DescThe P.U.L.S.E. Framework is a tiered, community intervention at the patient level (through the Community Referral Coordinator Program) and the systems level (through the Transitions of Care Task Force. The CRC Program uses Community Referral Coordinators to connect patients in inpatient units and/or emergency departments of hospitals with a primary care provider for follow-up and preventative care. CRCs work to ensure established Community Health Center patients are reconnected to their primary care homes as well as helping those identified patients with chronic diseases establish PCP care following a hospitalization. The goals of the program are to (1) enhance access to a primary care home and health resources for all patients regardless of ability to pay, (2) reduce non-emergent use of emergency departments and low acuity readmissions, (3) enhance continuity of care, and (4) strengthen communications/processes among safety net providers.Efforts To Outcomes - ETO is a web-based, customizable data management software. The Organization currently uses ETO to manage outcomes across IHN programs and initiatives. ETO is a HIPAA compliant tool that helps CRCs and Community Health Workers to track patient/clients outreach and follow-up efforts to improve utilization of healthcare services, promote wellness and reduce recidivism for the Re-Link regional initiative.Behavior Health Network Partnership - The Behavior Health Network contracts the Organization to design and implement the web-based data tracking system for the BHN's Hospital-Community Linkages Programs patterned after the Community Referral Coordinator Program.Project Network Connect - An initiative to build and expand secure health information data elements ranging from insurance information to clinical indicators warehoused by the Missouri Primary Care Association. This robust data repository will be valuable for future quality reporting initiatives and will position St. Louis region safety-net providers to participate in any future regional or state Health Information Exchange (HIE) developments.Transitions Of Care (TOC) - The TOC Task Force is a cross-functional and collaborative group that works to foster effective transitions of care for patients. The Task Force facilitates communication and process improvements between providers, hospitals, patients, and other safety net institutions. The TOC Task Force is charged with oversight of the CRC program. The main goal of the Task Force is to promote and sustain efficient patient transitions between the hospital (inpatient or emergency department) and the outpatient setting (primary care or urgently-needed specialty care) through improved communication and coordination across health care organizations and with patients.Desc

DescPipeline to Compassionate Care formerly known as Saint Louis University Institute For Medical Education Research A two-year partnership funded by the Institute for Medical Education and Research with three primary goals: (1) further integrate topics on care delivery to the underserved into MS1 curriculum as appropriate and feasible, (2) add an inter-professional education component so students from multiple health professions can explore team based care together, and (3) provide longitudinal applied learning opportunities for students of health professions who are interested in underserved populations.Safety Net Marketing - Safety Net Marketing are designated funds remaining from the 2011-2012 Marketing efforts. The funding is available to promote IHN, the network of providers within IHN and increase public awareness of available health resources and the way to navigate these resources.
