Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - SAINT FRANCIS HOSPITAL, INC.. Saint Francis Hospital conducted its Fiscal Year 2025 Community Health Needs Assessment (CHNA) as part of a joint assessment led by Saint Francis Health System in collaboration with Ascension St. John and the Tulsa Health Department. The University of Oklahoma Hudson College of Public Health was retained to support data collection, analysis, and reporting to ensure compliance with Section 501(r)(3) requirements. This CHNA covered the three-year period through FY25 with an assessment period that occurred between April 2024 and January 2025. The CHNA was adopted by the Saint Francis Health System Board of Directors in April 2025. The community served by Saint Francis Hospital was defined as Tulsa County, Oklahoma, encompassing 43 zip codes, as validated in collaboration with the local public health department. Input from people representing the broad interests of the community was obtained through community engagement meetings, advisory and core group participation, and a community engagement group composed of local subject matter experts. Participants included representatives of medically underserved, low-income, and minority populations, including community-based organizations and public health agencies serving vulnerable groups. Meeting dates and participants are documented in Appendix C OF THE CHNA. Primary data collection included a 40-question community-wide survey structured within a Social Determinants of Health framework and incorporating validated screening elements, including PRAPARE. The survey was available in English, Spanish, and Burmese and was distributed over nine weeks via QR codes, hyperlinks, and randomized mailings across Tulsa County. A total of 1,499 complete surveys were collected, including 118 randomized mailings and 1,381 through stakeholder engagement efforts. Secondary data analysis was conducted using the County Health Rankings & Roadmaps framework to assess health outcomes and influencing factors. Significant health needs were identified and prioritized using established criteria, including size and severity of the problem, impact on vulnerable populations, and resource feasibility and sustainability. The prioritized needs for Tulsa County were access to healthcare, food, mental health, and housing.
Schedule H, Part V, Section B, Line 6a Facility , 1Facility , 1 - SAINT FRANCIS HOSPITAL, INC.. The CHNA Joint Assessment was conducted with Ascension St. John.
Schedule H, Part V, Section B, Line 6b Facility , 1Facility , 1 - SAINT FRANCIS HOSPITAL, INC.. The CHNA Joint Assessment was conducted in partnership with the Oklahoma State Department of Health District 4 and The Oklahoma University Hudson College of Public Health (OU COPH).
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - SAINT FRANCIS HOSPITAL, INC.. Based on insights from community engagement meetings, surveys, and resident feedback, Tulsa County's most significant health-related needs were identified. These include: * Housing - Homelessness, housing instability, and lack of affordable options, strongly tied to mortality, mental health, and safety. * Mental and Behavioral Health - High suicide rates, firearm fatalities, poor mental health days, and barriers to care access. * Access to Healthcare - Challenges with insurance coverage, medical bills, chronic disease management, and emergency preparedness, with disparities worsened during COVID-19. * Transportation - Limited options restricting access to food, healthcare, jobs, and social connections. * Education - Gaps in quality education and attainment that impact health literacy, income, and long-term well-being. * Employment - While unemployment is relatively low, underemployment and economic instability create barriers to meeting basic needs. * Food Security - Barriers to affordable, nutritious food, particularly for low-income households, often linked to transportation and poverty. * Childcare and Early Childhood Education - Limited affordable options, leading to financial strain for families and long-term impacts on child development. Following the identification of significant community health needs, Saint Francis Health System (SFHS) undertook a multi-step prioritization process involving review of survey and data findings, input from approximately 50 leaders, and discussions with Tulsa County leadership. While SFHS acknowledges the importance of all identified needs, four were selected as priorities for the formal CHNA implementation strategy: (1) Access to Healthcare, due to cost, insurance, and appointment barriers identified as the top concern; (2) Food Security, based on both internal screening data and survey findings highlighting affordability and preparation challenges; (3) Mental Health, given limited access, affordability issues, and alignment with existing SFHS behavioral health investments and the Tulsa Health Department's Community Health Improvement Plan; and (4) Housing, due to its broad impact, prominence in screening data, and strong community momentum to address homelessness. These prioritized needs reflect areas where SFHS is well positioned to lead or collaborate. The remaining needs did not reach the same level of priority as the four needs highlighted, and SFHS is not best positioned to address these needs, as other community stakeholders are already working on solutions for them. Needs are being addressed by carrying out the Implementation Strategy Plan. To improve access to healthcare in Tulsa County, we plan to mitigate barriers related to understanding the cost of care and the most appropriate site of care. This will be accomplished by organizing community education and expanding Medicaid enrollment support. To improve food insecurity in Tulsa County, we plan to extend the reach of nonprofit organizations and better understand the barriers in our communities. This will be accomplished by coordinating food distribution with local nonprofits and analyzing data by geography and demographics. To improve mental and behavioral health in Tulsa County, we plan to increase awareness and expand suicide prevention training and screening. To improve access to affordable housing in Tulsa County, we plan to educate internal stakeholders about legal aid solutions available to patients in need.

PRODUCES A CONSOLIDATED COMMUNITY BENEFIT REPORT INCLUDED IN AN ANNUAL ISSUE OF THE SAINT FRANCIS HEALTH SYSTEM MAGAZINE "PRESENCE" THAT IS MADE AVAILABLE TO THE PUBLIC THROUGH THE ORGANIZATION'S WEBSITE AT HTTPS://WWW.SAINTFRANCIS.COM/ABOUT-US/MEDIA/PRESENCE-MAGAZINE
Schedule H, Part V, Section B, Line 22 LINE 22B - AMOUNTS GENERALLY BILLEDAmounts generally billed (AGB) are determined under the look-back method based on claims allowed by Medicare fee-for-service and all private health insurers that pay claims to the hospital facility during a prior 12-month period. Additionally, a self-pay discount of 60 percent is provided on all charges for uninsured patients.
Schedule H, Part VI, Line 7 Community benefit reportThe State of Oklahoma does not require a community benefit report. SFHS publishes a community benefit report annually. A written report is included in an annual issue of the SFHS magazine "Presence", and can be found at: Presence Magazine | Saint Francis Health System
Schedule H, Part I, Line 7g Subsidized Health Services100% of Subsidized health services from worksheet 6 are allocatable to physician clinic costs
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceCOSTING METHODOLOGY: A RATIO OF PATIENT CARE COST TO CHARGES, AS DETERMINED IN WORKSHEET 2, WAS USED TO REPORT THE AMOUNTS IN PART I, LINES 7A - 7C. FOR AMOUNTS REPORTED ON LINES 7E - 7I, ACTUAL EXPENSES FOR EACH COMMUNITY BENEFIT ACTIVITY ARE TRACKED AND REPORTED USING THE ORGANIZATION'S ACCOUNTING GENERAL LEDGER AND ARE NOT BASED ON A COST TO CHARGE RATIO. THE NUMBER REFLECTED ON LINE 7, COLUMN (F) EXCLUDES BAD DEBT EXPENSE. THE SUPPLEMENTAL HOSPITAL OFFSET PAYMENT PROGRAM (SHOPP) WAS CREATED AND IMPLEMENTED IN CALENDAR YEAR 2011 FOR THE PURPOSE OF ASSURING ACCESS TO QUALITY CARE FOR OKLAHOMA MEDICAID MEMBERS. THE PROGRAM IS DESIGNED TO ASSESS OKLAHOMA HOSPITALS, UNLESS EXEMPT, A SUPPLEMENTAL HOSPITAL OFFSET PAYMENT PROGRAM FEE. THE COLLECTED FEES ARE PLACED IN POOLS AND THEN ALLOCATED TO HOSPITALS BASED ON MEDICAID REVENUES AS DIRECTED BY LEGISLATION. THE OKLAHOMA HEALTH CARE AUTHORITY (OHCA) DOES NOT GUARANTEE THAT ALLOCATION WILL EQUAL OR EXCEED THE AMOUNT OF THE SUPPLEMENTAL HOSPITAL OFFSET PAYMENT PROGRAM FEES THAT WERE PAID BY SAINT FRANCIS HOSPITAL, INC.
Schedule H, Part II Community Building ActivitiesCommunity-building activities are undertaken to improve community health and safety by addressing the root causes of health problems. These activities enhance the community's capacity to promote the health and well-being of residents through the expertise and resources of the healthcare organization. Related costs include cash contributions and expenses associated with the development of community-building programs and partnerships. Additional information regarding the community-building activities of Saint Francis Health System, Inc. and Saint Francis Hospital, Inc. is reported this Schedule H Part V, Section B, Line 5.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountSAINT FRANCIS HOSPITAL, INC. HAS AN ESTABLISHED PROCESS TO DETERMINE THE ADEQUACY OF THE ALLOWANCE FOR UNCOLLECTABLE RECEIVABLES THAT RELIES ON A NUMBER OF ANALYTICAL TOOLS AND BENCHMARKS TO ARRIVE AT A REASONABLE ALLOWANCE. NO SINGLE STATISTIC OR MEASUREMENT DETERMINES THE ADEQUACY OF THE ALLOWANCE FOR UNCOLLECTABLE RECEIVABLES. SOME OF THE ANALYTICAL TOOLS THAT SAINT FRANCIS HOSPITAL, INC. UTILIZES INCLUDE, BUT ARE NOT LIMITED TO, HISTORICAL CASH COLLECTION EXPERIENCE, REVENUE TRENDS BY PAYER CLASSIFICATION, AND REVENUE DAYS IN ACCOUNTS RECEIVABLE. ACCOUNTS RECEIVABLE ARE WRITTEN OFF AFTER COLLECTION EFFORTS HAVE BEEN FOLLOWED IN ACCORDANCE WITH SAINT FRANCIS HOSPITAL, INC.'S POLICIES.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyThe bad debt expense attributable to patients eligible under the organization's financial assistance policy is calculated from a sample review of all bad debt accounts and subsequent information. Many patients would qualify for financial assistance under the Saint Francis Financial Assistance Policy if the application process was completed, but for a number of reasons, patients are sometimes unable to complete the process. As result, their accounts end up in bad debt. For this reason, the cost associated with these accounts should be considered community benefit. Saint Francis exhausts all efforts to either collect from the patient or help them to complete the financial assistance process. After all efforts have been exhausted, Saint Francis Hospital uses data sources and quantitative analysis to identify the population of patients that, more likely than not, would have qualified for charity care had they been able to complete the financial assistance application. A cost to charge ratio is applied to the gross charges of the identified patients.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteSAINT FRANCIS HEALTH SYSTEM, INC. 73-1501972, THE PARENT ORGANIZATION OF SAINT FRANCIS HOSPITAL, INC., PRODUCES CONSOLIDATED FINANCIALS WHICH INCLUDE THE FINANCIAL POSITION AND RESULTS OF FINANCIAL OPERATIONS OF SAINT FRANCIS HOSPITAL, INC. THE CONSOLIDATED FINANCIAL STATEMENTS ARE AUDITED ON AN ANNUAL BASIS. SAINT FRANCIS HEALTH SYSTEM, INC.'S AUDITED CONSOLIDATED FINANCIAL STATEMENTS PROVIDE A SEPARATE FOOTNOTE ADDRESSING THE ORGANIZATION'S NET PATIENT ACCOUNTS RECEIVABLES ON PAGE 9. SAINT FRANCIS HEALTH SYSTEM, INC. REPORTS BAD DEBT IN ACCORDANCE WITH GENERALLY ACCEPTED ACCOUNTING PRINCIPLES (GAAP).
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsCosting Methodology: Medicare allowable costs were calculated using a cost-to-charge ratio and the Medicare filed cost report. The shortfall on Schedule H, Part III, Section B, Line 7 is considered community benefit. Saint Francis Hospital, Inc provides services to many low-income Medicare recipients. The Medicare losses sustained at the hospital are a result of Medicare reimbursing at less than operating costs. IRS Rev. Rul. 69-545 established the community benefit standard for hospitals and considers a hospital that serves patients covered by governmental health benefits (including Medicare) as operating to promote the health of the community and furtherance of its exempt purposes. Accordingly, treating Medicare patients is considered a community benefit.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceSaint Francis Hospital, Inc.'s debt collection policy is to pursue collections of patient balances from patients who have the ability to pay for the services. Saint Francis Hospital, Inc. applies its collections efforts consistently and fairly to all patients regardless of insurance. Saint Francis Hospital, Inc. works with those individuals who do not have the financial resources to pay outstanding balances to qualify for Saint Francis Hospital, Inc.'s financial assistance policy. Charges to patients qualifying for charity care under the Saint Francis Hospital, Inc. financial assistance policy are written off based on a sliding scale of family income relative to the federal poverty level such that persons with an annual family income up to 250% of the federal poverty level receive a 100% discount, persons with an annual family income between 251% and 300% of the federal poverty level receive an 80% discount and persons with an annual family income 301% to 400% of the federal poverty level receive a 70% discount. The policy is described on the website at: HTTPS://WWW.SAINTFRANCIS.COM/PATIENTS-AND-GUESTS/FOR-PATIENTS/BILLING-AND-INSURANCE/FINANCIAL-ASSISTANCE

Schedule H, Part VI, Line 2 Needs assessmentUsing a mixed-methods approach grounded in a Social Determinants of Health framework, the CHNA incorporated a community survey, direction from a community advisory board, stakeholder engagement meetings with local leaders and organizations, and secondary data analysis using County Health Rankings and Roadmaps (CHRR). A structured governance model was established to ensure broad input and accountability. The core group oversaw survey development, data analysis, and prioritization of health needs. Advisory groups, including the local county health departments, provided additional guidance and alignment. Engagement groups, consisting of community subject matter experts and organizational stakeholders, were convened at key stages to provide context, interpret findings, and validate emerging themes. This structure ensured that community voices were central to the process and that the final CHNA reflected both quantitative data and the lived experiences of the communities served. Saint Francis Hospital utilizes its Strategic Planning department to continuously identify unmet health and clinical needs across the communities it serves. Through a methodical planning approach, Strategic Planning evaluates gaps in services using population growth and demographic trends, national, state, and county-level data sources, physician workforce supply, community resource availability, and demand pressures on existing facilities and providers. The Strategic Planning team works in collaboration with hospital leadership, community partners, public health agencies, and other healthcare providers to identify unmet physician, hospital, and community health needs. These needs are prioritized based on feasibility, availability of clinical workforce, financial sustainability, and alignment with system strategy. Based on this prioritization, capital and operating resources are incorporated into Saint Francis Hospital's facility, system budgets, and long-term strategic plans.

About 10% of the population does not have a high school diploma, 22% of the population has a high school diploma, 30% has some college or associates degree, and 35% has a bachelors, graduate or professional degree.
Schedule H, Part VI, Line 5 Promotion of community healthThese hospitals are part of an integrated healthcare delivery system with the mission of extending the presence and healing ministry of Christ in all we do. As a Catholic organization, SFHS seeks to reflect the presence of Christ in every personal and corporate encounter. SFHS is dedicated to giving back to the community in which its employees live and work. This can be seen through SFHS's promotion of community health through community events such as sponsored on-site educational seminars, classes on health issues, flu clinics, and more. SFHS's governing body is comprised of community representatives on the Board of Directors that provide leadership and governance for the organization. The Board of Directors has the overall responsibility for the charitable contributions and the mission of SFHS. The members of the Board of Directors are selected based on their areas of expertise and experience including such areas as education, research, business, and government. The members of the governing body contribute their wisdom, insights, and expertise to ensure the organization is fulfilling its mission and charitable purpose while providing efficient administrative support services and direction for SFHS. SFHS provides financial assistance in the form of charity care to patients who are indigent and satisfy certain requirements. Additionally, SFHS is committed to treating patients who are eligible for means-tested government programs such as Medicaid and other government-sponsored programs including Medicare, with care being provided regardless of the reimbursement shortfall, and thereby relieves the state and federal government of the burden of paying the full cost of care for those patients. Often, patients are unaware of the federal, state and local programs open to them for financial assistance, or they are unable to access them due to the cumbersome enrollment process required to receive these benefits. SFHS assists in enrollment to these government programs or extends financial assistance in the form of charity care through the organization's Financial Assistance Policy. SFHS reinvests its net operating income back into the facility to improve patient care, to benefit society and to allow SFHS to carry out its vision to be the regional leader in the delivery of quality Catholic healthcare services.

Saint Francis History On October 1, 1960, Saint Francis Hospital, Inc., located in eastern Oklahoma, opened with 275 beds. It was a dream of the founders William K. Warren Sr. and his wife Natalie Overall Warren to give a gift to the City of Tulsa that would serve its citizens for years to come. To meet patient care requirements, as well as the demands of a rapidly growing population in Southeast Tulsa, Saint Francis Hospital, Inc. expanded in 1969 to 735 adult and pediatric beds and bassinets. A major milestone occurred on December 3, 1975 - the 15th anniversary of the hospital - when the Natalie Warren Bryant Cancer Center opened its doors. It was one of the first centers where radiation therapy, chemotherapy services, laboratory and support services were grouped in a single location for the patient's convenience. The Natalie Warren Bryant Cancer Center is now known as the Saint Francis Cancer Center and provides state-of-the-art medical oncology and radiotherapy technology to residents in eastern Oklahoma and surrounding states. The first area Warren Clinic, Inc. facility was established in Stillwater, Oklahoma, in January 1988. Warren Clinic, Inc. has grown significantly from the three internal medicine specialists in Stillwater to 285 providers in practice throughout eastern Oklahoma. Saint Francis Hospital at Broken Arrow officially became part of Saint Francis Health System, Inc. in January 1988. The union initially occurred when Saint Francis Hospital at Broken Arrow affiliated with Saint Francis Hospital, Inc. on January 1, 1986. As the Broken Arrow community grew, the Broken Arrow facility was unable to meet the demands. In June of 2007, the hospital relocated to a new facility and is now known as Saint Francis Hospital South, LLC, it continues to serve the Broken Arrow community. A fitness park was built where the former hospital once stood for community members to enjoy. Saint Francis Hospital South, LLC opened in 2007 with 96 beds servicing the needs of Southern Tulsa and Wagoner counties in Oklahoma. Built with a neighborhood feel, the hospital offers general services, as well as many sub-specialties generally reserved for larger city hospitals including a Level I NICU, ICU, Emergency Services, Cardiology, Urology, Stroke Center and 24-hour on-site anesthesia. In 1989, the William K. Warren Foundation established Laureate Psychiatric Clinic and Hospital, Inc., as well as the Laureate Psychiatric Research Center. Laureate Psychiatric Clinic and Hospital, Inc. was opened to provide a place where those suffering from mental illness would be treated the same as patients having any other illness. In addition to programs for adolescents and adults, Laureate Psychiatric Clinic and Hospital, Inc. offers successful specialty programs for chemical dependency, mood disorders and eating disorders. Saint Francis Children's Hospital, a "hospital within a hospital," was established in 1995 and was created with pediatric patients in mind. The goal was to improve access and efficiency, as well as to provide a larger and more "kid friendly" atmosphere for children and their families. Saint Francis Children's Hospital is a regional referral center specializing in pediatric inpatient care, as well as neonatal and pediatric intensive care and offers service in more than 25 different pediatric specialties including the region's only Level IV Neonatal Intensive Care Unit, pediatric hematology/oncology clinic and a pediatric cardiac surgery program. In March 2004, Saint Francis Health System, Inc. and area cardiologists joined as partners to open the Saint Francis Heart and Vascular Institute. The need for heart services is great in Oklahoma as heart disease is the leading cause of death in Oklahoma according to a report issued by the Centers for Disease Control and Prevention National Vital Statistic Reports for 2015. Smoking, high blood pressure, high cholesterol and lack of exercise are factors that contribute to the problem and place Oklahoma high on the unhealthy list. Saint Francis Trauma Emergency Center and Patient Tower, the largest expansion in Saint Francis Hospital, Inc.'s history, opened September 2014. The facility includes acute and critical care capacity; a new eight-story, 150-bed patient care tower; a new chapel and convent; clinical education rooms; and administrative and physician office spaces. In October 2016, Saint Francis Health System, Inc. began managing the Oklahoma State University Medical Center (OSUMC). Under the terms of the 10-year management agreement, Saint Francis Health System, Inc. provides executive leadership, operational oversight and strategic direction for the hospital and its affiliated clinics and programs to provide access to healthcare to the medically underserved and rural areas in the region. The partnership allows both organizations to bolster their ability to meet the needs of the vulnerable population. Formerly Craig General Hospital, Saint Francis Hospital Vinita is located on a piece of the homestead of Mr. and Mrs. W.F. Friend, who donated the land in the early 1960s. The hospital, which opened in 1963, became part of the Saint Francis Health System in December 2016. It, along with the existing Warren Clinic Vinita and the newly named Saint Francis Health Centers located in Langley and Monkey Island, provide area residents with conveniently located primary and specialty services. In April 2017, Eastar Health System and affiliated clinics became part of Saint Francis Health System. Along with Warren Clinic physician offices, the newly named Saint Francis Hospital Muskogee provides area residents with quality care and expanding services. The hospitals 288 beds serve a seven-county area in eastern Oklahoma.
