Mission Description	Community Health Partnership (CHP) is a community coalition, and the very heart and soul of our purpose is to encourage collaboration to improve health care for residents of the Pikes Peak region. CHP was formed by local health care leaders to foster a coordinated approach to health care issues. CHP is not a service delivery organization, rather, our role is to facilitate coordination among existing health care providers and to identify ways in which we can work together to maximize community resources to better address health issues. Description	CATCH PROGRAM - Community Access to Coordinated Health The CATCH program engages medical providers to participate in a coordinated referral system for primary care, specialty consults, and ancillary services like x-rays and laboratory tests for uninsured individuals, optimizing the generosity of providers in an efficient manner. Description	RCCO - REGIONAL CARE COLLABORATIVE ORGANIZATION In 2011, CHP was awarded the Regional Care Collaborative Organization (RCCO) contract by the Colorado Health Care Policy and Financing Department as part of Medicaid reform. When the RCCO project was first announced, local health care leaders (all members of CHP), wanting to maintain local control over health care decisions, responded with a unique community-based approach to building an Accountable Care Organization. Community Care of Central Colorado (aka RCCO Region 7) coordinates care within a patient-centered, medical home model for 22,000 Medicaid clients in El Paso, Elbert, Park and Teller Counties.
