instead of traditional tobacco use. Education on the long-term negative effects of vaping is often lacking. Action Plan: 1. Expand Education to Include Vaping Risks a. Update existing smoking cessation materials to include vaping content b. Train staff to discuss vaping risks during clinical visits c. Distribute vaping-specific educational brochures in schools and clinics 2. Launch Youth-Focused Prevention Campaigns a. Develop social media and school-based campaigns targeting middle and high school students b. Host assemblies or peer-led forums discussing vaping dangers c. Use real testimonials from former users or health professionals 3. Track and Share Local Data Trends a. Monitor local vaping trends through surveys and provider reports b. Share results with schools, clinics, and media to raise awareness 4. Promote Cessation Programs for Youth and Adults a. Offer vaping-specific cessation support groups or referrals b. Promote state/national quit line resources tailored to e-cigarette users c. Integrate support into primary care visits
Schedule H, Part V, Section B, Line 11 Facility , 3Facility , 3 - HILLSDALE COMMUNITY HEALTH CENTER. 3. Recommendation from HSN: Expansion of the use of Telehealth services Background: The rise of technology has given rise to many improvements in the health care system. In previous years, the HSN suggested that the development of a telemedicine system would benefit the community. A telehealth service became available in early 2021 in the Hillsdale County area through nearly all of the health clinics. Respondents in the 2022 CHNA survey were asked about it. The survey asked respondents if they had tried telehealth. Of the 816 who answered this question, only 18.6% said they used telehealth services, while 81.4% said they did not use telehealth services. Respondents were asked if they were aware Hillsdale Hospital offered telehealth services and of the 816 who answered the question 48% said they were aware, while 52% said they were not aware. Of the 525 respondents who answered why they had not tried telehealth services, 53.7% said it was because they did not know it was an option. Action Plan: 1. Launch Community Telehealth Awareness Campaign a. Create flyers, social media posts, and website updates explaining telehealth services b. Highlight common telehealth visit types (e.g., follow-ups, mental health, urgent care) c. Include information in discharge packets and clinic visits 2. Improve Telehealth Literacy a. Offer free "How to Use Telehealth" workshops at libraries, clinics, and senior centers b. Develop easy-to-follow guides and video tutorials c. Provide 1-on-1 tech coaching during clinic visits
Schedule H, Part V, Section B, Line 11 Facility , 4Facility , 4 - HILLSDALE COMMUNITY HEALTH CENTER. 4. Recommendation from HSN: Increase the number of residents that get a mammogram from 70.1 to 73 on The Michigan Behavioral Risk Factor Survey for the period 2025-2028. Background: The United States Preventive Services Task Force (2016) recommends women aged 40 to 74 should have a mammogram every two years. Women aged 40 to 49 and those at higher risk of breast cancer due to family history or another reason should follow the recommendations of their physicians. Females were asked in the 2024 CHNA survey if they had a mammogram in the past year. 332 answered this question though the survey only had 234 women. * 234 said yes. This would represent 35% of the women who took the survey. * 98 said no, probably including some men who answered the question. The Michigan Behavior Risk Factor Survey asked female residents if they had a mammogram in the last two years. The percent of Branch-Hillsdale-St. Joseph residents who said they did was much less in 2018-20 than in 2022-20. Action Plan: 1. Increase Community Awareness a. Launch a women's health awareness campaign (flyers, local media, social media) b. Promote National Cervical Cancer Awareness Month (January) c. Include Pap test reminders in patient portal messages 2. Host Accessible Screening Events a. Organize free or low-cost Pap test days in collaboration with community centers b. Offer mobile screening clinics in rural areas c. Partner with employers for workplace health screening events 3. Reduce Barriers to Access a. Provide transportation information/resources for screening appointments b. Offer extended hours and weekend availability for working women c. Provide care navigation for uninsured or underinsured women 4. Educate Providers to Encourage Screenings a. Implement provider scripts for discussing women's preventive care b. Integrate automatic Pap test reminders into electronic health records c. Encourage OB/GYNs and PCPs to flag overdue patients d. Provide educational Materials to all hospital clinics as well as the St. Peters Free Clinic in Hillsdale.
Schedule H, Part V, Section B, Line 11 Facility , 5Facility , 5 - HILLSDALE COMMUNITY HEALTH CENTER. 5. Recommendation from HSN: Increase availability of specialty care in Hillsdale; including Oncology, Neurology, Podiatry, and Infertility Specialists. Background: Hillsdale County is served by: * One hospital, the Hillsdale Hospital. * Three primary care health clinics owned and operated by Hillsdale Hospital: o Hillsdale Health and Wellness located at 240 W. Carleton Road, Hillsdale and is open seven days a week from 10 AM to 6 PM. o Reading Health Clinic, a Rural Clinic in Reading, Hospital located at 143 South Main Street, Reading, MI 49274. This clinic operates Monday, Wednesday, Thursday and Friday from 8 AM to 4 PM and Tuesday from 8 AM to 6 PM. o Hillsdale Medical Associates has two locations, one on located at 32 S. Broad St, Hillsdale and is open 4 days a week from 8 am to 6pm. The other Hillsdale Medical Associates is located at 1456 Hudson Rd, Hillsdale open five days a week from 9am to 5pm. * Hillsdale Hospital also owns and manages several other facilities in the county including: o Hillsdale Orthopedics. o Hillsdale Home Care o Hillsdale Home Oxygen & Medical Equipment o Three Meadows Medical Building * Neurology * Pulmonary. * Surgical Group. * Urology * Vascular Care * Weigh loss Surgery * Physical Therapy * Occupation Therapy * Laboratory * Speech Therapy * Hidden Meadows OB/GYN * Cancer Care * Primary Care One free clinic, St. Peter's, open only two nights a week. The hospital donates lab services and medical supplies in support of this clinic. Many hospital staff members also donate their time to serve clinic clients. Hillsdale County currently has no round-the-clock urgent care facility. All health clinics have limited hours. Schedule H, Part V, Section B, Line 11 Facility , 7Facility , 7 - HILLSDALE COMMUNITY HEALTH CENTER. 7. Recommendation from HSN: Expand mental health offerings by 15% - increasing access to telehealth, as appropriate, for the period 2025-2028. Background: Mental health impacts not just the individual but the community. Feelings of wellbeing even have a positive impact on the management and progress of some diseases. When respondents in the 2024 CHNA survey were asked about their mental health in the past year: 181 (20.4%) of the 887 respondents to this question said they had been depressed for more than a week; 13 (1.5%) of the 887 respondents to this question had been suicidal. The Michigan Behavioral Risk Factor Survey asked respondents how many felt their mental health was not good for 14 or more days out of the previous 30 days including stress, depression and problems with emotions. 15.4% of Branch- Hillsdale-St. Joseph residents said their mental health was not good compared to 16.0% of Michigan residents but both groups showed a marked increase in poor mental health than in the past. Hillsdale Hospital has a 10-bed psychiatric unit; this is the only inpatient Psych unit in the county. Examination of the hospital psychiatric patient discharge data shows a fairly consistent pattern of use. Action Plan: 1. Expand Outpatient Mental Health Capacity a. Expand clinic hours to include evenings/weekends 2. Scale Tele- Mental Health Services a. Launch or expand virtual therapy and psychiatry appointments b. Train staff and patients on how to access and use tele-mental health c. Ensure HIPAA-compliant platforms are user-friendly and mobile-accessible 3. Outreach & Mental Health Awareness a. Run a "Mental Health is Health" public education campaign b. Host community events and webinars during Mental Health Awareness Month (May) 4. Connect Patients to Services Through Referrals a. Integrate behavioral health screenings in primary care b. Train providers to recognize and refer patients to in-house or partnered services c. Create a mental health services referral guide for staff and patients.

Schedule H, Part I LINE 7, COL (F) - BAD DEBT EXPENSE EXCLUDED FROM FINANCIAL ASSISTANCE CALCULATION6,608,203
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountACCOUNTS RECEIVABLE FOR PATIENTS, INSURANCE COMPANIES, AND GOVERNMENTAL AGENCIES ARE BASED ON GROSS CHARGES. AN ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS IS ESTABLISHED ON AN AGGREGATE BASIS BY USING HISTORICAL WRITE-OFF RATE FACTORS APPLIED TO UNPAID ACCOUNTS STRATIFIED BY PAYOR AND NUMBER OF DAYS THE ACCOUNTS ARE OUTSTANDING. UNCOLLECTIBLE AMOUNTS ARE WRITTEN OFF AGAINST THE ALLOWANCE FOR DOUBTFUL ACCOUNTS IN THE PERIOD THEY ARE DETERMINED TO BE UNCOLLECTIBLE.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyHILLSDALE HOSPITAL SAW $2,146,307 OF ACCOUNTS WRITTEN OFF TO BAD DEBT THAT WERE ATTRIBUTED TO PATIENTS WITH NO INSURANCE COVERAGE. THE AVERAGE DISCOUNT APPLIED TO ACCOUNTS THROUGH OUR FINANCIAL ASSISTANCE PROGRAM WAS 94.8%. WE APPLIED THAT PERCENTAGE TO THOSE ACCOUNTS WRITTEN OFF WITH NO INSURANCE.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteTHE HOSPITAL REPORTS PATIENT ACCOUNTS RECEIVABLE FOR SERVICES RENDERED AT NET REALIZABLE AMOUNTS FROM THIRD-PARTY PAYERS, PATIENTS AND OTHERS. THE HILLSDALE COMMUNITY HEALTH CENTER PROVIDES AN ALLOWANCE FOR DOUBTFUL ACCOUNTS BASED UPON A REVIEW OF OUTSTANDING RECEIVABLES, HISTORICAL COLLECTION INFORMATION AND EXISTING ECONOMIC CONDITIONS. AS A SERVICE TO THE PATIENT, THE HILLSDALE COMMUNITY HEALTH CENTER BILLS THIRD-PARTY PAYERS DIRECTLY AND BILLS THE PATIENT WHEN THE PATIENT'S LIABILITY IS DETERMINED. PATIENT ACCOUNTS RECEIVABLE ARE DUE IN FULL WHEN BILLED. ACCOUNTS ARE CONSIDERED DELINQUENT AND SUBSEQUENTLY WRITTEN OFF AS BAD DEBTS BASED ON INDIVIDUAL CREDIT EVALUATION AND SPECIFIC CIRCUMSTANCES OF THE ACCOUNT. ACCOUNTS RECEIVABLE ARE REDUCED BY AN ALLOWANCE FOR DOUBTFUL ACCOUNTS. IN EVALUATING THE COLLECTABILITY OF ACCOUNTS RECEIVABLE, THE HOSPITAL ANALYZES ITS PAST HISTORY AND IDENTIFIES TRENDS FOR EACH OF ITS MAJOR PAYER SOURCES OF REVENUE TO ESTIMATE THE APPROPRIATE ALLOWANCE FOR DOUBTFUL ACCOUNTS AND PROVISION FOR UNCOLLECTIBLE ACCOUNTS. MANAGEMENT REGULARLY REVIEWS DATA ABOUT THESE MAJOR PAYER SOURCES OF REVENUE IN EVALUATING THE SUFFICIENCY OF THE ALLOWANCE FOR DOUBTFUL ACCOUNTS. FOR RECEIVABLES ASSOCIATED WITH SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY COVERAGE, THE HOSPITAL ANALYZES CONTRACTUALLY DUE AMOUNTS AND PROVIDES AN ALLOWANCE FOR DOUBTFUL ACCOUNTS AND A PROVISION FOR UNCOLLECTIBLE ACCOUNTS, IF NECESSARY (FOR EXAMPLE, FOR EXPECTED UNCOLLECTIBLE DEDUCTIBLES AND COPAYMENTS ON ACCOUNTS FOR WHICH THE THIRD-PARTY PAYER HAS NOT YET PAID OR FOR PAYERS WHO ARE KNOWN TO BE HAVING FINANCIAL DIFFICULTIES THAT MAKE THE REALIZATION OF AMOUNTS DUE UNLIKELY). FOR RECEIVABLES ASSOCIATED WITH SELF-PAY PATIENTS (WHICH INCLUDES BOTH PATIENTS WITHOUT INSURANCE AND PATIENTS WITH DEDUCTIBLE AND COPAYMENT BALANCES DUE FOR WHICH THIRD-PARTY COVERAGE EXISTS FOR PART OF THE BILL), THE HOSPITAL RECORDS A SIGNIFICANT PROVISION FOR UNCOLLECTIBLE ACCOUNTS IN THE PERIOD OF SERVICE ON THE BASIS OF ITS PAST EXPERIENCE, WHICH INDICATES THAT MANY PATIENTS ARE UNABLE OR UNWILLING TO PAY THE PORTION OF THEIR BILL FOR WHICH THEY ARE FINANCIALLY RESPONSIBLE. THE DIFFERENCE BETWEEN THE STANDARD RATES (OR THE DISCOUNTED RATES IF NEGOTIATED OR PROVIDED BY POLICY) AND THE AMOUNTS ACTUALLY COLLECTED AFTER ALL REASONABLE COLLECTION EFFORTS HAVE BEEN EXHAUSTED IS CHARGED OFF AGAINST THE ALLOWANCE FOR DOUBTFUL ACCOUNTS. THE HOSPITAL'S ALLOWANCE FOR DOUBTFUL ACCOUNTS IS AT 63 PERCENT OF TOTAL ACCOUNTS RECEIVABLE AT JUNE 30, 2024. IN ADDITION, THE HOSPITAL'S BAD DEBT WRITE-OFFS DECREASED FROM APPROXIMATELY $4,530,000 FOR THE YEAR ENDED JUNE 30, 2024, TO APPROXIMATELY $4,360,000 FOR THE YEAR ENDED JUNE 30, 2025.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsTHE COST TO CHARGE RATIO IS USED TO DETERMINE MEDICARE SURPLUS OR SHORTFALL AT COST. THE SHORTFALL SHOULD BE TREATED AS COMMUNITY BENEFIT AS THIS IS THE EXTENT TO WHICH COSTS RELATED TO MEDICARE SERVICES GO UNPAID.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceAFTER INSURANCE PAYMENTS, THE SELF PAY PORTION IS BILLED TO THE PATIENT ON A MONTHLY STATEMENT. THE STATEMENT INCLUDES INFORMATION ABOUT THE HOSPITAL'S FINANCIAL ASSISTANCE POLICY AND HOW TO APPLY. THE GUARANTOR RECEIVES FOUR STATEMENTS.
