There are two initiatives underway to increase the number of mothers who breastfeed as well as the length of time they continue to breast feed. The hospital has regular classes and support group meetings. NYP/Lawrence(also known as Lawrence Hospital Center) has implemented measures to increase the number of breast fed babies in our service area. These measures are described in detail in form 990, schedule H, part VI, line 2. The hospitals second prevention agenda area was preventing chronic disease, with focus on reducing the number of heart related deaths. Concerning this priority, we chose to focus our efforts on Reducing Obesity in Children and Adults. We have several interventions underway for 2015 that will further our focus on reducing heart related deaths. In 2014, the hospital's Outpatient Nutrition program, screenings and the promotion of cardiovascular health were at the core of the interventions. The hospital participated in many outreach events. These outreach events are described in detail in form 990, schedule H, part VI, line 2. In the hospitals most recently conducted Community health needs assessments both mental health services and alcohol and substance abuse were identified as areas of need by approximately 14% of respondents. NYP/Lawrence has several social workers and psychiatrists on staff, but not enough to support a full service offering. Since our new relationship with New-York/Presbyterian, The hospital is now able to refer patients in need of mental health and substance abuse services to New York Presbyterian Hospital for treatment. IN EVALUATING THE COLLECTABILITY OF ACCOUNTS RECEIVABLE, THE HOSPITAL ANALYZES ITS PAST HISTORY AND IDENTIFIES TRENDS FOR EACH OF ITS MAJOR PAYOR SOURCES OF REVENUE TO ESTIMATE THE APPROPRIATE ALLOWANCE FOR DOUBTFUL ACCOUNTS AND PROVISION FOR BAD DEBTS. MANAGEMENT REGULARLY REVIEWS DATA ABOUT THESE MAJOR PAYOR SOURCES OF REVENUE IN EVALUATING THE SUFFICIENCY OF THE ALLOWANCE FOR DOUBTFUL ACCOUNTS. THE ALLOWANCE FOR DOUBTFUL ACCOUNTS FOR SELF-PAY PATIENTS WAS APPROXIMATELY 81% AND 80% OF SELF-PAY ACCOUNTS RECEIVABLE AS OF DECEMBER 31, 2014 AND 2013, RESPECTIVELY. IN A SITUATION OF HIGH COST CARE, THE ACCOUNT WILL BE REVIEWED TO DETERMINE IF REASONABLE EFFORTS WERE UNDERTAKEN TO ENSURE THAT FINANCIAL ASSISTANCE WAS OFFERED OR APPROPRIATE BEFORE REFERRAL TO A COLLECTION AGENCY. IN ADDITION, FOR ELIGIBLE PERSONS, NEITHER THE HOSPITAL NOR ITS SUBSIDIARIES WILL FORCE THE SALE OR FORECLOSURE OF A PATIENT'S PRIMARY RESIDENCE TO PAY AN OUTSTANDING MEDICAL BILL. A COLLECTION AGENCY MUST RECEIVE WRITTEN CONSENT FROM A FINANCE DEPARTMENT DIRECTOR BEFORE IT CAN BEGIN LEGAL ACTION. ANY COLLECTION AGENCY REPRESENTING THE HOSPITAL OR ITS SUBSIDIARIES MUST FOLLOW THE FINANCIAL AID POLICIES AND PROCEDURES, INCLUDING THE APPLICATION PROCEDURE. COLLECTIONS ARE PROHIBITED AGAINST ANY PATIENT WHO WAS ELIGIBLE FOR MEDICAID AT THE TIME THE SERVICES WERE RENDERED WITH THE EXCEPTION OF CO-PAY, SURPLUS OR SPEND DOWN AMOUNTS. ADDITIONALLY, IF A PATIENT SHOULD RECEIVE A BILL FROM THE HOSPITAL OR ANY OF ITS SUBSIDIARIES AFTER FILING AN APPLICATION FOR FINANCIAL ASSISTANCE, THE BILL MAY BE DISREGARDED UNTIL A DECISION REGARDING ELIGIBILITY IS REACHED. Part VI, Line 2 NewYork-Presbyterian/Lawrence Hospital (also known as Lawrence Hospital Center), located in the heart of southern Westchester County, New York, is partnering with the Westchester County Department of Health and the area's hospitals and healthcare service agencies to fulfill requirements of the New York State Department of Health's Prevention Agenda 2013-17. In our efforts to meet the Agenda's requirements, NYP/Lawrence conducted a Community Needs Assessment Survey in 2013 to sample residents in its service areas to determine which two of the five Prevention Agenda initiatives it plans to address during this four year period. Based on the findings, NYP/Lawrence has focused on Preventing Chronic Diseases and Promoting Healthy Women, Infants and Children. More specifically, NYP/Lawrence and its partners will also address a health disparity by identifying ways to reduce premature deaths among African American and Hispanics and increasing breastfeeding among women. In 2014, the New York State Department of Health requested hospitals including NYP/Lawrence develop a summary of its priorities, goals and interventions found within its Community Service Plans submitted to the State in November 2013. The following is a summarization of NYP/Lawrence selected Prevention Agenda priority areas and any progress made as of December 2014. Promoting Healthy Women, Infants and Children The hospital selected two Prevention Agenda priority areas. The first is to "Promote Healthy Women, Infants and Children". Within that Agenda Priority item, the focus area was on Maternal and Infant Health. Specifically, we chose to identify strategies internally to encourage pregnant and post-partum mothers to choose to breastfeed their babies. The expected number of mothers we hoped to target 1,000. As of December 2014, the number of people actually reached was 800. Further, we were not addressing a disparity with this intervention. At this point in time, the current partner involved in the implementation of the Intervention is the Women's, Infants, and Children's (WIC) Office in New Rochelle, NY. The role of the partner is to coordinate programming and resources. Overall, we would describe our partners as engaged regarding the level of implementation of this intervention. Plans to work with WIC are set to begin in December 2014. Also, NYP/Lawrence is working with a coalition of local partners who each are addressing the breastfeeding initiative. The Westchester County Health Department is the lead agency in this coalition and the subsequent implementation of the overarching interventions. As a hospital, two initiatives underway to increase the number of mothers who breast feed as well as the length of time they continue to breast feed are regular classes and support group meetings. A two-hour class is held monthly to provide instruction and tips on how to prevent common problems and share positioning techniques. The hospital's Breast Feeding Support Group offers a time for new mother's to learn from each other and receive professional guidance from NYP/Lawrence's Lactation Counselor. The group meets on the second Monday of each month from 10 a.m. to noon in the hospital's Lobby Conference Room. There is no registration required and the meeting is free to attend and open to all, regardless of where a mother has given birth. Prenatal Education & Information: 1) Incorporated breastfeeding as topic in Childbirth Education classes. 2) Held Breastfeeding education classes monthly. 3) Provided breastfeeding information on prenatal tours. 4) Included information in pre-registration and discharge packets. 5) Included a schedule of support group meetings and contact information for Lactation Specialist in discharge materials. Intra-partum Measures: 1) Breastfeeding bill of rights provided and hanging in room. 2) Registered Nurse provides education to each patient. 3) Lactation counselor visits with each patient. 4) 81% of all Registered Nurses completed NYS 10 steps to successful breastfeeding (Gold standard in best practices). 5) Breast/bottle feeding removed as a feeding option in delivery room. 6) Physician orders indicate breastfeeding as primary method for nutrition of infant. 7) All NICU infant families have lactation counselor consultation. 8) Improve compliance with offering skin to skin contact within one hour for ALL deliveries. 9) Increase % of patients rooming in to between 18-24 hours. Post-Partum Measures: 1) Lactation counselor discharge phone call to all patients. 2) Lactation warm line available to patients. 3) Breast feeding support group meets at LHC regularly. Administrative Measures: 1) Maintain NYS approved Breastfeeding policy containing all 28 required criteria. 2) Mother-Baby Intent filed; next step is to file First Step on pathway to becoming a designated "Baby-Friendly" hospital. Breast Feeding Support Group: 1) Continue regular meetings of support group and engage more women through post-discharge follow-up, marketing and social media. Social Media & Marketing: 1) Utilize marketing initiatives including newsletter, website, and social media to share breastfeeding and nutritional tips for breastfeeding mothers 2) Engage with LHC Facebook group of breastfeeding mothers to announce support groups, tips, health information, etc. Preventing Chronic Disease: The second Prevention Agenda Priority Area NYP/Lawrence selected was Preventing Chronic Disease, with a focus on reducing the number of heart related deaths. Concerning this priority, we chose to focus our efforts on Reducing Obesity in Children and Adults. We have several interventions in this area including the construction of a new cardiac catheterization laboratory to open in May 2015 to be operated by our parent NewYork-Presbyterian here on site, the formation of a "Heart Disease Prevention Committee" made up of nurses, phExplanationTxt

Form And Line ReferencePart VII & Schedule JFormAndLineReferenceDesc
ExplanationCertain officers and key employees of the New York Presbyterian Hospital that are identified in part VII as officers or trustees of the Lawrence Hospital Center, Inc. are responsible for executing the mission and management of the New York Presbyterian Hospital (NYP) and its affiliated entities. Compensation for 2014 of these upper level executives includes the payout of an annual incentive plan and a long-term incentive plan. This performance-oriented program conditions payments upon the achievement of multiple individual and group performance measures. Measures to monitor performance include: operational and financial strength, patient quality and safety, patient satisfaction, advancement of patient care, and people development and partnership. Incentive awards may only be granted if the organization achieves a financial surplus. Even if all relevant performance measurements are achieved, the NYP Board of Trustees retains full discretion to make or not make any incentive awards, or to reduce the amount of any incentive award. This initiative is critical to assuring that NYP has the requisite leadership to create and manage a highly motivated and engaged workforce, to drive superior performance throughout the organization and to achieve top tier medical center status. As a separate matter, due to restrictions imposed by the Internal Revenue Code, upper level executives are limited in the amount of benefits received under a tax-qualified retirement plan. Like many employers, NYP supplements these executives' pension benefits through a supplemental ("nonqualified") retirement plan. The supplemental executive retirement plan (SERP) is subject to a multi-year vesting requirement (commencing after five years of participation in the SERP, in prorated amounts through age 65) which places an executive's supplemental retirement benefit at risk of forfeiture if the vesting requirements are not satisfied. Once vested, however, provisions of the Internal Revenue Code require that the vested executive include in current income the value of his or her vested supplemental retirement benefit. Notwithstanding the legal requirement to recognize the vested value of the supplemental retirement benefit as current income, the supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP (although, as permitted by the Internal Revenue Code, the supplemental retirement plan will effect a distribution of an amount necessary to satisfy the executive's tax liability resulting from the income recognition upon vesting). As noted, this supplemental retirement benefit will not be distributed to the executive until the executive actually retires from NYP. There are constantly changing legal, tax, accounting, and public disclosure rules for a SERP (supplemental executive retirement plan) in not-for-profit organizations. The executive Compensation Committee continuously monitors these changes and incorporates any changes into the overall SERP plan design. As in past years, the executive Compensation Committee of NYP requires a third party complete a review of the organization's compensation program to ensure its effectiveness in terms of government regulations, market conditions and the need to continually elevate organizational performance. The report also serves to meet the regulatory obligations to ensure that all elements of the executive compensation programs are reasonable. The individuals listed in part VII that are compensated by New York Presbyterian Hospital devote an average of sixty hours per week to perform their responsibilities for the New York Presbyterian Hospital and other related organizations in the aggregate. Part 1, Line 4a: Thomas Mastroianni received a severance payment effective 12/2014, amount $9,607.64.
Part VI, Section A, Line 2: Emme Deland and Gary Zuar have a business relationship Richard Dresdale and Dennis Glazer have a business relationship. Part VI, Section A, Line 4: On July 1, 2014, NYP Community Services, Inc became the active parent of the Lawrence Hospital center.
