Group West Virginia University Hospitals Line Part V, Section B, Line 5The Community Health Needs Assessment CHNA was prepared by the West Virginia University Health Sciences Center. The CHNA process began with a thorough review of the previous cycles needs assessment report. The CHNA also involved collecting both primary and secondary data. Primary data was comprised of surveying community members to gather perceptions of health issues, quality of life, and other related topics. Survey data was analyzed and presented back to the West Virginia University Hospital leadership team as well as the Monongalia County Collaborative Team at a data presentation hosted virtually. Secondary data included US Census Data and County Health Ranking Data. The leadership team included Assistant VP of Population Health and Telemedicine, VP of Finance, Chief Data Management Officer, and executive members of Mon Health Medical System and Monongalia County Health Department. The CHNA can also be found here https//wvumedicine.org/wp-content/uploads/2022/11/Ruby-Memorial-Hospital-CHNA-October-2022.pdf.
Group West Virginia University Hospitals Line Part V, Section B, Line 5The public survey was collected online from residents 18 years of age and older. The survey was disseminated via a web-based platform. Collection efforts included reaching local residents via social media and with assistance from the hospitals marketing team. Both health systems used their electronic medical record messaging platforms to target patients by zip code and increase survey response. Additional information was collected through feedback at the virtual data presentation meeting. This session solicited input about community health needs and sought to identify groups and organizations already providing essential services. Survey results were presented to this group and strategies for improving health outcomes in the top areas were discussed.
Group West Virginia University Hospitals Line Part V, Section B, Line 6bWest Virginia University Hospitals, Inc. partnered with Mon Health Medical System, another Monongalia County medical system, to develop a cooperative data collection effort for the 2022 community health needs assessment.
Group West Virginia University Hospitals, Inc Line Part V, Section B, Line 11Following the data presentation meeting, leadership met to review all input together and identify priority areas to address through implementation strategies. The list of top health concerns was organized into three groups that will be the focus moving forward with implementation planning. These three health concerns are as followed Obesity and Co-Morbid Chronic Disease, Substance Use and Mental Health, and Cancer. The prioritized needs identified in the CHNA are being addressed by West Virginia University Hospitals, Inc. WVUH leadership in the following ways. With respect to obesity and related chronic disease, WVUH believes that the hospital does already provide some programming to address this need identified by the stakeholders. Hospital officials also believe that WVUH has the outreach and capacity to positively affect residents living within the service area via existing and new strategies such as farmers market sponsorship, produce prescription program, walking/running events, fitness centers and programs, sidewalk improvements, and walking/nature trail improvements. WVUH is also addressing the growing issue of substance use/abuse and mental health issues not only within the service area but in the state of West Virginia as a whole. Hospital leadership sees various opportunities to address these issues with close collaboration with Chestnut Ridge Hospital. Other strategies to improve the substance use and mental health concern include the MUSHROOM project, naloxone distribution, school-based mental health counselors, and support for homeless population. Lastly, WVUH leadership believes that they can further expand cancer prevention and treatment options to further reduce the rates of cancer in the community by providing mobile screening services, low-cost screening services, and health education seminars or videos.
Group West Virginia University Hospitals, Inc Line Part V, Section B, Line 11WVUH has various programs that address the needs identified.WVUH has offered a Medical Weight Management program since 2019. Over the years the staff has expanded to include physicians, advanced practice professionals, health coaches, pharmacists, and a broad multidisciplinary staff. Part I Line 3cWVU Hospital WVUH uses 200 Federal Poverty Guideline FPG to determine free care eligibility. However, WVUH does not offer discounted care to individuals who fail the 200 FPG test.
Part I Line 6aWVUH community benefit numbers are reported in total with the other hospitals within West Virginia United Health System. The most recent amounts can be found on WVU Medicines website here https//wvumedicine.org/about/leadership-and-more/community-benefit/.
Part I Line 7Net community benefit expense for 2024 is 386,671,828 and is 16.67 of total net expenses. To calculate net expense, bad debt of 58,184,600 was deducted from total expenses of 2,377,431,508 as shown in Part IX line 25 of the core Form 990, for a net expense of 2,319,246,908.
Part I Line 7Worksheet 2 from the IRS Schedule H instructions was used to derive the Cost-to-Charge ratio, which was used to calculate Charity Care, Unreimbursed Medicaid and other means-tested government programs at cost.
Part IIPhysical improvements and housing - Annually WVUH employees spend a weekday afternoon - on work time - volunteering at a social service agency to do repairs and maintenance for various other non-profits in Monongalia and Preston Counties.
Part IICommunity Support - WVU Medicine dedicates marketing resources to educate the public on various health issues through Tuesday Talks on social media and Health Reports, which are distributed by WDTV in Clarksburg. Both feature WVU Medicine providers and staff.
Part IICoalition building - WVUH employees participate in various community coalitions and other organizations that focus on addressing community health and safety issues.
Part IICommunity Health Improvement Advocacy - WVUH is committed to improving the health and quality of life to West Virginians in the healthcare setting and in the communities served. WVUH organizes and provides space for a weekly farmers market. WVUH does not charge a fee to vendors that wish to come to the farmers market. This famers market is attended by WVUH employees as well as employees of neighboring businesses and community members. In 2019 WVUH also launched a Pharmacy Assistance Program that is easing the burden for uninsured and underinsured patients who need lifesaving medications but not the stress of worrying about how they will afford them. WVUH has a full time employee dedicated to this program. WVUH dedicates marketing resources to provide education to the public concerning health issues through Tuesday Talks with Physicians and Health Reports provided through social media.
Part IICommunity Events - WVUH employees organized several community events throughout 2024. These included Community Day at the recently opened Waynesburg Outpatient Clinic and WVU Medicines Childrens Kids Fair.
Part IIWorkforce Development The Talent Acquisition team, a subset of the Human Resources Department at WVUH, is committed to workforce development in and around our community. Throughout 2024, the Talent Acquisition team attended and/or hosted 186 events in an effort to better train and recruit healthcare professionals. Our events included attendance at many virtual school/educational events, partnerships with the unemployment office Workforce WV, Jobs and Hope placement program, veteran events, and technical programs.
Part III Line 2Bad Debt Expense at cost was calculated by multiplying bad debt expense of 58,184,600 by our cost to charge ratio of 39.08 for a total of 22,738,542.
Part III Line 3Estimated bad debt attributable to patients eligible for charity care was calculated by running a report within our patient revenue software of all bad debt account balances greater than 50,000. The total of that report was 13,233,367 which we then multiplied by our cost to charge ratio of 39.08 for a total of 5,171,600.
Part III Line 3The estimated bad debt attributable to patients eligible for charity care of 5,171,600 should be considered community benefit due to the fact that anyone with outstanding balances of 50,000 or greater usually qualifies as catastrophic if the patient completes the application process.
Part III Line 3In our charity care policy, we define catastrophic care as any illness or injury that will likely require continuous or frequent treatment for more than one year. Regardless, that bad debt should be considered community benefit as we provide services to those in need regardless of their ability to pay.
Part III Line 4Patient accounts receivables are primarily paid by federal and state governmental authorities under the Medicare and Medicaid programs, managed health plans, commercial insurance companies, workers compensation programs, employers and patients. Patient accounts receivable are reported at net realizable value. For accounts receivable associated with services provided to patients who have third-party coverage, the System estimates net realizable value based on the estimated contractual reimbursement percentage, which in turn is based on current contract provisions and historical paid claims by payor. For self-pay accounts, including uninsured and patient responsibility accounts, the net realizable value is determined using historical collection experience, adjusted for estimated conversions of patient responsibility portions, expected recoveries and changes in trends to estimate implicit price concessions. The System does not believe there are any significant concentrations of revenues from any particular payor that would subject the System to any significant credit risks in the collection of patient accounts receivable. Management continually reviews the estimated net realizable value of accounts receivable by monitoring cash collections, economic conditions and trends, changes in payor mix, changes in federal or state healthcare coverage and other matters. Changes in general economic conditions, patient accounting service center operations, payor mix, payor claim processing could affect collections of accounts receivable, cash flows and results of operations.
Part III Line 4The System performs periodic assessments to determine if an allowance for expected credit losses is necessary. The System considers its incurred loss experience and adjusts for known and expected events and other circumstances. In estimating its expected credit losses, the System may consider changes in the length of time its receivables have been outstanding, changes in credit ratings for payors, requests from payors to alter payment terms due to financial difficulty, and notices of payor bankruptcies or payors entering receivership. Because the Systems accounts receivable is typically paid for by highly solvent, creditworthy payors, such as Medicare, Medicaid, other governmental programs, and highly regulated commercial insurers on behalf of the patient, the Systems credit losses are immaterial to the consolidated financial statements.
Part III Line 4The mix of accounts receivable at December 31, 2024 and 2023, from patients and third-party payors is as follows For the year-end December 31, 2024 Medicare - 22, Medicaid - 11, Blue Cross - 24, Commercial, managed care and other - 40, Patients - 3, totaling 100. For the year-end December 31, 2023 Medicare - 25, Medicaid - 13, Blue Cross - 21, Commercial, managed care and other - 39, Patients - 2, totaling 100.
Part III Line 8The amount reported in Part III Line 6 248,818,938 was calculated using the total Medicare allowable cost from the Medicare cost report, less Medicare reimbursement of direct GME.
Part III Line 8WVUHs shortfall, 51,531,490 of Medicare program reimbursement should be considered a community benefit because we are relieving a government burden by providing care in excess of our costs to these patients. WVUH has the only Level 1 Trauma Center in the area, and we serve an aging population that relies on WVUH to provide the most state-of-the-art care available in the area.
Part III Line 9bWVUH does have a debt collection policy. When a patient has been approved for financial assistance under our charity care policy, they will not be sent to bad debt. Additionally, if a patient is being evaluated for charity, the patient will not be sent to bad debt agency pending charity guarantor status until the pending status has been finalized approved/denied. All other patients with outstanding balances will be processed through billing and collections pursuant to our Financial Policy.
Part VI Line 2West Virginia University Hospitals, Inc. WVUH considers several components in assessing how the organization determines the need of the community it serves. Data collection was structured so that the leadership team could analyze three different areas of West Virginia. The survey asked participants to select which county they resided in with options of Monongalia County, Marion County, or Somewhere Else.
