| Morris Hospital | Part V, Section B, Line 5: Interviews were used to gather information and opinions from persons who represent the interests of the community served by the Hospital. Community stakeholders identified by the hospital were contacted and asked to participate in the needs assessment. Interviewees included individuals who are leaders and/or representatives of medically underserved, low-income, and minority populations, local health or other departments or agencies that have current data or other information relevant to the health needs of the community. Input was obtained from the Grundy County health department and the Kendall County Health Department. Fourteen interviews were completed from July through August 2022. |
| Morris Hospital | Part V, Section B, Line 6b: CHNA planned and developed in cooperation with the following organization: Grundy County Health Department. |
| Morris Hospital | Part V, Section B, Line 11: Morris Hospital has created a three-year implementation strategy that focuses on three priority health areas identified in the study: access to health care (including mental health), chronic diseases (including overweight and obesity), and health behaviors and prevention. Since Morris Hospital cannot directly address all the health needs present in the community, we will concentrate on those health needs that we can most effectively address given our areas of focus and expertise. Taking existing hospital and community resources into consideration, Morris will not directly address the remaining health needs identified in the CHNA, including birth indicators, COVID-19, and substance abuse. |
| Morris Hospital | Part V, Section B, Line 13h: Morris Hospital also designates patients as presumptively eligible by utilizing an automated system called FastTag with our early out vendor, Meduit. They use a scoring system to rank patients based off eligibility to pay and if the account is scored as presumptive, we will adjust the account off without the patient having to submit a financial application. |
| Morris Hospital | Part V, Section B, Line 16j: Morris Hospital has information on financial assistance posted in all registration areas and a copy of the plain language summary for review. Where possible, prior to the admission or rendering of service, a Financial Service Representative will conduct an interview with the patient, the guarantor, and/or his other legal representative. If unavailable to meet prior to or due to medical reasons, the patient may schedule an appointment in person or over the phone with the Financial Service Representative to discuss their options. |

| Part I, Line 3c: | Not applicable as the organization does follow federal poverty guidelines to determine eligibility for providing charity and discounted care to low-income uninsured and underinsured individuals. |
| Part I, Line 6a: | The Hospital's community benefit report is available upon request and is provided to the state agency that makes it available to the public. |
| Part I, Line 7g: | The Hospital has not included any costs attributable to physician clinics as part of their subsidized health services. |
| Part I, Ln 7 Col(f): | The Bad Debt expense included on Form 990, Part IX, Line 24(A), but subtracted for purposes of calculating the percentage in this column is $11,833,821. |
| Part III, Line 2: | Uncollectable patient accounts are written off as bad debt in the period they are determined to be uncollectible. The accounting for this is to post bad debts against the Allowance for Doubtful Accounts, which is a contra Accounts Receivable reserve to reduce the expected amount to collect. At each month end close, the Allowance for Doubtful Accounts is replenished based upon the new estimates for the Accounts Receivable that evaluates the amount expected to be collected. This estimate is based upon historic collection rates. The replenishment amount is recorded as bad debt expense on the financial statements. |
| Part III, Line 3: | Review of prior year bad debt amounts that subsequently converted to charity pursuant to the time allotted in our financial assistance policy. This method compares accounts that are actually written off to bad debt that are subsequently converted to charity. |
| Part II, Community Building Activities: | Community Coalition and Collaborative EffortsThroughout the year, Morris Hospital furthers its mission of improving the health of area residents by collaborating with other organizations in the community. Morris Hospital is able to further its mission of improving the health of area residents through a partnership with the Morris Community YMCA that has been ongoing since 2015. In addition to covering half of the facility operating expenses in 2022, Morris Hospital holds its senior exercise classes at the YMCA and encourages graduates of the Morris Hospital Phase II cardiac rehabilitation program to continue their exercise program at the YMCA so they can maintain optimal heart health. Through the YMCA's Prescription for Fitness program, Morris Hospital physicians are able to refer patients with chronic disease or certain health risks to the YMCA by prescribing a free, 30-day membership. In order to promote a healthy workforce, Morris Hospital employees can receive a free individual membership to the Morris Community YMCA, with the hospital reimbursing the YMCA for the cost. Together, the hospital and YMCA offer a Pedaling for Parkinson's class that is taught by Morris Hospital Wellness and Physical Therapy staff. In 2022, it was announced that the new YMCA that will open in Morris in 2025 will be called the Morris Hospital YMCA.As a Resource Hospital within Illinois Region VII Emergency Medical Services (EMS), Morris Hospital is licensed to provide initial education and continuing training for area emergency medical technicians and paramedics, working side by side with local EMS providers to ensure the very best pre-hospital care for local communities. Morris Hospital also serves as the Resource Hospital for Braceville Fire Protection District, Braidwood Fire Department, Coal City Fire Protection District, Dwight EMS, Dwight Fire Department, Eco Lab, Grand Ridge Volunteer Fire Department, Grundy County 911 Center, Lisbon Seward Fire Protection, Lisbon-Plattville Fire Department, Lyondell Basell Fire Department, Mazon-Verona-Kinsman Rescue Squad, Mazon Fire Department, Minooka Fire Protection District, Morris Fire Protection & Ambulance District, Newark Fire Protection District, Reddick Community Fire Protection District, Seneca Ambulance, Seneca Fire, and Troy Fire Protection District, assuring that ambulances are appropriately stocked with supplies and equipment; that EMS personnel meet all of the specified licensure qualifications, and that EMS providers comply with standard policies and protocol. Morris Hospital staff also helps to coordinate area disaster drills working in collaboration with the Grundy County Emergency Management Agency. Morris Hospital supports the Will-Grundy Medical Clinic by providing free use of office space for the Will-Grundy's free clinic each month, along with volunteer time from Morris Hospital employed physicians, and free hospital services to Will-Grundy Medical Clinic clients. Morris Hospital works collaboratively with area schools in many ways, including providing financial support for post prom activities at Morris, Dwight, and Coal City High Schools to provide safe activities for the students. In response to requests from area schools, providers from Morris Hospital went onsite to provide sports physicals at Coal City High School, Lisbon Grade School, Mazon Verona Kinsman School, Minooka Jr High School, Morris High School, Newark Middle School, Seneca Grade School, Seneca High School, and South Wilmington Grade School in 2022. The schools requested this onsite service because the convenience of the service helps the athletes better comply with the sports physical requirement in a timely manner. 342 sports physicals were performed in 2022 for an estimated community benefit of $70,938. While $20 was collected from each student who had a physical, Morris Hospital donated the money back to the athletic departments of the respective school. In 2022, Morris Hospital offered free EKG screenings to students at Morris High School, Coal City High School, and Minooka High School through its free Rhythm of Our Youth screening program, which was established to identify youth who may be at risk of sudden cardiac death. The screenings are held on site at the schools during the school day and require significant collaboration with the schools. In 2022, Morris Hospital hosted 6 students through the Grundy Economic Development Council's summer internship program. The students were paid and worked in the Emergency Department, Rehabilitation Services, 2 South nursing unit, Surgical Services, and Cardiac Diagnostics. Participation in the internship program is beneficial for both organizations, with the GEDC building the future workforce in Grundy County and Morris Hospital building its future healthcare workforce. Through a coordinated campaign that took place in November 2022, Morris Hospital employees donated $21,531 to the United Way of Grundy County through gifts of cash and PTO hours. The United Way of Grundy County has nearly 30 community partner agencies, so the collective gift from Morris Hospital employees goes very far to support health, education, youth, crisis and intervention, and basic human needs that are vital to the community. Morris Hospital also supports the United Way's annual dinner with a donation and attendance.Morris Hospital improves access to health services by providing community members free rides to medical appointments through its free Patient Transportation Service. Rides are provided regardless of whether the medical provider is associated with Morris Hospital. The service was started in 1998 in response to an identified need for increased transportation service in the county. The service is available to any community member needing a ride and is especially used by seniors and low-income community members. In coordinating the rides, Morris Hospital works in cooperation with a number of other area agencies and healthcare providers when providing this service, including Illinois Valley Industries, Elliott Manor, Saratoga Towers, the Grundy County Health Department, Morris Senior Center, Morris Library, Hospice, Catholic Charities, Will-Grundy Medical Clinic, Grundy County Housing Authority, Narvick House, Veterans Assistance, We Care of Grundy County, Morris Community Dialysis, DaVita Dialysis, Grundy Transit, Gardner American Legion, and area home health agencies. Many of these riders have indicated that they have no other way to get to their appointment.Through its commitment to supporting the community's future healthcare providers, Morris Hospital devotes staff time mentoring students who are studying to become nurses, paramedics, respiratory therapists, pharmacists, lab and radiology technologists through Joliet Junior College, College of DuPage, Illinois Valley Community College, Kankakee Community College, Lewis University, Midwestern University, University of St. Francis, Rasmussen University, Chamberlain University, Midwestern University, Illinois State University, St. Ambrose University, Benedictine College, Elmhurst University, Aurora University, Grundy Vocational Center, and more. It is imperative that these students have a place to complete their critical training. By helping students train in preparation for entering the healthcare workforce, we are helping to improve access to healthcare services in the future.Morris Hospital provides financial donations to a number of community organizations whose missions closely mirror the Hospital's. In 2022, donations were provided to: Joliet Junior College Foundation, the Healthy Kids Running Series, Living Alternatives Pregnancy Resource Center in Coal City, Greater Joliet Area YMCA, Starved Rock Regional Center for children with developmental disabilities, Lightways Hospice, Will-Grundy Medical Clinic, Grundy County Emergency Management, Grundy County Health Department, NAMI Will-Grundy, CASA of River Valley, Aden Lamps Foundation for safe sleep packets for babies, Channahon-Minooka Rotary for their school backpack program, National Fire Safety Council for fire safety education materials in Coal City, Morris Lions Club, United Way of Eastern LaSalle County, and Grundy County Lending Closet. Morris Hospital collaborates with the Illinois Department of Aging by sponsoring local volunteers from their Senior Health Insurance Program (SHIP). The program provides free health insurance counseling for Medicare beneficiaries and their caregivers, with the SHIP volunteers certified by Medicare to educate consumers and answer questions about Medicare, Medicare supplements, prescription drug plans, and Advantage plans. Morris Hospital provides free meeting space for an Epilepsy Support Group offered through the Illinois Valley Epilepsy Advocacy Network. |
| Morris Hospital collaborates with We Care of Grundy County each year | by providing volunteers to ring the Salvation Army bell at holiday time, with the money collected going to We Care. The hospital also supports the United Way of Eastern LaSalle County by providing volunteers to rehab a home at their annual Labor of Love day. |
| Part III, Line 4: | The following information can be found in the attached audited financial statements on page 7, significant accounting policies for accounts receivable:Accounts receivable from patients, insurance companies, and governmental agencies are based on gross charges, reduced by explicit price concessions provided to third-party payors, discounts provided to qualifyingindividuals as part of our financial assistance policy, and implicit price concessions provided primarily to self-pay patients. Estimates for explicit price concessions are based on provider contracts, payment terms for relevant prospective payment systems, and historical experience adjusted for economic conditions and other trends affecting the hospital's ability to collect outstanding amounts. For receivables from self-pay patients, which include both patients without insurance and patients with deductible and copayment balances due for which third-party coverage exists for part of the bill, the hospital records significant implicit price concessions in the period of service on the basis of its past experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. |
| Part III, Line 8: | Morris Hospital continually strives to provide excellent patient care in the most cost-effective fashion. Nonetheless, the Medicare program, in many cases, does not provide payment that covers the full cost of the care provided. Since it is the mission of the hospital to respond to community need, hospital management continually advocates for improved Medicare payment so that the cost of quality care to those patients that are not able to afford it is not compromised and is fairly subsidized by all payers. While this shortfall in Medicare payments is not classified as community benefit by the IRS, we nonetheless believe it is an important contribution made by the hospital to the health and wellbeing of the community. Our mission calls us to serve all patients with the highest possible quality and efficiency, even if we are not paid fully for doing so.Morris Hospital uses the information obtained from its most recent Medicare Cost Report to calculate the program costs; the methodology utilizes direct costs for each department plus the overhead cost assigned/allocated using simple step-down accounting methods. A series of activity drivers are assigned to each clinical area and allocated to each department as a percent of total overhead costs for each driver. Examples of drivers to allocate overhead (support departments) includes square footage, patient days, dietary meals prepared, and other unique units to allocate support costs to each clinical area. |
| Part III, Line 9b: | Morris Hospital & Healthcare Centers will not engage in any extraordinary collection actions (ECAs), either directly or by any debt collection agency or other representative, before reasonable efforts are made to determine whether the patient is eligible for assistance under MHHC's Financial Assistance Policy. Patients are given 120 days from their first statement to work with us on a payment plan or determine if they're eligible for financial assistance and before placing with a bad debt agency. Prior to placing with a collection agency, we use a presumptive scoring system to scrub those accounts for financial assistance. For any account already placed with an agency, they have 240 days from the first statement date to still apply for financial assistance. If a Financial Assistance Application (whether complete or incomplete) is submitted by a patient at any time during the application period, MHHC will suspend any ECAs underway for so long as the patient's Financial Assistance is pending. |
| Part VI, Line 2: | In compliance with provisions of the patient protection and affordable care act and internal revenue service regulations for tax-exempt hospitals, Morris Hospital conducted a community health needs assessment in 2022 through Biel Consulting, Inc., and through the oversight of the Morris Hospital Vice President - Professional Services, Morris Hospital Planning Analyst, and Morris Hospital Health Educator.Primary data collection was gathered through targeted interviews from persons who represent the broad interest of the community served by the hospital. This included leaders and representatives of medically underserved, low-income and minority populations, including representatives from Coal City School District, Grundy County, LaSalle County, and Kendall County Health Departments, Channahon Park District, and area healthcare providers. Secondary data was collected from a variety of local, county, and state sources including the US census, American Community Survey, Illinois Department of Public Health, Illinois Department of Employment Security, National Cancer Institute, County Health Rankings, Illinois Youth Survey, Illinois State Police, and others.Like previous years, heart disease, cancer, stroke, and lung disease continue to surface as the leading causes of death in the communities served by Morris Hospital. Whereas the last community health needs assessment showed that over one-quarter of adults in the counties served by Morris Hospital (Grundy, Kendall, LaSalle, Livingston, and Will) were overweight and close to one-third were obese, new data shows close to one-third of adults are overweight and one-third of those 20 years of age or more are obese. These percentages equate to over 60% of the adult population being overweight or obese. In Grundy County, 29.3% of the adult population is overweight and 33% are obese. Here are some of the other key findings:- In Grundy County, 14% of adults had frequent mental distress. 18.9% of Grundy County adults rated their mental health as not good' for 1 to 7 days in the past month. Mental health ratings of not good' for 8 to 30 days in the past month were reported by 14.5% of Grundy County adults. Mental or physical health issues limited the activities of 12.7% of Grundy County adults for 1 to 7 days out of the past 30 days. Activities were limited for 11.0% of Grundy County adults for 8 to 30 days of the past 30 days.- Service area counties have a higher overall cancer incidence rate than the state rate. Among service area counties, Grundy County has the highest incidence rate for all cancers (546.8 per 100,000 persons), the highest incidence rate of female breast cancers (157.7 per 100,000 persons), leukemia (18.7 cases per 100,000 persons), colorectal cancer (51.0 per 100,000 persons), esophageal cancer (8.2 per 100,000 persons), and prostate cancer (144.3 per 100,000 persons) among service area counties. Livingston County has the highest rate of lung and bronchial cancers (86.6 per 100,000 persons).- 35.3% of adults in LaSalle County have been diagnosed with arthritis, which is higher than the state rate of 24.7%. Among adults, 15.6% in Grundy County have been diagnosed with asthma, which is higher than the state rate of 8.2%. 11% of LaSalle County adults have been diagnosed with diabetes, which is lower than the state rate of 11.3%. Grundy County (6.7%), LaSalle County (5.3%), Will County (4.1%), and Livingston County (3.8%) have higher rates of coronary heart disease than the state (3.6%). - The rate of excessive drinking, defined as binge drinking (five or more drinks per occasion for males and four or more drinks per occasion for females), or heavy drinking (daily average of more than one drink for women and more than two drinks for men) in one area county exceeds the state rate of 23% and is equal to the state rate in two counties.- Smoking continues to be a leading cause of preventable death in the United States. Smoking rates in the service area range from 13% in Kendall County to 21% in Livingston County. The healthy people 2030 objective for smoking is 5%. The assessment also includes the county health rankings which are based on adult smoking, obesity, physical inactivity, excessive drinking, sexually transmitted infections, and others. Illinois counties are ranked from 1 (most healthy) to 102 (least healthy). Of the five counties served by Morris Hospital, Kendall County is ranked 3rd healthiest in the state, Will County is ranked 5th, Grundy County is ranked 12th, LaSalle County is ranked 47th and Livingston County is ranked 70th. As a result of the review of needs and application of the above criteria, Morris Hospital will address: access to health care (including mental health); chronic diseases (including overweight and obesity); health behaviors and prevention through a commitment of community benefit programs and charitable resources. An entire copy of the 2022 community health needs assessment is available for public viewing on the hospital website, https://www.morrishospital.org/wp-content/uploads/2022/12/Morris-Hospital-2022-CHNA-Report.pdf. Paper copies are available by calling 815-705-7009 or by filling out the form under contact us on the hospital's website and typing "community health needs assessment" in the questions/comments field. |
| Part VI, Line 3: | As stated in the provision of financial assistance policy, Morris Hospital & Healthcare Centers is a not-for-profit, tax-exempt entity with a charitable mission of providing emergency and medically necessary health care services to residents of the City of Morris and the Hospital's defined primary and secondary service area, regardless of their financial status and ability to pay. The purpose of the policy is to ensure that processes and procedures exist for identifying and assisting patients whose care may be provided without charge or at a discount commensurate with their financial resources and ability to pay. When possible, a financial service representative conducts an interview with the patient, the guarantor, and/or his or her legal representative, prior to the admission or rendering of service. If an interview is not possible prior to the admission or rendering of service, the interview is conducted upon admission or as soon as possible thereafter. In the case of an emergency admission, the evaluation of payment alternatives may not take place until the required medical care has been provided. At the time of the initial patient interview, the financial representative will gather routine demographic information and information regarding all existing third-party coverage to determine if the patient qualifies for medical assistance from other existing financial resources such as Medicare, Medicaid, kid care, family care, Will-Grundy Medical Clinic, or other state and federal programs. In 2018, the hospital instituted a point of service collection process in which patients are asked to make a modest deposit toward their out-of-pocket expense at the time of service. Having the financial conversation in advance of providing service enables the registration staff to better identify patients who may qualify for financial assistance so the paperwork can get started in advance, rather than waiting until after the patient is billed. The Morris Hospital website prominently features a link to financial assistance from the home page. | Form 990, Part VI, Section B, line 12c | Conflict of Interest Disclosure A. Duty To Disclose. In connection with any actual or possible conflicts or dualities of interest, an interested person must disclose in writing the existence of his or her financial interest and all material facts to the directors and committee members considering the proposed transaction or arrangement. "Disclosure" as used in this policy shall mean providing promptly and fully to the appropriate persons a written description of the facts comprising the real or apparent conflict or duality. An annual disclosure statement shall be circulated to all persons to whom this policy applies to assist them in considering such disclosures, but disclosure is appropriate whenever conflicts or dualities arise. The written disclosure notices of conflicts or dualities shall be filed with the president/chief executive officer of the corporation, or any other person designated by the foregoing from time to time to receive such notifications. B. Determining Whether A Conflict Of Interest Exists. A disclosure may result in an actual conflict, or a duality of interest exists. If it does not, after disclosure of the financial interest and all material facts and after any discussion with the interested person, he or she shall leave the board or committee meeting while the determination of a conflict or duality of interest is discussed and voted upon by the disinterested directors. Such remaining and disinterested board or committee members shall decide if a conflict of interest exists. C. Procedures For Addressing The Conflict Or Duality Of Interest. 1. An interested person may make a presentation at the board or committee meeting, but after such presentation, he or she shall leave the meeting during the discussion of, and the vote on, the transaction or arrangement that results in the conflict of interest. 2. The chair of the board or committee shall, if appropriate, appoint a disinterested person or committee to investigate alternatives to the proposed transaction or arrangement. 3. After exercising due diligence, the board or committee shall determine whether the corporation can obtain a more advantageous transaction or arrangement with reasonable efforts from a person or entity that would not give rise to a conflict or duality of interest. 4. If a more advantageous transaction or arrangement is not reasonably attainable, under circumstances that would not give rise to a conflict or duality of interest, the board or committee shall determine by a majority vote of the disinterested directors whether the transaction or arrangement is in the corporation's best interest and for its own benefit, and whether the transaction is fair and reasonable to the corporation, and shall make its decision as to whether to enter into the transaction or arrangement in conformity with such determination. If the interested person is a disqualified person under IRC 4958, the board shall follow the prescribed procedure for entitling the corporation to a rebuttable presumption of reasonableness. D. Proscribed Activity By Persons Having Conflicts Or Dualities. When a disclosure concedes or the board or committee finds that an individual director, officer, agent, or employee has a conflict or duality, such individual shall, in addition to filing the disclosure notice required hereunder, abstain from making motions, voting, executing agreements, or taking any other similar direct or indirect action on behalf of the corporation or any affiliate where the conflict of duality might pertain, but shall not be precluded from all debate on the matter. When any interested person requests in writing, or upon its own initiative, the board of directors at any time may establish further guidelines consistent with the interests of the corporation for the resolution of any real or apparent conflict provided such guidelines are consistent with the corporation's nonprofit and tax-exempt status, and compliant with applicable federal and state laws and regulations. E. Violations Of The Conflicts Or Dualities Of Interest Policy. 1. If the board or committee has reasonable cause to believe that an interested person has failed to disclose actual or possible conflicts of interest, it shall inform the person of the basis for such belief and afford him or her an opportunity to explain the alleged failure to disclose. 2. If, after hearing the response of the person and making such further investigation as may be warranted in the circumstances, the board or committee determines that the person has in fact failed to disclose an actual or possible conflict of interest, it shall take appropriate disciplinary and corrective action. F. Records And Proceedings. The minutes of the board and of all board committees shall contain: 1. The names of all persons who disclosed or otherwise were found to have a financial interest in connection with an actual or possible conflict or duality of interest, the nature of the financial interest, any action taken to determine whether a conflict or duality of interest was present, and the board's or committee's decision as to whether a conflict or duality of interest in fact existed. 2. The names of all persons who were present for discussions and votes relating to the transaction or arrangement, the content of the discussion including any alternatives to the proposed transaction or arrangement, and a record of any votes taken in connection therewith. G. Compensation Committees. 1. A voting member of the board of directors who received compensation, directly or indirectly, from the corporation for services, is precluded from voting on matters pertaining to that member's compensation. 2. A physician who is a voting member of the board of directors and receives compensation, directly or indirectly, from the corporation, for services is precluded from discussing and voting on matters pertaining to that member's and other physicians' compensation. No physician, either individually or collectively, is prohibited from providing information to any committee regarding physician compensation. H. Annual statements.
