Schedule H, Part V, Section B, Line 3EThe significant health needs are a prioritized description of the significant health needs of the community and identified through the CHNA.
Schedule H, Part V, Section B, Line 5 Facility 1, 1Facility 1, 1 - UM Capital Region Medical Center. Similar to the 2022 Community Health Needs Assessment (CHNA), the 2025 CHNA was a County-wide joint CHNA process, led by the Prince George's County Health Department in collaboration with the county's hospitals. As part of the 2025 Joint Community Health Assessment, Prince George's County Health Department (PGCHD) conducted key informant interviews with 31 County leaders drawn from diverse backgrounds with varying perspectives on health in the county. The 2025 Community Resident Survey was available in English, French, and Spanish and open to anyone who lived, worked, played (engaged in recreation), studied, or worshiped in Prince George's County. The Key Informant Interviews were conducted to gain deeper insights into trends and relationships among social determinants of health, health outcomes, and mortalities. These interviews also explored the strengths and weaknesses of existing resources that impact population health in Prince George's County. Key informants represent various sectors, focus areas, and geographic regions of the County were invited to share their expertise and perspectives on the health needs and assets of the Prince George's County communities. Many of these informants represented subpopulations that face unique barriers and disparities. These subpopulations included: 1. Individuals actively involved in and/or transitioning from the criminal justice system. 2. Individuals experiencing housing insecurity or homelessness 3. Uninsured and underinsured adults and children 4. Spanish-speaking individuals (Spanish is the primary language spoken at home) 5. Active members of the military and veterans 6. Immigrant communities 7. Aging adults
Schedule H, Part V, Section B, Line 6a Facility 1, 1Facility 1, 1 - UM Capital Region Medical Center. Since 2016, the Prince George's County Health Department has partnered with the four hospitals located in the County to create a joint Community Health Assessment (CHA). These hospitals include Luminis Health Doctors Community Hospital, Adventist HealthCare Fort Washington Medical Center, MedStar Southern Maryland Hospital Center, and University of Maryland Capital Region Medical Center. For the 2025 CHNA, a Core Team of staff from all four hospitals and the Health Department's Office of Assessment and Planning led the process and prepared the report.

Schedule H, Part V, Section B, Line 11 Facility 1, 1Facility 1, 1 - UM Capital Region Medical Center. Together with partners across the County and Prince George's Health Action Coalition members, the CHNA Core Team collected the community's input on the County's health needs and priorities. Although many trends from the 2022 CHA and pre-COVID reports stayed the same, the Core Team found new areas to address based on the community's lived experience with staying healthy and maintaining their health. During the 2025 CHNA cycle, the Core Team analyzed the primary and secondary data sources to develop the five (5) leading health priorities: * Chronic Disease * Maternal and Infant Health * Behavioral Health * Access to Care * Social Determinants of Health After analyzing the data sources to understand the County's health needs, UM Capital worked collaboratively with a team of key interdisciplinary hospital leaders to further identify and prioritize the health priority areas for UM Capital. After an extensive review, the team determined that UM Capital will prioritize the health needs listed below. 1. Chronic Disease & Physical Health 2. Cancer 3. Maternal & Child Health Priority Health Issue #1: Chronic Disease & Physical Health UM Capital Region is committed to implementing strategic clinical-community partnerships to improve awareness of and access to chronic disease management and preventive health screening programs by continuing our health education and screening efforts and encouraging engagement in appropriate medical care. Additionally, with the recent implementation of the Food is Medicine Program, targeting pregnant and non-pregnant patients suffering from chronic diseases, the new Adult Sickle Cell Clinic, and the recently opened Cancer Center, UM Capital is positioned to increase access to care and increase behavioral change among those suffering with or at-risk for developing chronic diseases and other complex conditions. Lastly, UM Capital's Violence Intervention Program ( CAP-VIP) deploys a trauma team to all incoming trauma cases- the program offers a holistic approach to trauma-informed care that includes the survivor and caregiver. Priority Health Issue #2: Cancer UM Capital is committed to working collaboratively with community agencies to reduce cancer disparities within minority populations, particularly targeting Breast, Prostate, Colon, and Lung Cancers. UM Capital is committed to improving education and access to Cancer screenings to facilitate early detection and linkages to treatment through the recent opening of the Regional Cancer Center. Priority Health Issue #3: Maternal & Infant Health UM Capital is committed to improving access to high-quality prenatal healthcare for the most vulnerable in our community. UM Capital operates the Mama & Baby Mobile Health Unit in collaboration with the March of Dimes, providing under and uninsured women free access to prenatal care and social support, decreasing the risk of further complications and increasing the chances of a healthy delivery. UM Capital is also committed to reducing the Severe Maternal Mortality rate by launching a cadre of health equity initiatives in collaboration with clinical-community providers, including remote monitoring of pregnant patients with chronic diseases and establishing a referral network to post-pregnancy services. Our full 2026-2028 Community Health Implementation Strategy can be found at; https://www.umms.org/capital/-/media/files/um-capital/community/community reports/umcapchimplementationstrategyfy2628final.pdf?upd=20250624174219 While Social Determinants of Health, is not listed as a specific priority, we understand the significant impact environmental, behavioral, and socioeconomic factors have on ones' health. Thus, as an anchor institution, we are responsible for focusing on the factors outside our walls that affect health and well-being. To do this, we plan to foster opportunities to integrate social determinant initiatives and activities throughout our Community Health Implementation Strategy to provide a more holistic approach to health and well-being. Furthermore, while we also did not specifically name Behavioral Health or Access to Care in our top 3 health priority areas, we still plan to address these needs through targeted initiatives and activities integrated within the Chronic Disease & Physical Health priority area. Goals were established under each priority area, and initiatives were aligned with the goals to address the identified health needs. Needs not addressed: Housing and homelessness- was not selected as a priority, due to a lack of adequate resources to support this effort.
Schedule H, Part V, Section B, Line 13 Facility 1, 1Facility 1, 1 - UM PRINCE GEORGE'S HOSPITAL CENTER AND UM LAUREL REGIONAL MEDICAL CENTER. THE FINANCIAL ASSISTANCE POLICY EXPLAINS SEVERAL ELIGIBILITY CRITERIA, INCLUDING PARTICIPATION IN MEDICAID/MEDICARE PROGRAMS AS WELL AS ELIGIBILITY UNDER VARIOUS STATE REGULATIONS. IN ADDITION TO FPG, THE INCOME LEVELS DEFINED BY THE MARYLAND STATE DEPARTMENT OF HEALTH AND MENTAL HYGIENE (MD DHMH) ARE USED TO DETERMINE ELIGIBILITY FOR FINANCIAL ASSISTANCE. THE MD DHMH INCOME LEVELS ARE MORE GENEROUS THAN THE FPG INCOME LEVELS.

Schedule H, Part I, Line 3c CRITERIA FOR FREE OR DISCOUNTED CAREUM CAPITAL REGION IS COMMITTED TO PROVIDING FINANCIAL ASSISTANCE TO PERSONS WHO HAVE HEALTH CARE NEEDS AND ARE UNINSURED, UNDERINSURED, INELIGIBLE FOR A GOVERNMENT PROGRAM, OR OTHERWISE UNABLE TO PAY, FOR MEDICALLY NECESSARY CARE BASED ON THEIR INDIVIDUAL FINANCIAL SITUATION. THE FAP ALSO USES A FINANCIAL HARDSHIP THRESHOLD WHEN DETERMINING ELIGIBILITY. A PATIENT WITH MEDICAL DEBT EXCEEDING 25% OF FAMILY ANNUAL HOUSEHOLD INCOME MAY BE ELIGIBLE FOR FINANCIAL ASSISTANCE.
Schedule H, Part I, Line 5a BUDGET FOR CHARITY CARETHE ORGANIZATION DOESN'T BUDGET A PRESET PERCENTAGE FOR CHARITY CARE. IT IS THE ORGANIZATION'S POLICY TO PROVIDE FINANCIAL ASSISTANCE TO ANY INDIVIDUAL THAT QUALIFIES UNDER THE ORGANIZATION'S FINANCIAL ASSISTANCE POLICY, REGARDLESS OF THE AMOUNT OF CHARITY CARE PROVIDED BY THE ORGANIZATION DURING THE YEAR. IT IS PART OF OUR MISSION TO SERVE AS THE SAFETY NET FOR THE UNINSURED AND UNDERINSURED.
Schedule H, Part I, Line 6b RELATED ORGANIZATION BENEFIT REPORTTHE ORGANIZATION ANNUALLY FILES A COMMUNITY BENEFIT REPORT AS REQUIRED BY THE MARYLAND HSCRC. THE REPORT CAN BE FOUND AT HTTPS://HSCRC.STATE.MD.US/PAGES/INIT_CB.ASPX
Schedule H, Part VI, Line 5 Promotion of Community HealthPublic Safety continues to be a critical issue affecting Prince George's County area and surrounding communities. UM Capital's Violence Prevention Program hosted several community-based events to bring awareness and education to trauma informed care, recovery, and survivorship. Our Trauma program hosted a Gun Violence Summit in 2025, a conference bringing together researchers, legislators, law enforcement and community members and leaders to discuss the issue of gun violence in Prince George's County and discuss solutions. There were speakers, breakout sessions and Stop the Bleed training on critical wound management, and 110 attendees were present. The Trauma program also hosted a Survivor Empowerment Exchange, a culminating event after job readiness and professional dress training to empower survivors of violence. There were multiple employers and career preparedness organizations present including a Wider Circle and Employ Prince George's. A total of 68 attendees were present, and two program participants were highlighted for their personal and professional progress since injury. Finally, our trauma program hosted a community wellness fair summer of 2025 and provided comprehensive care needs for our participants and their families with blood pressure checks, stroke awareness, CPR and stop the bleed training and injury prevention, 450 attendees were present. Gilead Sciences Inc. Screening Grant Aligned with state public health goals, and in partnership with the Gilead FOCUS initiative, the University of Maryland Capital Region Health (UMCRH) implemented a comprehensive HIV, HCV, and STI program focused on education, case management, informatics, and testing to address high transmission rates in Prince George's County. The program seeks to improve provider awareness, community awareness, enhance data management, increase access to testing and improve linkage to care. Through clinician education on sexual health screening including STI an HIV diagnosis and linkage to care, enhanced data analytics, and integrating sexual health indicators into electronic health records (EHR), the initiative aims to streamline and expand testing access across inpatient and outpatient settings through, education, Navigation & Data Analysis, Informatics and Patient Testing. Summary of Early Impact: * 1,800+ patients screened since program launch ( 12/31/2004) * Opt-out screening protocols implemented across all inpatient admissions. * Comprehensive EMR workflows in place for rapid follow-up. * Multiple new partnerships established for care coordination. * Staff onboarding and training underway. * Budget modification approved and financial oversight strengthened. These early outcomes represent a strong foundation for ongoing implementation, scale-up, and sustainability. Food Is Medicine Program The Food Is Medicine Program at UM Capital Region is a transformative initiative aimed at alleviating food insecurity, supporting chronic disease management, and advancing health equity across Prince George's County. In 2025, our onsite food pharmacy continued to address critical gaps in maternal health and nutrition access by integrating healthy groceries, individualized nutrition education, and coordinated clinical care directly into the prenatal setting. The program offers consistent, dignified support to eligible participants while embedding social determinants of health into routine medical care. The program continues to serve predominantly pregnant and postpartum individuals facing high levels of food insecurity and chronic illness. Screening data show that 97.2% of participants reported food insecurity, with 77.5% lacking reliable transportation and 36.5% relying on emergency food sources, primarily SNAP and WIC. Nutritional intake prior to enrollment was limited, with 7.9% of participants consuming fruit fewer than once per week and 3.2% consuming vegetables at that same low frequency. In addition to meeting immediate needs, our program has demonstrated early signs of clinical impact. Baseline metrics for enrolled participants showed average A1C and glucose levels lower than the hospital-wide averages, and nearly all participants had access to a kitchen to prepare the food provided. A majority reported feeling confident or somewhat confident in managing their health conditions. Care Coordination & Care Transitions Support The UM Capital Region Care Transitions Team plays a critical role in supporting patients as they move from the hospital to post-acute care. By using evidence-based tools to identify patients at high risk for re-admission or complications, the team ensures care does not stop at discharge. Appointment Coordination Scheduling PCP and specialist follow-ups with details included in discharge instructions. Uninsured patients connected with Federally Qualified Health Centers; those without PCPs linked to Capital Region Medical Group. Post-Discharge Follow-Up Coordinator contact within 72 hours of discharge. All patients receive automated post-discharge calls, with additional personal weekly calls offered for 30 days to monitor progress and address emerging concerns. Resource Connection Assistance with social determinants of health including food insecurity, transportation needs, and medication affordability. Navigation services to access community-based support programs. Our comprehensive service package addresses both clinical and social needs affecting recovery. By ensuring timely follow-up appointments, monitoring health status post-discharge, and connecting patients with essential resources, we create a supportive bridge between hospital and home. This proactive approach helps identify and resolve potential issues before they escalate into complications requiring emergency care or readmission. In FY25, 18,799 patients discharged from inpatient units and the ED received a post-discharge phone call through the Connect system. Additionally, 1,136 patients were assessed at the bedside prior to discharge, and 896 follow-up appointments were scheduled before discharge.
Schedule H, Part VI, Line 7 State Filing of Community Benefit ReportAs required by Maryland statute for all hospitals, University of Maryland Medical Center submits a detailed, annual community benefit report, which provides information related to programs, services, contributions, etc. that the hospital makes with no or little expectation of financial return, to the Maryland Health Services Cost Review Commission (HSCRC), a state regulatory agency, by January 31 each year.
Schedule H, Part I, Line 7g Subsidized Health ServicesTHE ORGANIZATION INCLUDED A NET COST OF $5,855,900 ATTRIBUTABLE TO PHYSICIAN CLINICS ON LINE 7G.
Schedule H, Part I, Line 7 Bad Debt Expense excluded from financial assistance calculation37301599
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceMARYLAND'S REGULATORY SYSTEM CREATES A UNIQUE PROCESS FOR HOSPITAL PAYMENT THAT DIFFERS FROM THE REST OF THE NATION. THE HEALTH SERVICES COST REVIEW COMMISSION, (HSCRC) DETERMINES PAYMENT THROUGH A RATE SETTING PROCESS AND ALL PAYORS, INCLUDING GOVERNMENTAL PAYORS, PAY THE SAME AMOUNT FOR THE SAME SERVICES DELIVERED AT THE SAME HOSPITAL. MARYLAND'S UNIQUE ALL PAYOR SYSTEM INCLUDES A METHOD FOR REFERENCING UNCOMPENSATED CARE IN EACH PAYORS' RATES, WHICH DOES NOT ENABLE MARYLAND HOSPITALS TO BREAKOUT ANY OFFSETTING REVENUE RELATED TO UNCOMPENSATED CARE. COMMUNITY BENEFIT EXPENSES ARE EQUAL TO MEDICAID REVENUES IN MARYLAND, AS SUCH, THE NET EFFECT IS ZERO. ADDITIONALLY, NET REVENUES FOR MEDICAID SHOULD REFLECT THE FULL IMPACT ON THE HOSPITAL OF ITS SHARE OF THE MEDICAID ASSESSMENT.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountTHE HEALTH SERVICES COST REVIEW COMMISSION (HSCRC) STARTED SETTING HOSPITAL RATES IN 1974. AT THAT TIME, THE HSCRC APPROVED RATES APPLIED ONLY TO COMMERCIAL INSURERS. IN 1977, THE HSCRC NEGOTIATED A WAIVER FROM MEDICARE HOSPITAL PAYMENT RULES FOR MARYLAND HOSPITALS TO BRING THE FEDERAL MEDICARE PAYMENTS UNDER HSCRC CONTROL. IN 2014, MARYLAND'S WAIVER WITH MEDICARE WAS RENEGOTIATED AND UPDATED TO REFLECT THE CURRENT HEALTHCARE ENVIRONMENT. UNDER THIS NEW WAIVER, SEVERAL CRITERIA WERE ESTABLISHED TO MONITOR THE SUCCESS OF THE SYSTEM IN CONTROLLING HEALTHCARE COSTS AND THE CONTINUANCE OF THE WAIVER ITSELF: 1. REVENUE GROWTH PER CAPITA 2. MEDICARE HOSPITAL REVENUE PER BENEFICIARY 3. MEDICARE ALL PROVIDER REVENUE GROWTH PER BENEFICIARY 4. MEDICARE READMISSION RATES 5. HOSPITAL ACQUIRED CONDITION RATE
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyBECAUSE OF THE UNIQUE PAYMENT SYSTEM DESCRIBED ON LINE 2 (ABOVE), THE HOSPITAL IS UNABLE TO ESTIMATE HOW MUCH OF THE AMOUNT REPORTED IN LINE 2 IS ATTRIBUTED TO PATIENTS WHO ARE ELIGIBLE FOR FINANCIAL ASSISTANCE UNDER THE FAP.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteTHE CORPORATION RECORDS REVENUES AND ACCOUNTS RECEIVABLE FROM PATIENTS AND THIRD-PARTY PAYORS AT THEIR ESTIMATED NET REALIZABLE VALUE. REVENUE IS REDUCED FOR ANTICIPATED DISCOUNTS UNDER CONTRACTUAL ARRANGEMENTS AND FOR CHARITY CARE. AN ESTIMATED PROVISION FOR BAD DEBTS IS RECORDED IN THE PERIOD THE RELATED SERVICES ARE PROVIDED BASED UPON ANTICIPATED UNCOMPENSATED CARE, AND IS ADJUSTED AS ADDITIONAL INFORMATION BECOMES AVAILABLE. THE PROVISION FOR BAD DEBTS IS BASED UPON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS CONSIDERING HISTORICAL BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN HEALTHCARE COVERAGE, AND OTHER COLLECTION INDICATORS. PERIODICALLY THROUGHOUT THE YEAR, MANAGEMENT ASSESSES THE ADEQUACY OF THE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS BASED UPON HISTORICAL WRITE-OFF EXPERIENCE BY PAYOR CATEGORY. THE RESULTS OF THIS REVIEW ARE THEN USED TO MAKE MODIFICATIONS TO THE PROVISION FOR BAD DEBTS AND TO ESTABLISH AN ALLOWANCE FOR UNCOLLECTIBLE RECEIVABLES. AFTER COLLECTION OF AMOUNTS DUE FROM INSURERS, THE CORPORATION FOLLOWS INTERNAL GUIDELINES FOR PLACING CERTAIN PAST DUE BALANCES WITH COLLECTION AGENCIES. FOR RECEIVABLES ASSOCIATED WITH SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY COVERAGE, THE CORPORATION ANALYZES CONTRACTUALLY DUE AMOUNTS AND PROVIDES AN ALLOWANCE FOR BAD DEBTS, ALLOWANCE FOR CONTRACTUAL ADJUSTMENTS, PROVISION FOR BAD DEBTS, AND CONTRACTUAL ADJUSTMENTS ON ACCOUNTS FOR WHICH THE THIRD-PARTY PAYOR HAS NOT YET PAID OR FOR PAYORS WHO ARE KNOWN TO BE HAVING FINANCIAL DIFFICULTIES THAT MAKE THE REALIZATION OF AMOUNTS DUE UNLIKELY. FOR RECEIVABLES ASSOCIATED WITH SELF-PAY PATIENTS OR WITH BALANCES REMAINING AFTER THE THIRD-PARTY COVERAGE HAD ALREADY PAID, THE CORPORATION RECORDS A SIGNIFICANT PROVISION FOR BAD DEBTS IN THE PERIOD OF SERVICE ON THE BASIS OF ITS HISTORICAL COLLECTIONS, WHICH INDICATES THAT MANY PATIENTS ULTIMATELY DO NOT PAY THE PORTION OF THEIR BILL FOR WHICH THEY ARE FINANCIALLY RESPONSIBLE. THE DIFFERENCE BETWEEN THE DISCOUNTED RATES AND THE AMOUNTS COLLECTED AFTER ALL REASONABLE COLLECTION EFFORTS HAVE BEEN EXHAUSTED IS CHARGED AGAINST THE ALLOWANCE FOR DOUBTFUL ACCOUNTS.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsTHE ORGANIZATION FILES ANNUALLY A COMMUNITY BENEFIT REPORT WITH THE STATE OF MARYLAND'S HEALTH SERVICES COST REVIEW COMMISSION (HSCRC). THE HSCRC, WHICH OPERATES UNDER A MEDICARE WAIVER, DOES NOT CONSIDER MEDICARE SHORTFALL AS COMMUNITY BENEFIT. THE COSTING METHODOLOGY USED BY THE ORGANIZATION IS A COST-TO-CHARGE RATIO.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceTHE ORGANIZATION EXPECTS PAYMENT AT THE TIME THE SERVICE IS PROVIDED. OUR POLICY IS TO COMPLY WITH ALL STATE AND FEDERAL LAW AND THIRD PARTY REGULATIONS AND TO PERFORM ALL CREDIT AND COLLECTION FUNCTIONS IN A DIGNIFIED AND RESPECTFUL MANNER. EMERGENCY SERVICES WILL BE PROVIDED TO ALL PATIENTS REGARDLESS OF ABILITY TO PAY. FINANCIAL ASSISTANCE IS AVAILABLE FOR PATIENTS BASED ON FINANCIAL NEED AS DEFINED IN THE FINANCIAL ASSISTANCE POLICY. THE ORGANIZATION DOES NOT DISCRIMINATE ON THE BASIS OF AGE, RACE, CREED, SEX OR ABILITY TO PAY. PATIENTS WHO ARE UNABLE TO PAY MAY REQUEST A FINANCIAL ASSISTANCE APPLICATION AT ANY TIME PRIOR TO SERVICE OR DURING THE BILLING AND COLLECTION PROCESS, EVEN IN EXCESS OF 240 DAYS FOLLOWING THE FIRST POST-DISCHARGE BILLING STATEMENT. THE ORGANIZATION MAY REQUEST THE PATIENT TO APPLY FOR MEDICAL ASSISTANCE PRIOR TO APPLYING FOR FINANCIAL ASSISTANCE. THE ACCOUNT WILL NOT BE FORWARDED FOR COLLECTION DURING THE MEDICAL ASSISTANCE APPLICATION PROCESS OR THE FINANCIAL ASSISTANCE APPLICATION PROCESS. NO EXTRAORDINARY COLLECTION ACTIONS (ECAS) WILL OCCUR EARLIER THAN 120 DAYS FROM SUBMISSION OF FIRST BILL TO THE PATIENT AND WILL BE PRECEDED BY NOTICE 30 DAYS PRIOR TO COMMENCEMENT OF THE ACTION. AVAILABILITY OF FINANCIAL ASSISTANCE WILL BE COMMUNICATED TO THE PATIENT AND A PRESUMPTIVE ELIGIBILITY REVIEW WILL OCCUR PRIOR TO ANY ACTION BEING TAKEN. IF A PATIENT IS DETERMINED TO BE ELIGIBLE FOR FINANCIAL ASSISTANCE AFTER AN ECA IS INITIATED, THE ORGANIZATION WILL TAKE REASONABLE MEASURES TO REVERSE THE ECAS AGAINST THE PATIENT ACCOUNT.

Schedule H, Part VI, Line 2 Needs assessmentIn the early planning stages, the Core Team worked together to establish shared goals and expectations for how data would be collected and what parts the assessment would include. Looking back at the 2022 CHNA, the team wanted more primary and secondary data to understand what helps or prevents residents from getting the care they need. To ensure Prince George's County community's perspectives and perceptions were included in the CHNA, the Core Team continued using the Mobilizing for Action through Planning and Partnerships (MAPP) framework from the National Association of Counties and City Health Officials. The framework was beneficial during the 2022 CHNA cycle, and the team wanted to use more from the updated MAPP 2.0 version for 2025. Although the complete three-phase MAPP process usually takes two years to implement, the Core Team adjusted the framework to fit the timeline needed to complete the CHNA. The CHNA focuses on Phase 2 of MAPP: Tell the Community Story, which includes three assessments. * Community Partner Assessment- Assesses community partner's organizational capacity to address health inequities. * Community Status Assessment- Measures the community's health status using a survey, existing secondary quantitative data, and observational data. * Community Context Assessment- Measures the community's health status using a survey, existing secondary quantitative data, and observational data. The results of the assessments were reviewed to identify the County's leading health priorities. After analyzing the data sources to understand the County's health needs, UM Capital worked collaboratively with a team of key interdisciplinary hospital leaders to further identify and prioritize the health priority areas for UM Capital. The team included a mix of Community Health leadership, executives, clinicians, and programmatic leadership from our key service areas, including Cancer, Diabetes, Trauma, Sickle Cell, & Women's Health. The team reviewed the CHNA data sources and health priorities and reviewed current hospital strategic priorities and internal investments.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceOur financial assistance policy and the communication about our financial assistance policy is regularly reviewed to make sure it is available to our patients in a variety of formats and that it is available in a culturally/linguistically sensitive manner and at a reading comprehension level appropriate to the population of our Community Benefit Service Area. The availability of financial assistance from the hospital for patients is communicated to patients in multiple ways: Large signs are posted at all our points of registration in the hospital (general registration, Emergency Department) and our specialized service areas (Perinatal Center, Cancer Institute, etc.). The phone number for our financial counselors is included. When patients are registering in the hospital for inpatient treatment or outpatient treatment, they are given the Patient Financial Information Sheet that is printed on two sides in English and Spanish. This Patient Financial Information Sheet is available at every point of entry to the hospital and at every point of service delivery. It is also included in the patient information packet given to each patient as well as online. Inpatients who do not have any health insurance are visited by one of our financial counselors. The counselors discuss the availability of various government benefits such as Medicaid or state programs that offer health care assistance. The counselors assist patients with the application. The financial counselor sends information about potential eligibility for various government benefits such as Medicaid or state programs offering health care assistance to outpatients who do not have any health insurance. These patients are invited to call to discuss applying for these programs. Spanish and English-speaking financial counselors are available. When a patient applies for financial assistance, our bilingual financial assistance counselor works with the patient to gather appropriate documents and submit their application for financial assistance. Given the levels of certain populations in our county, our Financial Assistance Policy is available in the following languages: Arabic, Chinese, English, French, German, Gujarati, Haitian Creole, Haiti, Hindi, Igbo, Indic, Korean, Oromo, Spanish, Swahili, Tagalog, Urdu, Vietnamese, Yoruba.

Information needed to complete the return is gathered by accounting personnel in the Finance Shared Services department under the supervision of the UMMS Tax Director. Draft returns are prepared using IRS-approved tax software. Once a draft return is prepared, it undergoes multiple levels of review both internally by UMMS tax & finance personnel, and externally by Ernst & Young LLP. Following any necessary changes to the return, a final draft is reviewed by each affiliate's vice president of finance and/or CFO. Prior to filing the IRS Form 990, the organization's board chairman, treasurer, governance committee, finance committee or other member(s) of the board with similar authority will review the IRS Form 990. All board members are provided with a copy of the final IRS Form 990 before filing.
