Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - OUR LADY OF THE LAKE HOSPITAL:. OUR LADY OF THE LAKE HOSPITAL (OLOL) AND SURGICAL SPECIALTY CENTER (SSC) OF BATON ROUGE: Input from persons who represent the broad interests of the community served was an integral component of the 2024 Community Health Needs Assessment (CHNA) process and was incorporated through a structured, inclusionary approach that engaged community stakeholders, public health officials, healthcare providers, and community residents. A CHNA workgroup composed of representatives from participating hospitals worked collaboratively with the MedBR Steering Committee, MedBR stakeholders, and community members to review quantitative data and identify emerging trends not evident in historical data. Additional stakeholder input was provided by organizations such as the Louisiana Department of Health, American Heart Association, East Baton Rouge Parish School System, Pennington Biomedical Research Center, Blue Cross Blue Shield of Louisiana, and numerous communitybased organizations serving medically underserved, lowincome, and minority populations. Notably, Dr. Stephanie Henson, Region2 Medical Director for the Louisiana Department of Health, Office of Public Health, participated in CHNA stakeholder meetings during the summer of 2024, confirmed the identified community health needs, and served on the MedBR Steering Committee, contributing public health expertise to the prioritization of significant health needs. Community perspectives were further incorporated through surveys distributed at public library branches, community events, partner organizations, and hospital employees, as well as through stakeholder meetings that ranked and validated community health priorities. Through this process, broad community input directly informed the identification and prioritization of the significant health needs addressed in the CHNA and the development of related implementation strategies.

Input from persons who represent the broad interests of the community served was a central component of the 2024 Community Health Needs Assessment (CHNA) for Our Lady of the Lake Surgical Hospital and was incorporated through a structured process that combined surveys, focus groups, and oneonone stakeholder interviews. A Steering Committee composed of hospital leadership and community health professionals guided the process and engaged both an Internal Stakeholder Committee (ISC) and an External Stakeholder Committee (ESC) to ensure representation from healthcare providers, communitybased organizations, faithbased leaders, and advocates for vulnerable, lowincome, and medically underserved populations. Community input was gathered through community member and healthcare provider surveys, focus groups with ISC and ESC members, and individual interviews with key community leaders. Examples of community representatives involved include leaders from Hope House, Bogalusa Mission, Northshore Community Foundation, Team Capables, faithbased organizations, local government officials, and law enforcement representatives. Public health expertise was incorporated through direct consultation with Gina Lagarde, MD, MBA, Regional Medical Director and Administrator for Region 9 of the Louisiana Office of Public Health, who participated in a oneonone stakeholder interview and provided insight on regional health trends, disparities, access barriers, and opportunities for crosssector collaboration. Feedback from these community representatives and public health professionals was synthesized with quantitative data to identify, validate, and prioritize significant community health needs and to inform the development of the CHNA findings and subsequent implementation strategy.

DURING THE CURRENT YEAR, OUR LADY OF the LAKE SURGICAL HOSPITAL IN SLIDELL CONTINUED TO ADDRESS THE FOLLOWING PRIORITY AREAS IDENTIFIED DURING THE PRIOR CHNA. THOSE NEEDS WERE OBESITY/NUTRITION AND ACCESS TO HEALTH SERVICES. THE HOSPITAL ALSO CONDUCTED A NEW COMMUNITY HEALTH NEEDS ASSESSMENT, WHICH IDENTIFIED THE FOLLOWING PRIORITY AREAS: OBESITY/NUTRITION AND ACCESS TO CARE. Based on the findings of the 2024 CHNA, OLOLSH developed and implemented an implementation strategy to address prioritized community health needs, SUCH AS limited access to healthcare services including pediatric specialty care gapS and obesity and nutrition-related conditions. To improve access to care in 2025, OLOLSH expanded clinical and surgical services to Our Lady of the Angels HOSPITAL in THE Washington parish community on a weekly basis. These services were designed to reduce geographic and financial barriers to specialty medical care for underserved and vulnerable populations. In addition, OLOLSH introduced pediatric dental services and critical care neurological hearing services to address identified gaps in pediatric specialty healthcare access. To address CHNA-identified needs related to obesity, nutrition, and chronic disease risk, OLOLSH provided monthly nutrition education and weight management services to patients receiving care at the facility. A licensed dietitian delivered individualized counseling and group education focused on healthy eating, disease prevention, and long-term lifestyle modification. These services were offered to populations experiencing barriers to healthcare access. Through these initiatives, OLOLSH directly responded to priority health needs identified in the CHNA and furthered its mission to improve health outcomes and access to care for the communities it serves.

Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceTHE LOUISIANA DEPARTMENT OF PUBLIC HEALTH IMPLEMENTED A NEW MEDICAID PAYMENT PROGRAM WHICH MATERIALLY INCREASED HOSPITAL MEDICAID FUNDING. THE NEW PAYMENT MODEL IS DESIGNED TO PAY HOSPITALS ABOVE COST BY PAYING AMOUNTS ROUGHLY EQUIVALENT TO RATES PAID BY COMMERCIAL INSURANCE COMPANIES. ACCORDINGLY, EVEN THOUGH SERVICES TO MEDICAID PATIENTS REMAINED CONSISTENT WITH PAST YEARS, NO COMMUNITY BENEFIT IS BEING DERIVED FROM THE SERVICES PROVIDED TO MEDICAID PATIENTS. A COST TO CHARGE RATIO WAS USED TO CALCULATE PART I, LINE 7A AND 7B. THE COST TO CHARGE RATIO WAS DERIVED FROM IRS WORKSHEET 2, RATIO OF PATIENT CARE COST TO CHARGES. THE COST TO CHARGE RATIO IS APPLIED TO GROSS CHARGES RELATED TO CHARITY CARE AND CARE COVERED BY MEDICAID AND OTHER GOVERNMENT PROGRAMS. FOR PART I LINES 7E, 7F, 7H AND 7I, DIRECT COSTS WERE CAPTURED FROM THE HOSPITAL'S AUDITED FINANCIAL STATEMENTS AND THE MEDICARE COST REPORT WHERE APPLICABLE. FOR PART I LINE 7G, COST FIGURES WERE CALCULATED FROM DEPARTMENTAL COSTS LESS DIRECT OFFSETTING REVENUE.

This cutting-edge technology represents a significant advancement in cardiac care, offering patients an effective and more efficient option for managing their arrhythmias with significantly less complications. The hospital is the first in the region to offer this state-of-the-art treatment. The PFA system utilizes innovative pulsed field energy to precisely ablate heart tissue. Unlike traditional thermal ablation techniques, PFA targets specific heart tissues with high precision while sparing surrounding structures, leading to potentially fewer complications and faster recovery times for patients also in july 2024, ololRMC implemented an extracorporeal membrane oxygenation (ECMO) program to provide life support for patients with respiratory or heart failure. The hospital is the first and only in the region and only one of 4 in Louisiana with this lifesaving adult program. ECMO is a complex medical therapy that allows the heart and/or lungs to rest when the body does not respond to conventional medical treatment and the organs are too sick to properly support normal body functions. ECMO does not heal the body but allows it to rest while the illness or injury improves. Patients placed on ECMO have an acute and reversible injury or illness. Examples of this include shock, ARDS, respiratory failure, severe respiratory viruses, sepsis, inhalation injury and trauma. Once the X-rays, labs and other vital aspects show improvement, the ECMO support is gradually weaned. The patient is then removed from the ECMO machine, and their heart and lungs return to working on their own. in september 2025, ololRMC Heart and Vascular Institute announced that it had earned The Joint Commission's Gold Seal of Approval for Comprehensive Stroke Center Certification, the first and only hospital in the region to achieve this distinction. This designation is reserved for hospitals that can quickly and accurately treat the most complex stroke cases and have proven better outcomes for patients. As a Comprehensive Stroke Center of Excellence, the stroke center offers 24/7 care for clot retrieval and coiling procedures during hemorrhagic stroke, setting it apart from other stroke programs in the region. The team's average monthly "door to needle" clot-busting treatment time is under 30 minutes, among the fastest times in the state. A specialized, 20-bed neurocritical care unit is staffed by nurses certified in stroke and critical care, ensuring tailored, expert care for individuals with stroke and neurological conditions throughout their recovery. ololRMC is a leader in stroke education, partnering with the community and stroke navigators and medical staff in their treatment facilities to increase awareness of the signs, symptoms and risk factors for stroke. The stroke program at olol also benefits from a partnership with The Neuromedical Center, ensuring patients have access to the region's best resources for comprehensive neurosciences care. in october 2024, ololRMC Cancer Institute announced the implementation of a world-class Hematologic Malignancy Cell Therapy program.
