Schedule H, Part V, Section B, Line 3EThe significant health needs are a prioritized description of the significant health needs of the community and identified through the CHNA.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - UF HEALTH LEESBURG HOSPITAL. UFHL took into account input from persons representing the broad interests of the community served as part of its 2024 Community Health Needs Assessment. The CHNA covered the hospital's three-county primary service area. UFHL commissioned Conduent Healthy Communities Institute to support the CHNA process, including facilitation of community input, data analysis, and report preparation. Between February and April 2025 through a Community Partner Listening Session was conducted with community leaders from Lake and Sumter counties and key informant interviews were conducted in Marion County. Participants were selected because of their public health expertise, knowledge of community health needs, representation of the broad interests of the community, and/or ability to speak to the needs of medically underserved, low-income, minority, and at-risk populations. The discussions were conducted by phone and/or webinar using a structured questionnaire. Discussion topics included (1) the biggest perceived health needs in the community, (2) barriers of concern and (3) the impact of health issues on vulnerable populations. Following the interviews and listening sessions, the UFHL project team analyzed the data captured using qualitative analysis tools in Qualtrics . PRIMARY AND SECONDARY DATA INDICATED THAT THE POPULATIONS MOST AT RISK INCLUDE RESIDENTS OF RURAL COMMUNITIES, UNDERINSURED INDIVIDUALS, OLDER ADULTS, women, ethnic minorities AND LOW-INCOME RESIDENTS.
Schedule H, Part V, Section B, Line 6a Facility , 1Facility , 1 - UF HEALTH LEESBURG HOSPITAL. UF Health Spanish Plaines
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - UF HEALTH LEESBURG HOSPITAL. UFHL HAS IDENTIFIED THE THREE FOLLOWING AREAS ON WHICH TO FOCUS. STRATEGIC PRIORITY 1: ACCESS TO QUALITY HEALTHCARE UFHL is addressing access to quality healthcare by increasing access to needed health services, supporting care coordination, and reducing barriers to care for patients and community members. The organization is implementing and maintaining a Mobile Stroke Unit in Lake County and expanding rendezvous capability with surrounding counties to bring time-sensitive stroke care closer to patients. UFHL further supports access to care by screening self-pay patients and referring qualifying individuals to insurance and assistance programs, including SSI Disability, Medicaid, and charity care. In addition, UFHL maintains and expands clinical training site opportunities for medical and non-medical students to support development of the future health care workforce and strengthen long-term access to care in the community. In the year ended June 30, 2025, UF Health broke ground on a new hybrid emergency and urgent care center in Sumter County. The center is intended to increase access to lifesaving health care services and improve access to emergency services for residents in Oxford, Wildwood, The Villages, and surrounding communities. STRATEGIC PRIORITY 2: WOMEN'S HEALTH UFHL is addressing women's health by increasing access to education, screenings, outreach, and support services for women in the community. UFHL promotes cancer awareness, screenings, education, and support for women through the UF Health Cancer Center Mobile Cancer Bus, which has a mobile outreach footprint that includes 22 counties, including Lake, Marion, and Sumter counties. UF Health Cancer Center also created an alliance with Blooming Butterfly, a local nonprofit organization that supports women with breast, ovarian, and uterine cancers. Through this alliance, UF Health Cancer Center and Blooming Butterfly host community education events and work to reduce barriers to timely, high-quality cancer care. UFHL also supports women's health by increasing health education on women's health-focused topics through Wellness University programming and community health fairs in Marion, Lake, and Sumter counties. In the year ending June 30, 2025, UF Health launched a new OB/GYN mobile outreach vehicle to expand access to gynecologic and maternal care for individuals in the community who experience barriers to accessing these needed services. Strategic priority 3: Healthy Aging (older adults 65+) UFHL is addressing healthy aging by supporting older adults through resource navigation, safety initiatives, health education, and strategies that promote access to community-based supports. During the year ending June 30, 2025, UFHL used its increased community presence through initiatives such as the Mobile Stroke Unit and the UF Health Cancer Center Mobile Cancer Bus to connect residents with FindHelp. FindHelp is available through Epic MyChart and assists patients with identifying community resources and programs related to transportation, food, utilities, housing, and other social needs. By connecting older adults and other vulnerable community members to available resources, UFHL supports care coordination, helps address social determinants of health, and promotes healthier aging in the community. HEALTH NEEDS NOT ADDRESSED: The following health needs were identified during the CHNA process. While UFHL acknowledges these needs as concerning and persistent issues for many individuals in the UF Health Central Florida and UF Health Leesburg Hospital service areas, the organization is focusing its 2026-2028 implementation efforts on the three prioritized health needs because they were ranked highest through the CHNA prioritization process and align with the hospital's ability to make a meaningful impact. Many of the topics listed below overlap with the three prioritized health needs and are expected to be positively impacted through existing collaborative strategies and activities. Behavioral Health Cancer Chronic Disease Prevention Oral Health Prevention & Safety Wellness & Quality of Life

Schedule H, Part VI 501(r) FailureDescription of Failure For the taxable year ending June 30, 2025, LEESBURG REGIONAL MEDICAL CENTER did not complete the required implementation strategy approval process within the timeframes required by section 501(r)(3). The related implementation strategy was adopted by the governing body on December 2, 2025, 17 days after the required deadline of November 15, 2025. Description of Correction In accordance with Rev. Proc. 2015-21, LEESBURG REGIONAL MEDICAL CENTER obtained formal governing body approval of the implementation strategy on December 2, 2025. Following approval, the implementation strategy was finalized and made widely available to the public. THE FAILURE WAS DISCOVERED ON MAY 8, 2026, AFTER THE CORRECTIVE ACTION HAD ALREADY BEEN COMPLETED. Description of Practices or Procedures Going Forward This situation was inadvertent and administrative in nature and did not reflect a failure to identify or address community health needs. The underlying CHNA activities and related planning were performed, and the delay was limited to the timing of formal governing body approval. LEESBURG REGIONAL MEDICAL CENTER has updated its internal compliance calendar and governance procedures to ensure that future CHNAs are completed within the required three-year cycle and that implementation strategies are adopted within the required timeframe. LEESBURG REGIONAL MEDICAL CENTER remains committed to full compliance with Section 501(r)(3).
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceTHE ORGANIZATION USED THE COST TO CHARGE RATIO TO COMPUTE THE COST OF FINANCIAL ASSISTANCE AND MEDICAID REPORTED ON PART I, LINE 7. A DIRECT COST METHODOLOGY WAS USED FOR ALL OTHER AMOUNTS.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountThe organization has determined that amounts not expected to be collected from patients represent implicit price concessions rather than bad debts. Accordingly, patient service revenue is recorded at the estimated transaction price, reflecting these concessions at the time of service, and is reported net of such amounts. As a result, no separate bad debt expense is recognized in the audited financial statements.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyThe organization does not record bad debt expense, as amounts not expected to be collected from patients-including those eligible for financial assistance-are treated as implicit price concessions. These amounts are reflected as a reduction of patient service revenue. Accordingly, there is no bad debt attributable to patients eligible for financial assistance.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteTHE PROVISION FOR BAD DEBTS IS BASED ON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS, CONSIDERING BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN FEDERAL AND STATE GOVERNMENTAL HEALTH CARE COVERAGE, AND OTHER COLLECTION INDICATORS. THROUGHOUT THE YEAR, MANAGEMENT ASSESSES THE ADEQUACY OF THE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS BASED UPON THESE TRENDS. THE RESULTS OF THIS REVIEW ARE THEN USED TO MAKE ANY MODIFICATION TO THE PROVISION FOR BAD DEBTS TO ESTABLISH AN APPROPRIATE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS. PATIENT ACCOUNTS RECEIVABLE ARE WRITTEN OFF AFTER COLLECTION EFFORTS HAVE BEEN FOLLOWED UNDER UFHL's POLICIES.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsTHE ORGANIZATION UTILIZES THE MEDICARE COST REPORT TO REPORT THE TOTAL REVENUE RECEIVED FROM MEDICARE AND THE MEDICARE ALLOWABLE COSTS. UFHL ACCEPTS ALL MEDICARE PATIENTS WITH THE KNOWLEDGE THAT THERE MAY BE SHORTFALLS AND OPERATES TO PROMOTE THE HEALTH OF THE COMMUNITY. UFHL BELIEVES THAT ANY MEDICARE SHORTFALL SHOULD BE TREATED AS A COMMUNITY BENEFIT BECAUSE MEDICARE DOES NOT TYPICALLY FULLY COMPENSATE UFHL FOR THE COST OF PROVIDING HOSPITAL CARE TO MEDICARE BENEFICIARIES.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceAccording to the organization's Financial Assistance Policy (which contains certain billing and collection provisions), any balance not qualifying for financial assistance or account balances not related to initial approval based on diagnosis and/or place of service shall be subject to extraordinary collection actions (ECAs) unless appropriate financial arrangements have been established. Thus, patients known to qualify for financial assistance are not subject to ECAs.

Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceTHE HOSPITAL HAS SEVERAL MECHANISMS FOR EDUCATING PATIENTS ABOUT ITS FINANCIAL ASSISTANCE POLICY: A PLAIN-LANGUAGE SUMMARY OF THE POLICY IS POSTED under "Step 1: Completed a financial assistance application" on the financial assistance portion of the Hospital's website, ALONG WITH LOCAL AND TOLL-FREE TELEPHONE NUMBERS WHERE FINANCIAL REPRESENTATIVES MAY BE REACHED TO DISCUSS QUESTIONS; SIGNAGE IS POSTED IN THE EMERGENCY DEPARTMENTS, ADMISSIONS DEPARTMENTS, AND OTHER ANCILLARY DEPARTMENTS INDICATING THAT FINANCIAL ASSISTANCE IS AVAILABLE FOR THOSE UNINSURED AND UNDERINSURED PATIENTS MEETING ELIGIBILITY REQUIREMENTS, AND AFTER EMTALA SCREENING AND STABILIZATION REQUIREMENTS ARE MET, FINANCIAL COUNSELORS WORK WITH THOSE PATIENTS WHO MOST LIKELY QUALIFY FOR MEDICAID OR UNDER THE HOSPITAL'S FINANCIAL ASSISTANCE POLICY; NEARLY EVERY HOSPITAL INPATIENT IS VISITED BY A REPRESENTATIVE FROM ADMISSIONS, TO ENSURE WE HAVE THEIR CORRECT INSURANCE INFORMATION AND TO INFORM THEM ABOUT THE ASSISTANCE POLICY; AND INFORMATION ON THE FINANCIAL ASSISTANCE POLICY IS INCLUDED WITH THE INVOICES/FINANCIAL STATEMENTS THAT ARE MAILED TO PATIENTS. IN ADDITION, THE HOSPITAL PROVIDES FINANCIAL COUNSELING AND INFORMATION TO INDIVIDUAL PATIENTS AND ASSISTS THEM IN APPLYING FOR LOCAL, STATE, AND FEDERAL HEALTH CARE PROGRAMS SUCH AS MEDICARE AND MEDICAID OR ENROLLING IN AN INSURANCE PLAN THROUGH THE FEDERAL HEALTH CARE EXCHANGE. UFHL PROVIDES A SELF-PAY DISCOUNT FOR UNDER-INSURED PATIENTS WHO DO NOT OTHERWISE QUALIFY UNDER THE FINANCIAL ASSISTANCE PROGRAMS.
Schedule H, Part VI, Line 4 Community informationPopulation UF Health Leesburg Hospital serves residents of Lake, Marion, and Sumter counties in Central Florida. The primary service area had an estimated 2024 population of 1,027,802 residents, including 444,204 in Lake County, 428,905 in Marion County, and 154,693 in Sumter County. Lake and Marion counties have age distributions more similar to Florida overall, while Sumter County has a significantly older population, with 66.0% of residents age 65 or older compared to 23.7% statewide. The service area is majority White, with Hispanic residents representing 19.6% of Lake County, 17.6% of Marion County, and 6.5% of Sumter County, compared to 27.9% statewide. English is the most common language spoken at home, and Spanish is the second most common language across the service area. In Lake County 12.1% of the population over the age of 5 speaks Spanish. Life Expectancy The service area includes several factors associated with premature death and poorer health outcomes, including income, poverty, housing instability, unemployment, access to care, and socioeconomic need. Higher-income individuals are more likely to have greater life expectancy and reduced risk of conditions such as heart disease, diabetes, obesity, and stroke. Areas of high socioeconomic need are also associated with preventable hospitalizations, premature death, and poorer self-reported health and well-being. Economic Characteristics The service area includes communities with varying economic conditions. Marion County had the lowest median household income at $60,771, below the Florida median of $69,226. Lake County's median household income was $69,443, slightly above Florida, while Sumter County's median household income was $67,873, slightly below Florida. Marion County also had the highest percentage of families living below the federal poverty level at 9.6%, compared to 9.3% statewide; Lake and Sumter counties were each lower at 6.0%. All three counties had higher unemployment rates than Florida overall; 6.4% in Lake and Marion Counties and 6.3% in Sumter county, compared to 5.4% in Florida overall. Housing affordability is also a concern, particularly in Sumter County, where 60.3% of renters spend 30% or more of household income on rent, compared to 58.7% statewide. Safe, stable, and affordable housing provides a critical foundation for health and well-being; the lack of affordable housing is linked to increased stress and mental health struggles, and an increased risk of disease. Education Lake County and Sumter County have a higher percentage of residents age 25 and older with a high school diploma or higher than Florida overall, at 90.9% and 92.8%, respectively, compared to 89.4% statewide. Marion County is slightly lower than the state, with 88.9% of residents age 25 and older having a high school diploma or higher. Bachelor's degree attainment is lower in Lake and Marion counties than in Florida overall. In Lake County, 26.4% of residents age 25 and older have a bachelor's degree or higher, and in Marion County, 22.2% have a bachelor's degree or higher, compared to 32.3% statewide. Sumter County is higher than the state, with 34.0% of residents age 25 and older having a bachelor's degree or higher.
