MissionThe mission of the Corporation is to nurture the healing ministry of the Church, supported by education and research. Fidelity to the Gospel urges the Corporation to emphasize human dignity and social justice as it creates healthier communities. The Corporation, sponsored by a lay-religious partnership, calls other Catholic sponsors and systems to unite to ensure the future of Catholic health care. To fulfill this mission, the Corporation, as a values-based organization, will assure the integrity of the ministry in both current and developing organizations and activities; research and develop new ministries that integrate health, education, pastoral, and social services; promote leadership development and formation for ministry throughout the entire organization; advocate for systemic changes with specific concern for persons who are poor, alienated, and underserved; and steward resources by general oversight of the entire organization.MissionDesc

Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - Lakewood Health Center. A community group consisting of nearly 60 community members was convened and met in a single session on September 24, 2015. The session was held at 5:00 p.m. at First Lutheran Church, Baudette, MN and a complimentary evening meal was served to all in attendance. Demographic information of individuals attending was collected in a private manner and separated from the registration area. Members of the community group represented the broad interests of the community served by CHI Lake Wood Health. Invitations were sent to organizations representing the school district, county social services, occupational and development office, ministerium, city and county government, local business owners and health care workers/leaders and the public health director. The meeting was announced on the local radio station and posted in the local newspapers. The session was facilitated by Ms. Horntvedt, Extension Educator, University of Minnesota. An overview of the past community health needs assessment was provided along with the objectives for the evening. The forum objectives were to: 1) Collect quantitative and qualitative information for the Lake of the Woods Community Health Needs Assessment and 2) Demonstrate proactive healthy community interaction and identification of community needs. The assessment included areas of health, safety-violence prevention, nutrition, physical activity, and quality of life. Ten tables were set up with 4 - 6 participants at each table. Each table was labeled with a general topic with worksheets listing the problems and concerns generated at the 2012 forum. Space was also provided on each worksheet to add new items. Two rounds were held allowing participants to choose two areas of interest or expertise. A leader was assigned at each table to record participant comments and the level of importance for each item. Each participant voted high, moderate or low on the importance and this could be done verbally or by hand signal to give everyone the opportunity for input and full participation. At the conclusion of each round, each table selected five priorities and ranked them 1 to 5. (1 most important). Each table reported out their findings by writing the problem or concern on a letter size sheet of paper and these were posted on the wall under the various broad categories for the whole group to view. The facilitator provided a time of conversation, observations and concerns that have a common thread. All of the individual notes were collected to review any comments and note any listing of resources needed or resources that are currently in place. This process was not intended to be scientific or statistically valid. It was designed to collect qualitative data and quantitative data form the community - specifically related to community perceived health needs and what resources are needed.

Schedule H, Part V, Section B, Line 13 Facility , 1Facility , 1 - LakeWood Health Center. When Catholic Health Initiatives (the ultimate parent organization to LakeWood Health Center) established its financial assistance policy it was determined that establishing a household income scale based on the HUD very low income guidelines more accurately reflects the socioeconomic dispersions among urban and rural communities in 18 states served by CHI hospitals and health care facilities. In comparing HUD guidelines to the Federal Poverty Guidelines (FPG), we find that on average HUD guidelines compute to approximately 200% to 250% (and sometimes 300%) of FPG. LakeWood Health Center bases its financial assistance eligibility on HUD's 130% of Very Low Income Guidelines based on geography, and affords the uninsured and underinsured the ability to obtain financial assistance write-offs, based on a sliding scale, ranging from 25%-100% of charges.
Schedule H, Part V, Section B, Line 22 Facility , 1Facility , 1 - Lakewood Health Center. The organization will not charge individuals who are eligible for financial assistance more than amounts generally billed to those who have insurance. Lakewood Health Center USES ITS BEST CONTRACTUAL AGREEMENT TO ESTABLISH THE SELF-PAY DISCOUNT FOR THOSE THAT ARE UNINSURED. THIS IS APPLIED TO THE TOTAL CHARGES AND THE REMAINDER IS BILLED TO THE PATIENT.

Schedule H, Part VI Lines 2, 4, & 5 and Part II: Needs Assessment and Community InformationIntroduction: I. The mission of CHI LakeWood Health and Catholic Health Initiatives is to nurture the healing ministry of the Church, supported by education and research. Fidelity to the Gospel urges us to emphasize human dignity and social justice as we create healthier communities. The vision of CHI LakeWood Health and Catholic Health Initiatives' is to live up to our name as one CHI: Catholic: Living our Mission and Core Values Health: Improving the health of the people and communities we serve Initiatives: Pioneering models and systems of care to enhance care delivery CHI LakeWood Health in Baudette, Minnesota, is a Catholic, not-for-profit care organization, and is sponsored by Catholic Health Initiatives, one of the largest Catholic, not-for-profit health care systems in America. CHI LakeWood Health is dedicated to the health and well-being of the community and to providing quality, holistic health care services and facilities for the residents of Baudette, Lake of the Woods County and surrounding communities of the Lake of the Woods Region. CHI LakeWood Health is located in a rural and isolated area of Northern Minnesota serving Lake of the Woods County and surrounding counties of Roseau, Koochiching, Beltrami and on occasion residents of Canada. CHI LakeWood Health is federally designated, 15-bed Critical Access Hospital. CHI LakeWood Health offers a continuum of care including long term care, assisted residential community (board and lodging with services) hospital care and services to include clinic office visits. CHI LakeWood Health closed OB services in September 2009. CHI LakeWood Health had a total of six different medical providers during this fiscal year; three family practice physicians (two MDs and one DO) two certified nurse practitioners and one part time physician's assistant. One nurse practitioner resigned her full time position in February 2016 and moved to PRN; the other nurse practitioner resigned in April 2016. LakeWood welcomed back a medical doctor who had worked in the clinic and health center, approximately three years previous. CHI LakeWood continues to utilize locum providers for the emergency room and some clinic coverage. LakeWood Nursing Service, which included home care, hospice, public health and school nursing, formed a new division within Catholic Health Initiatives and these services transferred out of LakeWood Health Center operations in December of 2010. Lakewood Nursing Service was closed in 2012, discontinuing hospice and home care services. Public health and school nursing returned to CHI LakeWood Health campus under the name of LakeWood Public Health. A new program was developed through the rural health clinic and approved in August 2012. (Rural Health Clinic Visiting Nurse Program). CHI LakeWood Health serves all persons in the community on a nondiscriminatory basis and participates in Medicaid, Medicare programs and has an active charity care program. The emergency room is available 24 hours a day, 365 days per year, and is open to all persons regardless of ability to pay. CHI LakeWood Health Center has a governing body (Board of Directors) of independent persons representative of the community comprising the majority along with three board members from within the organization. History CHI LakeWood Health opened in March 1950 as Baudette Community Hospital. It was owned by the community and operated by the Sisters of St. Joseph out of Crookston, Minnesota. The hospital was leased to the Sisters of St. Joseph in 1954 and in 1957 the Sisters purchased the hospital and changed the name to "Trinity Hospital". In 1982, the Sisters entered into a management contract with United Health Resources of Grand Forks, North Dakota until they sold the hospital to Franciscan Sisters Health Care, Incorporated in 1985. During this time, Trinity Hospital took over management of the Pioneer Nursing Home, the long-term term care facility located one mile east of the hospital. In 1986, the hospital acquired the county-owned public health service. Trinity Hospital bought the Pioneer Nursing Home in 1990 and the name was changed to "LakeWood Health Center" to reflect the coming together of acute care, long-term care (LakeWood Care Center), and home health/public health (LakeWood Nursing Service) under one umbrella. Catholic Health Corporation of Omaha, Nebraska took over sponsorship of LakeWood Health Center in 1993. Two years later they merged with two other large Catholic health care systems to form Catholic Health Initiatives (CHI), our current sponsor, based in Englewood, Colorado. LakeWood Health Center completed a building and renovation project in 2000 that resulted in all health care services being available on one campus. LakeWood Nursing Service transferred to Catholic Health Initiatives Health Connect At Home. The operation moved off site in June 2010 and the financial operations were separated in December of 2010. Catholic Health Initiatives At Home discontinued home health services in Lake of the Woods County and public health, school nursing, Parker's Arc transferred back to LakeWood Health Center by July of 2012. CHI LakeWood Health is the only hospital, clinic and licensed long term care provider in Lake of the Woods County. CHI LakeWood Health is committed to the promotion of healthier communities. CHI LakeWood Health is proud of its Catholic heritage and its ability to provide direct and indirect support to the communities it serves. CHI LakeWood Health is included in the Official Catholic Directory as a tax-exempt hospital.
Schedule H, Part VI Lines 2, 4, & 5 and Part II: Needs Assessment and Community InformationCOMMUNITY BENEFIT APPROACH CHI LakeWood Health is located in Baudette, Minnesota, a rural and isolated area of Northern Minnesota, serving Lake of the Woods County and surrounding counties of Roseau, Koochiching, Beltrami and on occasion residents of Canada. According to the 2014-2015 Census Data Bureau, the City of Baudette population is 1,059 and Lake of the Woods County census is 3,923; a 3% decrease since 2010 and a decline of 10.5% in population in the county from year 2000-2010. The decline in population is thought to be related to the economic downturn and loss of young families from the area. Persons 65 years and over make up 22.7 % of Lake of the Woods County population in 2015. The median age is estimated at 48.9 years. The median household income (2010 - 2014) is $45,119 and 15.9 4% of persons in Lake of the Woods County are below poverty level according to data for 2007-2011. The annual average unemployment rate for Lake of the Woods County was 5.6 (2013), 4.7 (2014) and 3.9 (2015). There are 3.0 persons per square mile in Lake of the Woods County. LakeWood Health is one of the largest employers in the county with an economic impact of about 18.3 million dollars according to a study done in 2005 by Minnesota Rural Health Resource Center. Combining all health care services in the county, the economic impact is 29.025 million dollars according to a study done in 2009 by Minnesota Department of Health - Office of Rural Health and Primary Care. In September of 2015 another community forum was held as part of the CHNA. The top four priorities established are: 1) Elderly Services to include community support and future consideration of housing for the elderly; 2) Mental Health Services; 3) Alcohol and Drug Abuse/Dependency; 4) Nutrition, Physical Activity and Obesity (Cardiovascular Wellness). Community forums are held with the community every three years to review progress on community goals, assess current needs and update action plans. CHI LakeWood Health has been involved in community health collaboration and has a history of sponsoring community forums to look at area health issues, set priorities and plans for improvement. CHI LakeWood Health continues a deep commitment and involvement in the community health activities. CHI LakeWood Health strives to have community benefit a visible part of who we are and provides an annual report for our community in one of our newsletters published 4 times per year. Collaborating and cooperating with our community is "a way of life" and part of the culture for CHI LakeWood Health and its employees. We are thankful to have the resources of time, talent and other gifts to share for the good of our community.
Schedule H, Part VI Lines 2, 4, & 5 and Part II: Needs Assessment and Community InformationQUALITATIVE DESCRIPTION OF COMMUNITY BENEFIT Community Outreach for the Poor LakeWood Health Center provides traditional charity care, an internal program for the medically needy that assists patients, residents or clients financially by forgiving or reducing their medical debt at CHI LakeWood Health. The billing/insurance office or the social worker identifies those individuals or families without insurance coverage or those that are underinsured and provide application assistance for this program. Total expense for this program is $49,684 with 66 persons served. Unpaid cost of Medicaid is $1,239,641 with 1,736 persons served for fiscal year ending June 30, 2016. Community Outreach for the Broader Community * LakeWood Health sponsors and provides support for development and maintaining an Alzheimer's support group in Lake of the Woods County and surrounding area. A need was identified during the 2009 community assessment and through LakeWood Care Center Family Council members. Volunteers have come forward to lead the support group and monthly meetings have been established. The meetings are held at LakeWood and refreshments are provided by LakeWood dietary department. An Alzheimer's Association Support Group is a safe place to learn, offer and receive helpful tips, and meet others coping with Alzheimer's disease or another dementia. The atmosphere of an Alzheimer's Association Support Group is one of sharing and caring friendship. The environment is a confidential and non-judgmental place to share ideas, frustrations, anger and joy. Members receive positive reinforcement. LakeWood Health collaborated with Lake of the Woods County Social Services to sponsor a community grief support class/group. LakeWood provided space, materials, and refreshments for the sessions. The county provided the trained facilitator for the six to eight week sessions. Plan to repeat the grief support class/group 1 - 2 times per year. * LakeWood partnered with the Lake of the Woods School to screen all fall, winter and spring sports athletes. The program also extended to the community's youth hockey program. The screening involves a 10-minute baseline test on each athlete looking for any physical or cognitive signs of previous concussion. The athlete's normal physical and cognitive abilities are documented, shared with the coach and kept with the student's medical record. Minnesota State High School League adopted new rules (2011) about what steps should be taken before an athlete with a head injury can return to play or practice and this program is assisting the athletic programs to maintain compliance and safety for young athletes. * CHI LakeWood Health in conjunction with the clinic developed the Diabetes Resource Center to bring awareness of the chronic disease to the public and provide education for people with diabetes to help them understand and manage their disease. The program is offered in class sessions and encourages self-management and long-term health maintenance. CHI LakeWood Health provides a dietitian consult as needed for the program. The Diabetes Resource Center Staff is available to meet with individuals to assist them with managing their diabetes. The Diabetic Resource Center is staffed out of the Cardio-Pulmonary Rehab Department. The Diabetes Resource Center offered "I CAN Prevent Diabetes!--Individuals and Communities Acting Now to Prevent Diabetes" a 16 week program (classes started November 2009 and continue) offered to those who meet screening criteria. The "I CAN Prevent Diabetes!" program has been adapted from the Diabetes Prevention Program (DPP). The DPP is an evidenced-based lifestyle change program focused on diet and exercise. The original DPP showed a 60% reduction in risk for diabetes when participants made lifestyle changes related to food choices and exercise. A community support group was established for those affected by diabetes and this group is meeting at CHI LakeWood Health three to four times a year, sponsored by CHI LakeWood Health. * CHI LakeWood Health partnered with LakeWood Regional Healthcare Foundation and local farmer's markets. Utilizing the Diabetic Resource Program and Cardiac Rehab staff to identify participants who could benefit from improved access to fruits and vegetables. Screening was provided before and after the program. Vouchers were provided for the participant (the number of vouchers provided included the family members) to purchase fresh fruit and vegetables at local farmer's market. It is recognized that the household needs to make changes for the patient to be successful. This is the second year this program was offered. * CHI LakeWood Health held the annual health fair in July 2015 at the Lake of the Woods County Fair. A three day event was held (Thursday, Friday, Saturday) providing glucose, cholesterol and blood pressure screening. About 150 screened. Nurses provided health education during the time of screening. LakeWood also participated in screening events for the Marvin's Health Fair in October 2015 (2 days). LakeWood also participated in a wellness and screening event at NorthStar Electric Cooperative, Lake of the Woods School District, Lake of the Woods County and a community Frost Fest event. Approximately 350 - 375 people were screened totaling all events.

Lake of the Woods County Prevention Coalition (Alcohol, Tobacco and other Drugs): CHI LakeWood Health Center (mission leader, public health staff) are members of the coalition and CHI LakeWood Health provides space for meetings and occasional refreshments. Lake of the Woods County works with the Department of Human Services/Alcohol and Drug Abuse Division by focusing in part - changing the environment the youth live in. Investing in prevention measures will not only enhance the community it will enhance the lives of our youth. The prevention measures are aimed at: 1) Reducing access and availability 2) Enforcement of existing laws and policies and 3) Changing public attitudes and social norms. 5. Suicide Awareness and Prevention Coalition: CHI LakeWood Health is participating in this coalition partnering with local county social services, school social work and administration, youth group leaders, clergy, public health. A media and education campaign was developed to include access to help and life lines. This group has partnered with the school and assisted with grant writing to provide educational guest speakers for students and the public on topics of bullying and self-esteem. 6. The SHIP process has engaged a team of interested community members representing the school, county, city, and CHI LakeWood Health. The group meets to discuss activities that have been shown to prevent chronic disease by increasing access to healthy foods, decreasing tobacco use, and increasing active living opportunities. They work on policy and system changes that can be adopted with our school, communities, places of work and healthcare facilities. Activities include the coordination and prep of community gardens (land, tilling and hauling of garden debris - donated by CHI LakeWood Health), assessment of student health policies at Lake of the Woods School and strengthening the availability of healthy foods at school. Community gardening has been a successful and sustainable project. CHI LakeWood Health provides the garden spaces and prep work each season. 7. Community Health Needs Assessment and Implementation Strategies The purpose of conducting a community health needs assessment is to describe the health of local people, identify use of local health care services, prioritize community needs and identify action needed to address the future delivery of health care in the defined area. A health needs assessment benefits the community by: 1) collecting timely input from the local community, providers and staff; 2) providing an analysis of secondary data related to health conditions, risks and outcomes; 3) compiling and organizing information to guide decision making, education, and marketing efforts, and to facilitate the development of a strategic plan; 4) engaging community members about the future of health care community by: delivery; and 5) allowing the charitable hospital to meet federal regulation requirements of the Affordable Care Act, which required not-for-profit hospital to complete a community health needs assessment at least every three years. LakeWood Health Center completed a health needs assessment and published the findings on the facility website. The Community Health Needs Assessment was completed during September 2015-June 2016. Community Health Improvement Advocacy and Support Services 1. CHI LakeWood Health offers personalized, confidential assistance at no charge to Medicare beneficiaries who wish to enroll or update participation in the Medicare Part D Prescription Drug Program. The process is complicated and internet access has proven to be a barrier for people, especially the elderly. LakeWood has a dedicated high-speed internet connection and private consulting space. Designated staff members at CHI LakeWood Health have received training and will schedule appointments to assist with this enrollment process. 2. Minnesota Health Care Programs outreach efforts enhanced through Minnesota Community Application Agent (MNCAA) program. Level I organizations are referred to as a Minnesota Community Application Agent (MNCAA). MNCAA contracts with DHS to serve as an application site for those needing assistance with the Minnesota Health Care Programs Application (HCAPP). Employees from LakeWood participated in this training. Level I sites are contracted to: a) Identify the potential health care enrollees b) Assist potentially eligible enrollees with the HCAPP c) Assist applicants with obtaining documentation needed to determine health care eligibility d) Offer use of fax and copy services e) Follow up as needed until an eligibility determination is reached
Schedule H, Part I, Line 3c Eligibility criteria for free or discounted careWhen Catholic Health Initiatives (the ultimate parent organization to LakeWood Health Center) established its financial assistance policy it was determined that establishing a household income scale based on the HUD very low income guidelines more accurately reflects the socioeconomic dispersions among urban and rural communities in 18 states served by CHI hospitals and health care facilities. In comparing HUD guidelines to the Federal Poverty Guidelines ("FPG"), we find that on average HUD guidelines compute to approximately 200% to 250% (and sometimes 300%) of FPG. An individual's income under the HUD guidelines is a significant factor in determining eligibility for financial assistance. However, in determining whether to extend discounted or free care to a patient, the patient's assets may also be taken into consideration. For example, a patient suffering a catastrophic illness may have a reasonable level of income, but a low level of liquid assets such that the payment of medical bills would be seriously detrimental to the patient's basic financial (and ultimately physical) well-being and survival. Such a patient may be extended discounted or free care based upon the facts and circumstances.
Schedule H, Part I, Line 7g Subsidized Health ServicesThere are no physician clinics included in subsidized health services.
Schedule H, Part I, Line 7 Bad Debt Expense excluded from financial assistance calculation241664
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceThe approach taken for the Community Benefit Calculation was as follows: - An approach of including assigned indirect overhead based on medicare cost report methodology was utilized. - Medicaid Community Benefit was calculated based upon the Medicaid estimated inpatient, outpatient, and long term care cost to charge ratios times the charges for inpatient, outpatient, and long term care Medicaid patients. Additionally, Minnesota Care Tax and MA Surcharge Taxes totaling $271,217 were included in the costs for Medicaid. This was then compared to projected reimbursement for Medicaid Services. This was then compared to projected reimbursement for Medicaid services. The total community benefit is estimated at $1,239,641. - The remaining community benefit was taken by compiling the cost of traditional charity care $49,684; Community Health Improvement Services $57,207; subsidized health services $222,394; Financial and In-Kind Contributions $25,579; Community Building Activities $6,613; and Community Benefit Operations $7,129. - The Cost to Charge Ratio for the year ended 6/30/2016 was computed using the following formula: Medicare Allowable Costs divided by revenues for each service line type inpatient, outpatient, and long term care. Subsidized health services was calculated using Estimated Net Revenue as compared to direct costs and overhead allocated. Over allocated follows the Medicare Cost Report assignment of allocating overhead to cost centers. The Medicare Cost Report is not yet prepared at time of calculation of the Community Benefit for financial statement purposes. As a result, we utilize an internal calculation model to project this benefit in July. The community benefit from subsidized health services below was $222,394. Worksheet 2 was not used to derive the cost-to-charge ratio.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountThe amount reported as bad debt expense represents total charges written off to bad debt. When discounts are extended to self-pay patients, these patient account discounts are recorded as a reduction in revenue, not as bad debt expense.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyLakeWood Health Center does not believe that any portion of bad debt expense could reasonably be attributed to patients who qualify for financial assistance since amounts due from those individuals' accounts will be reclassified from bad debt expense to charity care within 30 days following the date that the patient is determined to qualify for charity care.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteLakeWood Health Center does not issue separate company audited financial statements. However, the organization is included in the consolidated financial statements of Catholic Health Initiatives. The consolidated footnote reads as follows: "The provision for bad debts is based upon management's assessment of historical and expected net collections, taking into consideration historical business and economic conditions, trends in health care coverage, and other collection indicators. Management routinely assesses the adequacy of the allowances for uncollectible accounts based upon historical write-off experience by payor category. The results of these reviews are used to modify, as necessary, the provision for bad debts and to establish appropriate allowances for uncollectible net patient accounts receivable. After satisfaction of amounts due from insurance, CHI follows established guidelines for placing certain patient balances with collection agencies, subject to the terms of certain restrictions on collection efforts as determined by each facility. The provision for bad debts is presented on the consolidated statement of operations as a deduction from patient services revenues (net of contractual allowances and discounts) since CHI accepts and treats all patients without regard to the ability to pay."
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsLakeWood Health Center is designated as a Critical Access Hospital ("CAH"). CAHs are rural community hospitals that are certified to receive cost-based reimbursement from Medicare. The reimbursement that CAHs receive is intended to improve their financial performance and thereby reduce hospital closures. CAHs are certified under a different set of Medicare Conditions of Participation (CoP). Shortfalls are created when a facility receives payments that are less than the costs of caring for program beneficiaries. Because shortfalls are based on costs, not charges, LakeWood HealthCenter, due to designation as a CAH, received cost-based reimbursement for Medicare purposes, LakeWood Health Center will not experience Medicare related shortfalls. LakeWood Health Center believes that excluding Medicare losses from community benefit makes the overall community benefit report more credible for these reasons: Unlike subsidized areas such as burn units or behavioral-health services, Medicare is not a differentiating feature of tax-exempt health care organizations. In fact, for-profit hospitals focus on attracting patients with Medicare coverage, especially in the case of well-paid services that include cardiac and orthopedics. Significant effort and resources are devoted to ensuring that hospitals are reimbursed appropriately by the Medicare program. The Medicare Payment Advisory Commission (MedPAC), an independent Congressional agency, carefully studies Medicare payment and the access to care that Medicare beneficiaries receive. The commission recommends payment adjustments to Congress accordingly. Though Medicare losses are not included by Catholic hospitals as community benefit, the Catholic Health Association guidelines allow hospitals to count as community benefit some programs that specifically serve the Medicare population. For instance, if hospitals operate programs for patients with Medicare benefits that respond to identified community needs, generate losses for the hospital, and meet other criteria, these programs can be included in the CHA framework in Category C as "subsidized health services." Medicare losses are different from Medicaid losses, which are counted in the CHA community benefit framework, because Medicaid reimbursements generally do not receive the level of attention paid to Medicare reimbursement. Medicaid payment is largely driven by what states can afford to pay, and is typically substantially less than what Medicare pays.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceLakeWood Health Center's debt collection policy provides that LakeWood Health Center will perform a reasonable review of each inpatient account prior to turning an account over to a third-party collection agent and prior to instituting any legal action for non-payment, to assure that the patient and patient guarantor are not eligible for any assistance program (e.g. Medicaid) and do not qualify for coverage through LakeWood Health Center's community assistance policy. After having been turned over to a third-party collection agent, any patient account that is subsequently determined to meet LakeWood Health Center's community assistance policy is required to be returned immediately by the third-party collection agent to LakeWood Health Center for appropriate follow-up. LakeWood Health Center requires its third-party collection agents to include a message on all statements indicating that if a patient or patient guarantor meets certain stipulated income requirements, the patient or patient guarantor may be eligible for assistance. All of Catholic Health Initiatives' hospitals' contracts with third party collection agencies include the following standards: * Neither CHI hospitals nor their collection agencies will request bench or arrest warrants as a result of non-payment; * Neither CHI hospitals nor their collection agencies will seek liens that would require the sale or foreclosure of a primary residence; and * No Catholic Health Initiatives' collection agency may seek court action without hospital approval. Finally, collection agencies are trained on the Catholic Health Initiatives Mission, Core Values and Standard of Conduct to make sure all patients are treated with dignity and respect.

Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceLakeWood Health Center, along with its affiliated outpatient facilities are part of Catholic Health Initiatives "CHI". CHI has a written policy (Stewardship policy No. 15) governing the procedures for determining and informing patients about the entity's financial assistance policy. All hospital facilities included in the filing organization have adopted the policy. The policy states the organization will include information concerning its financial assistance policy on its website. In addition, LakeWood Health Center prominently displays its financial assistance policy in English in obvious locations throughout the hospitals, including the emergency rooms and other patient intake areas, as well as in LakeWood Health Center's outpatient facilities. In addition, LakeWood Health Center registration clerks are trained to provide consultation to those who have no insurance or potentially inadequate insurance concerning their financial options including application for Medicaid and for financial assistance under LakeWood Health Center's financial assistance policy. Upon registration (and once all EMTALA requirements are met), patients who are identified as uninsured (and not covered by Medicare or Medicaid) are provided with a packet of information that addresses the financial assistance policy and procedures including an application for assistance. LakeWood Health Center registration clerks read the organization's medical assistance policy to those who appear to be incapable of reading, and provide translators for non English-speaking individuals. LakeWood Health Center's staff will also assist the patient/guarantor with applying for other available coverage (such as Medicaid), if necessary. Counselors assist Medicare eligible patients in enrollment by providing referrals to the appropriate government agencies.
Schedule H, Part VI, Line 6 Affiliated health care systemLakeWood Health Center, along with its affiliated outpatient facilities, are part of Catholic Health Initiatives. Catholic Health Initiatives (CHI) is a national faith-based nonprofit health care organization with headquarters in Englewood, Colorado. CHI's exempt purpose is to serve as an integral part of its national system of hospitals and other charitable entities, which are described as market-based organizations, or MBOs. An MBO is a direct provider of care or services within a defined market area that may be an integrated health system and/or a stand-alone hospital or other facility or service provider. CHI serves as the parent corporation of its MBOs which are comprised of 104 hospitals, including four academic medical centers, and 30 critical access facilities; community health service organizations; accredited nursing colleges; home health agencies; and other facilities that span the inpatient and outpatient continuum of care. Together, these facilities provided $1.1 billion in financial assistance and community benefit in the 2016 fiscal year, including services for the poor, free clinics, education and research. CHI provides strategic planning and management services as well as centralized "shared services" for the MBOs. The provision of centralized management and shared services - including areas such as accounting, human resources, payroll and supply chain -- provides economies of scale and purchasing power to the MBOs. The cost savings achieved through CHI's centralization enable MBOs to dedicate additional resources to high-quality health care and community outreach services to the most vulnerable members of our society. LakeWood Health Center operates with its wholly owned affiliates and community partners to serve the health care needs of Lake of the Woods County, Minnesota and surrounding communities.

Form 990, Part VI, Line 14 WRITTEN DOCUMENT RETENTION & DESTRUCTION POLICYTHE ORGANIZATION'S DOCUMENT RETENTION AND DESTRUCTION POLICY HAS NOT YET BEEN FORMALLY ADOPTED BY THE BOARD. HOWEVER, THE ORGANIZATION HAS A WRITTEN DOCUMENT RETENTION AND DESTRUCTION POLICY ADOPTED BY THE BOARD-APPOINTED FINANCE COMMITTEE.
Form 990, Part VI, Line 15a PROCESS FOR DETERMINING COMPENSATION FOR TOP MANAGEMENT OFFICIALThe organization's top management official's compensation is paid by Catholic Health Initiatives (CHI), a related organization. CHI has a defined compensation philosophy. Both the executive and non-executive compensation structures and ranges are reviewed annually in comparison to market data. CHI uses The Korn Ferry Hay Group as the independent third party to assess executive compensation programs and to ensure the reasonableness of actual salaries and total compensation packages. Compensation of the senior most executives is reviewed annually. The Korn Ferry Hay Group reviews both cash and total compensation for overall reasonableness, for adherence to CHI's compensation philosophy, and for comparability to the not-for-profit healthcare market. This independent review is delivered by Korn Ferry Hay Group to the HR committee of the CHI Board of Stewardship Trustees annually at their September meeting and minutes are shared with the full board at the December meeting. The last review was September 13, 2016. In addition, Korn Ferry Hay Group completed a comprehensive review of all positions at the level of vice president and above in the fall of 2014 to determine and validate appropriate compensation levels. These levels have been reviewed annually since and revised based on market data, where applicable.

The CFO will provide a copy of the return to the board either at a board meeting or by emailing a copy of the return to each board member. Subsequent to the return being provided to the board, the tax department files the return with the appropriate federal and state agencies, making any non-substantive changes necessary to effect e-filing. Any such changes are not re-submitted to the board.
Form 990, Part VI, Line 12c Conflict of interest policyCatholic Health Initiatives ("CHI") has a Conflicts of Interest ("COI") policy in place to maintain the integrity of all of its activities. The policy applies to CHI Board of Stewardship Trustees and members of its committees; all board and board committee members of CHI Entities; all CHI employees; all CHI physicians (both employed and non-employed) and all physician administrators and leaders; advanced practice clinicians (both employed and non-employed); and all CHI research personnel (both employed and non-employed). Disclosure, review and management of perceived, potential or actual conflicts of interest are accomplished through a defined COI disclosure process. Each person has a general ongoing obligation to promptly and fully report to his/her direct manager, supervisor, medical staff office, board or board committee chair any situation or circumstance that may create a conflict of interest. The person must report the actual or potential conflict as soon as she/he becomes aware of it. In any situation where the person may be in doubt, a full disclosure should be made to permit an impartial and objective determination. In addition to the general ongoing obligation, there are initial disclosure obligations. The board, board committee members, and new employees are required to make disclosures at the time of their initial hiring/appointment. All non-employed, credentialed or contracted physicians are required to make disclosures at the time of their credentialing and during any subsequent reappointment or recredentialing. All researchers are required to make disclosures upon consideration of affiliation with a research sponsor. In addition to the general ongoing and initial disclosure obligations, there is an annual disclosure obligation. All corporate officers, board and board committee members, employees at the level of manager and above, researchers, supply chain employees, employed physicians, physician administrators and leaders, and employed advanced practice clinicians must complete a new conflict of interest disclosure annually. Disclosures of perceived, potential or actual conflicts involving financial interests are forwarded to the Conflicts of Interest Review Committee ("C-CIRC") or Legal Services Group for review depending on the position of the person involved. The C-CIRC reviews COI questionnaires containing disclosures of perceived or possible conflicts for employees at a level of manager or above, supply chain employees, researchers and physicians, physician administrators and leaders, and advanced practice clinicians (both employed and non-employed). In the determination of a conflict, a COI management plan will be developed for that person. With respect to those audiences for which the C-CIRC has review responsibility, the C-CIRC will facilitate development of any such conflict of interest management plan in collaboration with local CRP staff. A designated CHI Entity staff will be responsible for monitoring the COI management plan and for documenting monitoring activities. At its sole discretion, a CHI Entity may reject a Person's request to enter into the relationship in question, or require the relationship be sufficiently altered to avoid a potential COI. If the C-CIRC determines that there is a potential or actual conflict of interest that does not currently have appropriate controls to address the conflict of interest, it may recommend that the disclosing person be allowed to participate in the activity or transaction subject to restrictions as outlined in the COI management plan. If a Person does not agree with a determination made by the C-CIRC, its interpretation of the Policy or Addenda, or seeks an exemption or exception, the following steps should be followed. The Employee disputing the review decision, interpretation of the Policy, or seeking exemption or exception must present the matter to the Employee's immediate direct manager or supervisor for review and determination. If the Employee and the manager do not agree with the review decision, interpretation of the Policy, or seek exemption or exception, the manager shall consult with the manager's Vice President (or higher if the manager is a Vice President) to reach a determination. If the matter remains unresolved, it shall be referred to the CHI Vice President of Human Resources and the CHI Corporate Responsibility Officer. If they are unable to reach agreement, the matter shall be referred to the CHI General Counsel, whose decision shall be final. Reviews and determinations involving board and board committee members and corporate officers will be the responsibility of the board, board executive committee, or board chair, with guidance from the Legal Services Group (LSG). Annual COI disclosures of all trustee and corporate officers will be reviewed by the CHI Senior Vice President, Legal Services, and General Counsel or his or her designee who will report potential conflicts to the applicable Board Chair. The Board Chair or designee shall make such further investigation of any conflict of interest disclosures as he or she may deem appropriate. If the conflict involves the Board Chair, the Vice Chair will assume the Chair's role. Based on review and evaluation of the relevant facts and circumstances, the Board Chair will make an initial determination as to whether a conflict of interest exists and whether, pursuant to the COI Policy, review and approval or other action by the Board is required. A written record of the Board Chair's determination, including relevant facts and circumstances, will be made. The Board Chair shall then make an appropriate report to the Executive Committee of the Board concerning such review, evaluation and determination. If a difference of opinion exists between the Board Chair and another Trustee as to whether the facts and circumstances of a given situation constitute a conflict of interest or whether Board review and approval or other action is required within the COI Policy, the matter shall be submitted to the Board's Executive Committee, which shall make a final determination as to the matter presented. Such determination, including relevant facts and circumstances, will be reflected in the Executive Committee minutes and will be reported to the Board. When any conflict of interest is considered by the board, the trustee or corporate officer, as appropriate, must disclose all of the material facts to the Board. The trustee shall not vote and the trustee or corporate officer shall not use his or her personal influence on the matter. The trustee or corporate officer shall be excused from the meeting during discussion and vote on the conflict of interest. In reviewing such transactions between CHI or CHI Entities and vendors or other contractors who are, or are affiliated with, Trustees or Corporate Officers, the Board will act as it would in reviewing transactions with unrelated third parties. The transaction is not to be approved unless the Board determines that the transaction is fair to CHI or the CHI Entity. The Board must approve the transaction by a majority of the Trustees on the Board, without counting the vote of any individual who has an interest in the transaction. All determinations of conflicts of interest are reported as required by law, regulations, and CHI policy.
