DescMilford Regional Medical Center serves the healthcare needs of the residents of 20-plus communities in Central Massachusetts, a population now approaching 250,000. Guided by its mission to provide exceptional healthcare services to the community with dignity, compassion and respect, MRMC strives to be responsive to diverse constituencies - individuals and families of all ages, employees and employers of large and small businesses, physicians and other healthcare providers, our school systems, our local and state officials and safety personnel, churches, community organizations, our insurers and payers and our own employees and volunteers. As a non-profit hospital, we acknowledge the needs of the underserved, especially, low-income and minority populations. A full-service community and regional teaching hospital, Milford Regional is a 145-bed acute care facility. There 300 primary care and specialty physicians on the medical staff, our physicians are skilled in the most advanced procedures and technology, and provide personalized patient care in a warm and caring environment. MRMC is an affiliate of UMASS Memorial as a teaching hospital. It is also affiliated with Children's Hospital, Dana Farber Cancer Institute, Brigham and Women's Hospital, and Brigham and Women's Physician Organization. In addition to providing basic acute care services (medical, surgical, emergency medicine, family-centered obstetrics and pediatrics)MRMC has eight state-of-the-art operating suites, consolidated surgical services (including admitting and pre-admission testing), and a medical/surgical floor with advanced patient monitoring capabilities. Milford Regional recorded 13,214 inpatient discharges/observation stays and 5,910 day surgeries in FY 2016. In FY 2016, Milford Regional introduced the Elder Wellness Program, which is designed to prevent a decline in mental and physical abilities often seen in older adults during hospitalization. Milford Regional earned the designation of 2016 Center of Excellence in Neonatal Abstinence Syndrome Care. In response to the increased opioid use in pregnant women resulting in a rise in the number of newborns developing severe signs of opioid withdrawal, Milford Regional became one of 110 medical centers around the country to enroll in a national quality improvement collaborative dedicated to improving the quality, safety and value of care for substance-exposed infants and their families. As a result, the Milford Regional team implemented Baby Steps, a new program to help prepare families to care for their newborn that has been exposed to medications, drugs or alcohol. Another program introduced in FY 2016 that provides support for moms with their newborns was The Snuggle Squad, a group of highly trained volunteers who comfort and soothe newborns in Milford Regional's Maternity Center. Their role is particularly important in assisting babies with Neonatal Abstinence Syndrome, and new mothers recovering from a difficult birth. Milford Regional has also invested in many upgrades to provide the most up-to-date diagnostic technologies by installing the region's very first 3T MRI, as well as upgrades in digital radiology, low dose CT scanners and interventional radiology equipment. Milford Regional has earned awards in the past year, including a patient safety grade of A from the Leapfrog Group and the Stroke Gold Plus Quality Achievement Award from the American Heart Association/American Stroke Association. The awards also extend to our physicians. Four Tri-County Medical Associates (TCMA) primary care sites were recognized this year by National Committee for Quality Assurance (NCQA) as a Patient Centered Medical Homes; two practices received local recognition with Community Newspaper Company's Readers' Choice Gold Awards; and TCMA was named to Harvard Pilgrim's Honor Roll in 2016. In addition, TCMA was rated highly by patients in the Massachusetts Health Quality Partners (MHQP) annual patient experience survey.Desc

Schedule H, Part V, Section B, Line 5-Milford Regional Medical Center IncMilford Regional Medical Center completed our Community Health Needs Assessment (CHNA) in September 2015. The process was led by the Community Benefits Steering Committee with support from HRiA and under the leadership of then-CEO Frank Saba. The CHNA included quantitative data analysis, a community survey with over 1,000 respondents and interviews with key stakeholders. Interviewees represented health care, public safety, government, education and the clergy. A Strategic Implementation Plan for FY 2014-2016 developed by the Steering Committee was reviewed and evaluated, addressing goals and objectives of the priority areas: behavioral health, healthy weight for youth, access to care, and chronic disease prevention. With community partnerships, support from hospital departments, and grant funding through the Executive Office of Health and Human Services and Metrowest Health Foundation, Milford Regional has addressed many of these issues in a substantive way. Milford Regional plays a leadership role in convening groups to improve health outcomes and works closely with the other agencies and institutions including Metrowest Health Foundation, MA Department of Public Health, Health Care for All, MA Executive Office of Health and Human Services, local elected officials, and 70 partner organizations within CHNA 6. Given the scope of the issues identified in the assessment, this work is ongoing.
Schedule H, Part V, Section B, Line 11-Milford Regional Medical Center IncMilford Regional Medical Center Inc. - The Community Needs Assessment was presented to community leaders, CEOs, school superintendents and boards of health directors at a leadership roundtable hosted by Milford Regional. The focus was on access to care with a panel representing primary care, behavioral health and specialty care. Milford Regional continues to work with community partners to focus on the four priority areas of behavioral health, chronic disease management, access to care and healthy weight for youth. Working as part of CHNA 6, grants have been awarded for work being done in those priority areas. The hospital continues to work internally, as well, with nutrition, diabetes, maternity, volunteer services and care management departments to address these priorities.
Schedule H, Part V, Section B, Line 13h-Milford Regional Medical Center IncAsset level is used for Medicare patients only. This follows Medicare guidelines.
Schedule H, Part V, Section B, Line 22d-Milford Regional Medical Center IncThe Hospital used the Amount Generally Billed (AGB) method to determine the amount that can be charged to FAP-eligible individuals for emergency or other medical necessary care. The AGB is calculated using twelve months of Medicare and Commercial claims paid data and by dividing the total payments received from all parties by the covered charges.
Schedule H, Part V, Section B, Line 24-Milford Regional Medical Center IncAt the Hospital, all patients are charged the gross charges for any services. Upon contact from an uninsured patient, a discussion occurs which may result in the discounting of gross charges.

Schedule H, Part I, Line 7Line 7 a/b/c were calculated using worksheets 1,2,3 provided in the instructions.
Schedule H, Part I, Line 7, Column fLine 7F was determined by the cost and reimbursement information on the Medicare Cost Report. It was calculated by dividing Column E, Net Community Benefit Expense by total of Part IX Line 25 of Form 990 Core Form of $
Schedule H, Part III, Section A, Line 2The Hospital's bad debt expense was estimated as the product of the Hospital's bad debt multiplied by the Hospital's overall cost-to-charge ratio calculated on Worksheet 2 of the instructions.
Schedule H, Part III, Section A, Line 3Bad debt expense attributable to patients eligibility for financial assistance was calculated as the accounts for which patients were unemployed to the time of service multiplied by the Hospital's overall cost-to-charge ratio.
Schedule H, Part III, Section A, Line 4In Line 2, Worksheet A in the instructions was used by taking bad debt as reported in the Hospital's audited financial statements multiplied by the cost-to-change ratio calculated in Worksheet 2 in the instructions.
Schedule H, Part III, Section B, Line 8The Hospital's Medicare Cost Report was used to determine the amount reported in line 6.
Schedule H, Part III, Section C, Line 9bOnce it has been determined that a patient qualifies for full or partial free care, all collection procedures cease and the full balance, or amount that qualifies under partial free care, is written off. Massachusetts hospitals are required to adopt a credit and collections policy under the Health Safety Net (HSN) regulations (114.6 CMR 13.08 (1) (c)) that outlines the specific process for determining eligibility for public assistance programs and hospital collection practices.
Schedule H, Part VI, Line 2Milford Regional stays current with studies by other agencies, particularly the Metrowest Adolescent Health Survey conducted by the MetroWest Health Foundation and state and local data. The Office of Community Benefits is actively engaged with the Community Health Network Are (CHNA) and other community coalitions/committees focusing on substance abuse, access to services, interpersonal violence prevention, cancer prevention and nutrition/physical activity/wellness to address emerging health needs.
Schedule H, Part VI, Line 3Patients are informed of the availability of financial assistance by way of payment statements printed with financial assistance information. At the time of discharge from the Emergency department, uninsured patients are encouraged to meet with an on-site Financial Counselor. If unable to meet with a Financial Counselor at the time of discharge, the patients are given a screening letter that gives contact information for our financial assistance department. The day following any emergency room visit, a form is generated on each uninsured patient and sent to the Financial Counselors. At that time, the Financial Counselors contact the patient to coordinate a meeting to apply for assistance, Scheduled self-pay Surgical Day Care (SDC) and inpatients are reviewed prior to admission to determine payment arrangements or to coordinate a meeting with a Financial Counselor. Insured SDC and inpatients who are unable to leave a deposit or make payment arrangements toward a large out of pocket expense are also encouraged to meet with a Financial Counselor.
