DescInpatient Services - The Uniontown Hospital has146 adult, pediatric, and nursery level II beds. The Hospital delivered state-of-the-art primary inpatient services to approx. 8,200 inpatients during FY 2017. Since 1903, the Hospital has been serving the greater Uniontown and Fayette County area, providing quality healthcare regardless of the persons ability to pay. The Hospital has earned several recognitions from HealthGrades, the nations leading independent healthcare ratings organization. The Hospital draws the majority of its patients from Fayette County. Although the population of the County has been decreasing, the demand for the Hospitals services remain due to the downsizing and service cuts at nearby facilities along with the growth in both the regions uninsured and elderly populations. The Uniontown Hospital offers lines of services that are unavailable elsewhere in Fayette County. In addition to being the primary source of orthopedic care, it is the only hospital to offer obstetrics, interventional cardiology and radiation oncology services.Desc

Group Uniontown Hospital Line Part V, Section B, Line 5BKD, LLP conducted a formal community health needs assessment on behalf of the Uniontown Hospital. Interviews were performed by BKD personnel between October 2015 and February 2016 with 15 key stakeholders. Stakeholders were determined based on a their specialized knowledge or expertise in public health, b their involvement with underserved and minority populations or c their affiliation with local government, schools and industry. The following is a complete list of those consulted Darlene Centofanti, Past /Director, Fayette County Department of Public Welfare Gina DAuria, Administrator, Fayette County Children and Youth Nancy Decker, President/CEO, Laurel Business Technology Lisa Ferris, CEO, Fayette County Behavioral Health Administration Melissa Ferris, Assistant Executive Director, Fayette County Drug Alcohol Commission Dr. Jeffrey Frye, Medical Director, Uniontown Hospital Dr. Larry Glad, Medical Director, UPMC Jana Kyle, Executive Director, Fayette County Drug Alcohol Commission Melissa Minor, Board Member, Children Health Improvement Partnership Dr. Richard Pish, President, Medical Executive Committee Gwendolyn Ridgley, Supervisor, Southwestern Area Agency on Aging Jim Stark, Executive Director, Fayette County Community Action Agency Dana Valente, Manager, Fayette County Department of Public Welfare Jesse T. Group Uniontown Hospital Line Part V, Section B, Line 11The Uniontown Hospitals most recently conducted Community Health Needs Assessment and Implementation Plan adopted in tax year 2015 can be found at www.uniontownhospital.com. The health needs identified by the CHNA were reviewed and priority areas were determined by hospital management. Priority 1 Healthy Behaviors. Goal Work to improve the behaviors towards health care and lifestyle choices in the community. Strategies to be implemented FY 2017 - 2019 A Work with the local YMCA to develop and maintain a cardiac rehab/maintenance program and consider developing other related programs. Year 1 Progress - Continued relationships with Phase III Cardiac Rehab. B Develop an end of life/palliative care program to provide the patient/family with the resources needed to help in decision making and communication. Year 1 Progress - Educated all Social Workers in Palliative Care. Continue with Advance Directive Education. Anticipate educating Pastoral Care Dept. Added a family section to all ICU policies. C Further develop patient/family centered care education model to include families in the patients care. Year 1 Progress - Developed Transition/Discharge model to enhance discharge of patient. D Develop patient educational materials. Year 1 Progress - 1 Technology Review A Committee analyzed products through two separate vendors Truven Health 3/23/17 and Wolters Kluwer 3/24/17. After reviewing the programs, there was a collective decision made to keep the current Krames product and investigate how it could be better used in the organization. Future goals are for the Education Department to determine the use of Krames, review curriculum needs, and develop stategies to offer alternative education delivery, ie videos through IPad use, etc. 2 Education Materials Development In absence of a diabetic educator, the Education Department has embraced and revised the process for diabetic patient teaching in the organization. Nurses providing care will document in Cerner, specifics regarding the patient, to initiate diabetic teaching based on individual need instead of just a history of diabetes. A formal referral process was developed to meet the needs of the patient as he/she transitions home.
Group Uniontown Hospital Line Part V, Section B, Line 11E Coordinate an annual health/wellness event for the community. Year 1 Progress - Uniontown Hospital is proud to partner with the City of Uniontown and the Uniontown Redevelopment Authority to bring a series of health education events to the community through our annual Storey Square Summer Concert Series. Consisting of 12 musical concerts throughout the summer, each event showcases health needs and health topics in an effort to educate and assist the public, from heart health to information regarding mammography and regular diagnostic testing to ensure the best health of area residents. Employees, clinicians and physiciians attend these weekly outdoor gatherings to provide literature and insight to those in attendance regarding the featured health topic, to engage and assist when possible, and to conduct limited testing and screening, when applicable. F The Community Engagement team of the Uniontown Hospital will engage in fundraisers to support one charity event per year at a minimum such as, the American Cancer Societys Relay for Life. Year 1 Progress - During the Fiscal Year ended June 30, 2017 the Community Engagement Team held a vendor fair and raised money for the American Cancer Societys Relay for Life. Through the UH Community Engagement Team the Hospital also sponsored Fayette County Wednesday Walks that were held during the summer to promote healthly behaviors. The Community Engagement team also raised donations of food, toiletries and hats gloves for local organizations such as Children and Youth, City Mission, St. Vincent DePaul and the Salvation Army to insure healthy living. G Partner with local health care assistance programs such as Cornerstone Care and health-related charitable organizations like the American Heart Association and the March of Dimes to provide support and assistance in local health care initiatives and educational efforts. Year 1 Progress - A Uniontown Hospital representative went to the State Capitol with AHA to advocate for Hands Only CPR. Currently, we are educating all Uniontown Hospital employees. Afterwards, we plan to reach out to the community. Additionally, a Uniontown Hospital representative is the Fayette County chair of the March of Dimes, she is working closely with the organization to raise funds and support all the research recommendtions of the MOD within our Family Beginnings Birthing Center.
Group Uniontown Hospital Line Part V, Section B, Line 11Priority 2 Social and Economic Factors. Goal Continue to decrease the number of uninsured people in the communtiy and those with limited insurance. Strategies to be implemented FY 2017 - 2019 A Increase financial assistance discussions in the Emergency Department with the development of a discharge discussion desk. Year 1 Progress - We are providing on-site real time assistance to help our patients apply for Medical Assistance, Marketplace Health Plans and our own Charity Care Program. We submit the applications for our patients and help them navigate through the process. B Annually evaluate the Uniontown Hospitals Charity Care Program and adjust related policies and procedures. Year 1 Progress - The Financial Assistance Policy and Procedures and the Revenue Cycle Principles and Policies are both reviewed annually and then approved by our Board of Directors. C Track and improve statistics related to services provided by external resources regarding self-pay accounts and pre-service collection, charity care applications, and all financial assessments. In addition, monitor services provided to assist with Financial Assistance applications. Year 1 Progress - Tracking, monitoring and analysis of these items is compiled and reviewed on a monthly basis by the Executive Director of the Revenue Cycle and the Director of Financial Access/Registration. Necessary adjustments are made based on the review.
Group Uniontown Hospital Line Part V, Section B, Line 11Priority 3 Access to Health Services. Goal Increase access to healthcare through availability of physicians and alternative healthcare locations. Strategies to be implemented FY 2017 - 2019 A Significantly expand the availability of primary care physicians. Year 1 Progress - We have added two primary care physicians to our community. B Increase the number of physician specialists including a, Neurologist, General Surgeon, Pulmonologist, Orthopedic Surgeon, Urologist, and Neurosurgeon. Year 1 Progress - We have added two new general surgeons and a urologist has joined the local urology group. Anticipating the start of two new orthopedic surgeons. C Open additional outpatient service centers throughout the county starting with Brownsville. Year 1 Progress - We explored a few sites to expand our outpatient service centers into Brownsville. No viable location was found. We continue to explore other options in the county.
Group Uniontown Hospital Line Part V, Section B, Line 11Some issues identified through the Community Health Needs Assessment have not been addressed in this plan. In initial discussion and subsequent prioritization, Uniontown Hospital considered the levels to which some needs were already being addressed in the service area. Additionally, some community needs fall out of the scope of expertise and resources of Uniontown Hospital. The following outlines how some of the needs identified in the assessment are addressed by others or in different ways A Substance Abuse - Mental health and substance abuse is not an expertise of the Uniontown Hospital. There is a small hospital on the border of the county which is dedicated to mental health and substance abuse issues and may focus on these issues. B Poor Nutrition/Access to Healthy Food Options - The Hospital is an active supporter of The Be Local Network program that is operated by a group of local media companies and community organizations who are dedicated to improving the climate of the local economy and to build a community of buyers that understands the importance of investing in local businesses and corporations. Part of this program focuses on healthy eating and consumption of healthy foods from the local farmers.
Group Uniontown Hospital Line Part V, Section B, Line 13hEligibility criteria will be based on whether the patient is uninsured or underinsured. For uninsured, liquid assets may be considered for accounts over 5,000. For underinsured, liquid assets may be considered for personal account balances over 5,000.
Group Uniontown HospitalPart V, Section B, Line 16i, There are no groups with limited english proficiency that rise to the threshold required under IRC Section 501R. However, language translations of the financial assistance policy and application are available by request at the financial counseling department of the hospital.
Group Uniontown Hospital Line Part V, Section B, Line 16jNotice of financial assistance programs are included in the Uniontown Hospital Service Directory and in the Patient Information Books in each inpatient room. There is a paragraph on the second patient statement discussing free or discounted services and directing patients to the proper location to see the FAP. Brochures are available in the emergency waiting room, Patient Financial Services and admissions office which discuss the FAP.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I Line 3cEligibility for the Uniontown Hospital Community Charity Care Program will be considered for those individuals who are uninsured, underinsured, ineligible for any government health care benefit program, and who are unable to pay for their care, based upon a determination of financial need. In addition to FPG, consideration of other resources available, such as assets and other financial obligations may be reviewed. For uninsured Liquid assets may be considered for accounts over 5,000. For underinsured Liquid assets may be considered for personal account balances over 5,000. Eligibility exceptions will exist for those patients that are eligible but failed to enroll for subsidized marketplace health plans during open enrollment periods or apply for Medicaid. Other exceptions may exist for certain other situations such as need of patients who have exhausted their insurance benefits, face extraordinary medical costs, and exceed financial eligibility criteria.
Part I Line 7Uniontown Hospital used the best available data to calculate the cost of charity care reported in line 7 for certain categories, the best available data was derived from a cost accounting system in other categories, specific cost-to-charge ratios from worksheet 2 were calculated and applied to that category.
Part I Line 7column f - The amount of expenditures used to calculate column f were reduced by bad debt expense of 3,076,213.
Part III Line 2The Gross charges of Hospital provided services to individuals whom were determined to be ineligible under the Hospitals financial assistance policy for which payments were not received. These charges have been determined uncollectible by management.
Part III Line 3Line 3 was estimated by reviewing data from the PA Department of Health, Bureau of Health Statistics and Research to determine the percentage of the population in the Hospitals service area that falls below federal poverty guidelines. This percentage was then adjusted by subtracting the Hospitals average percentage of individuals eligible under the organizations financial assistance policy. The resulting percentage was applied to line 2 to estimate line 3. Rationale for including bad debt amounts in community benefit It is the Hospitals contention that the cost of bad debt should be stated in Part 1 Line 7 of Schedule H as they represent the costs of provision of services to patients for which the entity has exhausted all recourse for reimbursement. The service provided to patients who present themselves are provided regardless of a patients ability to pay as in line with the organizations charitable mission and service to our community.
Part III Line 4The audited financial statements do not contain a footnote that describes bad debt expense. They do, however, contain a footnote that describes patient accounts receivable. That footnote reads as follows Accounts receivable are reduced by an allowance for uncollectible accounts. In evaluating the collectability of accounts receivable, the Health System analyzes its past history and identifies trends for each of its major payer sources of revenue to estimate the approriate allowance for uncollectible accounts and provision for uncollectible accounts. Management regularly reviews data about these major payer sources of revenue in evaluating the sufficiency of the allowance for uncollectible accounts. The difference between the standard rates or the discounted rates if negotiated or provided by policy and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for uncollectible accounts. See page 9 of the attached financial statements for the patient accounts receivable footnote in its entirety.

This includes helping them understand what services are covered by their health insurance, providing assistance in obtaining health insurance and helping seek eligibility for financial assistance. Convenient office hours, in a convenient location, will be established for patients seeking assistance. Currently this program is called the Uniontown Hospital Community Charity Care UHC3.
