Schedule H, Part V, Section B, Line 3ETHE SIGNIFICANT HEALTH NEEDS OF THE COMMUNITY ARE IDENTIFIED AND PRIORITIZED THROUGH THE CHNA.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - St. Bernard Hospital. To provide oversight of the CHNA process, St. Bernard Hospital convened a CHNA Advisory Committee comprising nine individuals that represented a blend of various areas of the hospital's clinical staff. The CHNA Advisory Committee received the recommendations that came out of the work with the focus groups. The focus groups were a blend of internal and external parties. The focus groups were vital, to ensure that the final CHNA product, reflected the needs that the Hospital and community leaders uncovered during their regular operations. The 2 focus groups were constituted to provide input on the service area broken down into 2 demographics, residents younger than 50 year and those 50 years and older. Focus Groups members were stakeholders who function in STBH's service area. The focus groups included a total of 22 community stakeholders. The various groups were selected and recruited with the goal of achieving a diverse cross-section of the community, including representation of seniors, community leaders, and hospital patients. While no group this size can represent the experience or perspectives of the whole community, the participants reflected a broad array of sectors, ages, and lived experiences. In addition to residents, participants had the following affiliations: Greater Englewood Chamber of Commerce Imagine Englewood If... Grow Greater Chicago Bright Point Family Center Growing Home Prince of Peace Church Englewood Senior Community Health Advocate Gift from God Ministry (community Nurse and local pastor) Center for Better Aging STBH Patient Family Advisory Council STBH staff and patients
Schedule H, Part V, Section B, Line 6b Facility , 1Facility , 1 - St. Bernard Hospital. St. Bernard Hospital worked with Sinai Urban Health Institute (SUHI) a nationally recognized community research center that works in partnership with community organizations to identify and address health inequities to develop the 2024 CHNA. SUHI is the community research arm of Sinai Health System, located on the West Side of Chicago. The team that worked on the St. Bernard Hospital CHNA included Rebecca Metrick, Melissa Gutierrez-Kapheim, Madison Offstein and Alyn Dougherety.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - St. Bernard Hospital. Excerpts from the Community Benefits Reports that describes how St. Bernard Hospital is responding the to identified community health needs identified by the 2024 CHNA Mental Health, Behavioral Health and Substance Abuse The clinical team capitalizes on every opportunity to educated the public about the services offered by the Hospital and increase awareness of Mental and Behavioral Health. During 2024, the team visited a number of community events talking about this health condition and connecting it to the real-life situations that residents experience daily. They also educated mothers at the Hospital's annual community baby shower about postpartum depression and options for treatment. The Hospital recently expanded its mental health care services to include outpatient trauma-informed mental health care including individual therapy, telehealth, family therapy, and more. In support of substance abuse, the hospital provides space at no cost to the substance abuse support group every Saturday so that the members can continue on their path to a clean lifestyle with support from former addicts. Heart Disease St. Bernard operates a cardiology clinic in our ACC to address the needs of patients with these conditions discharged from the Hospital and referred by their primary care physician. Through a partnership with Rush University Medical Center, St. Bernard is able to transfer patients with stroke symptoms from the emergency room directly to them. Respiratory Disease (Lung Disease and Smoking) St. Bernard continued working with the Respiratory Health Association to reduce tobacco use on the South Side of Chicago. During 2019 St. Bernard Hospital focused on educating staff, physicians and guests about the dangers of smoking and became a Tobacco Free Campus on January 1, 2020. Smoking Cessation classes were offered to patients and staff members who were interested in quitting smoking. In the Emergency Room we conduct emergency asthma care and education. While treating the patient we take time to educate the patient and caregiver of triggers, proper uses of aids and good practices to help to manage the condition. The patient is then referred to a higher level of care for follow up or to their Primary Care Physician or a hospital clinic to establish a medical home. Older Adult Health Center for Better Aging: The Center for Better Aging is a collaborative lead by St. Bernard Hospital. Supported by State of Illinois funding the program aims to provide comprehensive care for older adults, on Chicago's South Side with a health equity mandate. The clinic which opened in the hospital's ACC in July 2024, provides medical and wellness services designed for persons 50 years and older. Patients of this clinic will enjoy access to primary care, urgent care, geriatric-certified ER (GEDA), diagnostic and pharmacy services all located on the St. Bernard Hospital campus. The program will also conduct in home wellness visits and have continuous community outreach. The CBA brings together medical experts, physicians specializing in geriatric care and advanced diagnostic equipment to enhance well being through predictive, preventative, personalized and participatory care. Preventive Care Services & Access In August 2022, St. Bernard Hospital opened a Family Practice Clinic. The clinic was created to provide a place for patients, discharged from the Emergency Room to visit for follow up care, if they have no primary care physician or would like to establish a medical home. This clinic operates as a traditional family practice clinic that services patients within our community who need primary care. Patients may be referred from the Emergency Room or the Immediate Care Clinic. The number of patients seen in this clinic continues to grow. Also see Ambulatory Care Center above Interpersonal Violence The Hospital cannot directly impact interpersonal violence. But St. Bernard Hospital is a founding member of Teamwork Englewood, a community organization that focuses on safety and promotion of healthy lifestyles in the community. One of the organization's main focus it to engage young men in positive activities as an alternative to the gangs that are actively recruiting them. In addition, St. Bernard Hospital has participated several events focused on violence prevention with the 7th District of the Chicago Police Department, like National Night Out. Physical Environment The Hospital has a team that maintains the grounds of the Hospital and extends that care to adjoining properties. The Hospital does not have the resources to take this effort beyond its immediate surroundings. However, the Hospital continues to support community-based organizations that focus on the physical environment of the Englewood community.

Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsTHE ENTIRE SHORTFALL SHOULD BE TREATED AS COMMUNITY BENEFIT. CARING FOR MEDICARE PATIENTS FULFILLS A COMMUNITY NEED AND RELIEVES A GOVERNMENT BURDEN. Ratio of Cost to charge to total charges by Payer.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceOf the nearly 82,000 patients seen annually at St. Bernard Hospital, approximately 66% of them receive Medicaid as coverage for their health care services. Another 13% are on Medicare; 13% receive charity care for which no bill is generated, and 8% of our patients are self-pay patients. Our patient mix necessitates that a fundamental component of St. Bernard Hospital's community benefit strategy is the provision of charity care to those who cannot afford to pay for their care. The Hospital performs normal accepted Collection procedures on legitimate balances owed to the Hospital. Hospital Patient Access staff assist uninsured or under insured patients in applying for any governmental assistance programs they may qualify for. Uninsured patients with incomes less than 200% of the federal poverty level are eligible for a 100% charity care discount. There are no further collection activities once a patient balance has been designated as a charity write off consistent with the Financial Assistance Policy. In addition, patients with incomes between 200% and 600% of the federal poverty guidelines are eligible for an automatic 50% discount with the remaining balance discounted on a sliding scale.

Schedule H, Part VI, Line 2 Needs assessmentThe hospital periodically commissions the community needs assessment to provide information to the healthcare needs of the community. The latest community needs assessment was done in 2024. The hospital also participates in community building and community healthcare advocacy groups to help identify needs and coordinate efforts to provide quality healthcare services to the community. The hospital participates in local health fairs and operates a mobile medical van that goes to schools providing immunizations and school physicals. From its work the van identified that there was a lack of dental services for children. The hospital has opened a dental clinic geared towards pediatric patients to fill that need.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceSigns are posted in the waiting areas of the hospital as well as the hospital website announcing the availability of charity care. Hospital Patient Access staff assist uninsured or underinsured patients in applying for any governmental assistance programs they may qualify for as well as the hospital's charity care program. Hospital bills to patients also have information on applying for financial assistance.
Schedule H, Part VI, Line 4 Community informationThe urban communities surrounding the Hospital from which the Hospital draws most of its patients are some of the poorest communities on the South Side of Chicago. Within the last 30 years, they have continued to lose population and have seen housing abandonment and commercial disinvestment grow. High rates of unemployment have had a disastrous effect on the social fabric necessary to maintain a stable community. People who have choices often leave and move to neighborhoods with greater amenities, leaving behind those with fewer options. Drugs and violent crime are rampant while 35% of the population lives below the federal poverty guidelines. 66% of the patients the hospital serves are on Medicaid, 13% Medicare, while 8% are self-pay, and 13% qualify for charity care, mostly with no payment source. The area is a federally designated underserved area. The hospital serves over 750,000 patients in its primary and secondary service areas. There are no other hospitals in its primary service area, but there are three other hospitals located in its secondary service area.
Schedule H, Part VI, Line 5 Promotion of community healthSt. Bernard Hospital is a 174-bed Catholic community hospital founded in 1904 by the Religious Hospitallers of St. Joseph under the sponsorship of Catholic Health International. The Sisters of the Religious Hospitallers' enduring commitment to the hospital and the Englewood community for one hundred and twenty years epitomizes St. Bernard's mission: to care for the sick and promote the health of South Side Chicago residents, while sustaining values of respect, dignity, caring and compassion for all persons. The hospital operates 174 licensed beds including medical/surgical, intensive care, emergency medicine and inpatient mental health. The Englewood community, although challenged by the impacts of economic hardship, the community looks to St. Bernard Hospital as an institutional anchor. It is the community's single largest employer, employing nearly 800 people. Over 30% of its employees live in the Hospital's primary and secondary service area. THE MAJORITY OF IT'S BOARD OF TRUSTEE MEMBERS LIVE IN ITS PRIMARY OR SECONDARY SERVICE AREA. The Hospital maintains an open medical staff of approximately 165 physicians and credentialed staff and extends medical staff privileges to all qualified physicians in its community for all specialties. St. Bernard is a catalyst for community development, creating affordable housing for first time home buyers and the only newly constructed healthcare center in over 30 years. Beyond engaging development, itself, the hospital participates in conversations, planning meetings about development in the larger community, because it has an interest in the success of any development coming to the neighborhood. As the community's major health care facility, the hospital provides services ranging from emergency medicine to mental health and substance abuse recovery. In keeping with modern demands, St. Bernard is proud to provide state-of-the-art technological capabilities that greatly enhance the hospital's high quality of care and promote patient safety. Of the more than nearly 82,000 patients seen at St. Bernard Hospital last year, close to 79% receive Medicaid or Medicare as coverage for their health care. The hospital provided more than $8.0 million in charity care charges in 2024, or 7% of total hospital revenue. Community outreach addresses unmet health care needs of children through childhood physical and immunization service delivered by the Pediatric Mobile Health Unit, and on-site Dental Center. As a setting for education, St. Bernard is a vibrant clinical training classroom for medical and nursing students, including Emergency Medicine Residents, Surgery, Psychiatric medical students, and x-ray tech students. St. Bernard remains a constant, driving force helping to strengthen the foundations of the community by delivering high quality, comprehensive health care services, developing affordable housing, and fostering opportunities for economic growth.
